The position leads a team of Disability Support Workers (DSWs) by promoting person-centred active support to facilitate community inclusion for clients in one of the following programs: adult respite; children s respite; supported and independent living accommodation, individualised support packages and carers support. This includes promotion of client participation and empowerment in all relevant program activities, participation in personal and decision-making, dignity and respect, relationship building and skill development to promote full community inclusion. Key Result Area (KRA) Key Accountabilities and Responsibilities Accountable = The buck stops here Responsible = The doer Service Delivery Establish and maintain a supportive environment for people according to the principles of inclusion and person centred active support Promote and encourage maximum community integration through person-centred participation in all aspects of a person s life Support and facilitate client bookings, intake and assessment. Promote, encourage and empower clients to maximise personal decision-making opportunities Maintain effective interpersonal communication with staff and clients and liaise with relevant agencies/community resources, colleagues and families to ensure continuity of care and support for clients, ensuring program goals are met. Commit to best practice principles by identifying and contributing to projects which develop and review policies and practices to enhance service delivery and budget effectiveness. Implement Continuous Improvement in all aspects of service delivery by identifying risks and areas for improvement via regular audits and developing evaluation and action plans. Support and empower vulnerable people to report and speak out against instances of abuse or potential abuse. Undertake regular monitoring and maintenance of service provision data including client, program and organisational records in accordance with all Jewish Care policies, funding agreements, legal and statutory requirements. Participate and contribute to occupational health and safety activities to ensure a secure and supportive work environment for self and others; Cooperate in processes that improve health and safety by using relevant tools, systems and policies. Supervise volunteers in line with Jewish Care volunteer policies and protocols. Monitor all program expenditure and deliver all services within defined budgets. Utilise information technology competently. Together with the clients and other staff, develop, implement, monitor and evaluate meaningful individual program plans, activities and outings Promote and facilitate full community integration through the application of personcentred active support principles. Facilitate and support the development and maintenance of consumer support networks through outreach to family, community members and professionals. Assist clients with the provision of appropriate personal support (including provision of medication where necessary) and directly provide that support or skills training where necessary. The duties required may be amended periodically to reflect the needs of the residents in the program Manage compliance and billing in line with NDIS requirements. Leadership Manage programs providing direct care services to clients with regular liaison with associated services according to the principles of relevant legislation. Co-ordinate, supervise and support program staff to ensure high quality personcentred program delivery based on the assessed needs and desires of individual clients. Participate in the recruitment, orientation, in-service training and appraisal of staff. Ensure that the established processes for the secure management and storage of household/client funds, medication and service records are maintained.
