Today s Healthcare landscape Worldwide sales of counterfeit drugs is in excess of $75 billion annually. World Health Organisation: eit/overview/en/ By the end of , the NHS has to realise 1.5bn of procurement efficiencies in England. Better Procurement Better Value Better Care report, 2013 It is completely unacceptable that half of the world still lacks coverage for the most essential health services. Tedros Adhanonom Ghebreysus, Director General WHO Medical error the third leading cause of death in the US BMJ 2016, Prof. Martin A Makary, Michael Daniel If we project into the future, we have to realise that our current healthcare system is going to break under pressure. In the US, healthcare today is 17.4% of GDP, rising to 25% by 2040, so we might find ourselves with rationed healthcare. Peter Carter, CEO, ISQua 3
Many developments across the world Regulatory bodies need to address Public Health Hospitals look for improvement in patient care and cost reduction 7
Healthcare in a changing environment The patient in the center of action and activities Quality of care missing staff, medication errors A complex and not always safe supply chain Active and informed patients Influence of the digital world Payment of care - measuring health outcomes against the cost of delivering the outcomes. (VBH) Need to capturing more exactly the costs/data during treatments More partnership and collaboration 5
McKinsey & Company report quantifies supply chain issues in Healthcare McKinsey report Strength in unity: The promise of global standards in healthcare Highlights the cost savings and patient safety benefits of adopting a single global supply chain standard in healthcare Available at: Source: 6
Huge cost savings and patient safety benefits when adopting a single global standard in healthcare Implementing global standards across the entire healthcare supply chain could save 22,000-43,000 lives and avert 0.7 million to 1.4 million patient disabilities Rolling out such standards-based systems globally could prevent tens of billions of dollars worth of counterfeit drugs from entering the legitimate supply chain [We] estimate that healthcare cost could be reduced by $40 billion-$100 billion globally from the implementation of global standards Adopting a single set of global standards will cost significantly less than two (between 10-25% less cost to stakeholders) SOURCE: McKinsey report, Strength in unity: The promise of global standards in healthcare, October
Global supply chain standards - GS1 1 million 150 countries 6 billion 112 MOs over 1 million companies worldwide use GS1 standards 25 industries served across 150 countries Barcodes scanned more than 6 billion times per day globally 112 Member Organisations around the world 8
Why standards? 9
Lack of standards Multiple bar codes on one package which one to scan? Different types of bar codes inconsistency; incompatibility No bar code need to bar code; re-package; re-label 1 0
Harmonisation is critical Diverging country requirements Manufacturing headache Customised action means cost & complexity!
GS1 Healthcare a voluntary, global Healthcare User Group To lead the healthcare sector to the successful development and implementation of global standards by bringing together experts in healthcare to enhance patient safety and supply chain efficiencies.
GS1 Healthcare - an expanding, committed community of globally engaged stakeholders much more locally and there are many more companies working with GS1 at a local level 1 3
Leading hospitals - implementing GS1 1 4
15
GS1: global system of standards GS1 standards make it possible to identify, capture and share information automatically and accurately about products, business locations and more
GS1 DataMatrix: THE data carrier for pharmaceuticals Product Identifier (GTIN) Serial Number Expiry date Batch/Lot number 17
Standards can support: key hospital support processes Product recalls Inventory management Asset management Patient-level costing Order-to-cash management Catalogue management
Support for clinical processes Bedside scanning patient, caregiver, product scanning and verification Electronic Health Records (EHR) patients records management Implant registry Instrument management and tracking (e.g., surgical instrument tracking, loanset tracking, etc.) Product recalls Regulatory compliance Traceability Preparation of patient specific infusion solutions (e.g. cytostatics) Supporting clinical pathways
Support for administrative processes Procurement, delivery of goods in central warehouse, stock keeping Enterprise resource program, interoperability Backoffice administrative procedures Traceability Traceability Supporting procurement Management processes Quality control Administrative procedures at the bedside Supporting clinical pathways Patient wristband Electronic patient record
Barcoding can reduce errors 21
We know that standards could help significantly: UDI 2010 Severe patient harm Traceability issues Hospitals face major costs and litigation Manufacturer bankrupt Focus on patient safety Need for traceability Damaged breast implant 22
Barcodes improving supply chain efficiency and patient safety 23
a world where 2 4
the patient rights were reality, all the time! right caregiver right patient right route right time right medication right dose right medical device 2 5
