Explanation of Payphone Origination A per call charge applies to each call originating from a payphone and terminating to one of TelePacific's Inbound Toll Free numbers or billed to a TelePacific calling card. This charge applies in addition to per minute usage charges. Such calls are identified by a pound sign (#) on your invoice. Official Messages Explanation of Deniable/ Non Deniable s For California The total amount of your invoice may include both basic telephone service charges and non basic charges. Failure to pay basic charges may result in disconnection of your telephone service for nonpayment. Non basic charges are those charges which are indicated by an asterisk (*) on your invoice. Failure to pay non basic charges appearing on your invoice will not result in disconnection of your telephone service but may result in collection action. You are responsible for payment of all charges on your invoice. This bill is now due and payable; it becomes subject to a late payment charge if not paid by the date shown on your bill. Should you question the bill, please request an explanation from the Company. If you believe that you have been billed incorrectly, you may file a complaint with the California Public Utilities Commission, Consumer Affairs Branch, 505 Van Ness Avenue, San Francisco, CA 9402. To avoid having service disconnected, payment of the disputed bill should be made 'under protest' to the CPUC or payment arrangements acceptable to the Company should be made pending the outcome of the Consumer Affairs Branch Review. The Consumer Affairs Branch shall review the basis of the billed amount, communicate the results of its review to the parties and inform you of your recourse to pursue this matter further with the Commission. For Nevada If you believe you have been billed incorrectly, please contact TelePacific. The Company will investigate the matter and report the results of its investigation to you. You may request a written report. If you are not satisfied with the Company's resolution of your dispute, you may file a complaint with the Public Commission of Nevada, Consumer Division, 50 E. William Street, Carson City, Nevada 8070, telephone number 800 922 0900 (toll free). You may be required to pay any disputed amount to TelePacific pending resolution of the complaint. TelePacific will refund any paid amount that the Commission finds to have been improperly charged.