dispensing and administration errors are a thing of the past I still remember when dispensing errors due to typos, illegible handwriting or oversights was a risk we all had to live with. It s good those days are over! A future where 1 : Computerised physician order entry (CPOE) has replaced hand-written prescriptions Barcode scanning procedures cut current potential adverse drug events by up to 75% There is a general >50% reduction of current medication errors saving lives, cost and time for better patient care 1 McKinsey report, Strength in unity: The promise of global standards in healthcare 2 6
where counterfeit medicines never reach a patient We never worry anymore that the drugs taken by your father could be counterfeit, endangering his life. That is a huge relief! A future where 1 : Counterfeit drugs rates drop from 5-10% globally and up to 30% in developing countries 1 McKinsey report, Strength in unity: The promise of global standards in healthcare 2 7
where recalls are easily managed We still can t avoid recalls of medical devices, but now we can easily identify which patients have these and can contact them. We know we have a 100% success rate. A future where 1 : Due to easily identified specific batch numbers clinical staff can spend less time on recalls and more on patients, improving care Patient safety is improved by being able to pinpoint affected products and patients more quickly There are huge costs savings due to efficient and selective inventory management 1 McKinsey report, Strength in unity: The promise of global standards in healthcare 2 8
where clinical decision making and time for patient care is better than ever The new technology available to positively identify patients as part of every single care giving process. This means that I don t have to worry about the wrong care being given to the wrong patient. A future where 1 : A global data standard can substantially reduce medication errors and supports improved clinical decision making Product ID scanning and point of care barcode scanning not only delivers increased patient safety, but also frees up time to care for patients 1 McKinsey report, Strength in unity: The promise of global standards in healthcare 2 9
Scan for safety the path forward Better inventory management and reduction of product waste: by scanning GS1 barcodes, supply chains will be made simpler, helping to reduce the estimated 150m the NHS wastes each year 1 NHS eprocurement Strategy, m estimated reduction in waste each year In the Scan4Safety programme, launched in 2016, six trusts were given 12m to introduce the scanning of every person, every product and every place into their hospitals, using GS1 barcodes. In 2017 already they over-delivered by 70% in estimated financial savings. The Department of Health predicts that future savings could exceed 1bn if the approach is adopted by all 148 remaining trusts 2 NHE Sept/Oct
Real-life examples: Improvement of processes Centro Medico Imbanaco de Cali S.A. implemented identification on pharmaceutical items, medical supplies, stationery and office supplies as well as for patient wristbands, locations, documents, bills, printed medical records, prescriptions, replenishment orders, trays and surgical devices, resulting in: 25% reduction in inventory levels 25% reduction in inventory levels at satellite pharmacies. 98% improvement in inventory shrinkage/waste. 55% improvement in invoice creation time required. Reducing medication errors. 31
Real-life examples: Improving quality and efficiency Fundación Valle di Lili implemented identification on pharmaceutical items, medical supplies, patients and locations resulting in: 25% reduction in inventory levels Validation of patient rights Identification of batches of medicines and supplies administered and used in patients. Control expiration Facilitation of recalls Traceability in the value chain of medicines and supplies Facilitation of physical inventory Allowing more time for patients Accreditation Watch more at f4xo&feature=youtu.be 32
Real-life example: Improving patient safety Hospital Israelita Albert Einstein, São Paulo, Brazil implemented GS1 DataMatrix for all drugs in their hospital ensuring traceability from manufacturer to patient resulting in: Ensuring the 7 or 9 patient rights Electronic bedside check of medication administration, according to the order and ensuring control over 7 of 9 rights proposed Agility in localizing products banned for recall. Important tool for obtaining quality certifications And much more 33
Real-life examples: Supply chain management Centre Hospitalier Universitaire de Dijon (France) 1 created a logistics platform for its warehouse that includes a traceability system using GS1 standards (GLN, GRAI, SSCC, GTIN). 60% reduction of stocks Resulting in: 40% increase in productivity 30% gain in storage capacity 80% reduction of item obsolescence 60% reduction of stocks within the hospital stock 1 GS1 Reference Book
Patients are in the center of our attention standards can improve the care for them GS1 UK Healthcare video November 2017
Summarising Barcoding technology and global standards are valuable tools to improve patient safety, fight counterfeits and increase visibility and efficiency in the supply chain We need global standards - a harmonized approach will be less complex and costly Do not wait further 36
Yes, we (s)can! 37
