WTAMU Graduate Field Evaluation (Foundation) Student: Agency: Field Supervisor: Evaluation for: Mid-term or Final (circle one) Instructions for Rating Interns on the Competencies in the First Part of the Evaluation: The competencies specified in this evaluation form are approved the Council on Social Work Education. Under each competency statement are several items that we ask that you rate according to the following criteria. Rating Scale for Evaluation of Student Performance CATEGORY DESCRIPTION RATING Unsatisfactory 1 Marginal Developing Competent Exemplary The student is performing below the expected level of performance and shows little evidence of understanding the concept and/or demonstrating the skill. The student is able to perform some tasks & understands some social work concepts, but lacks overall comprehension. Can apply some skills but is not consistent. The student performs tasks & understands social work concepts at the expected level. The student understands the concept & applies the skills with consistency. The student has suitable or sufficient skill, knowledge, experience, etc.; properly qualified (compared to other students) The student performs at a level above what would be expected for a social work student at this educational level. He/she displays an above average ability to understand social work concepts & demonstrate social work skills. (in comparison with other social work professionals) 2 3 4 5 Comments may be made under any competency statement, if desired. Please be sure to indicate those areas in which you think the intern is particularly strong and those areas in which the student need improvement. This evaluation is intended to give the intern feedback about her or his performance. The field instructor s rating of these items will not directly be used to calculate the grade given to the intern. The field coordinator has responsibility of assigning the grade for field instruction. The grade that is assigned will be based on: the faculty s overall evaluation of the student s performance in field placement in conjunction with the field instructor s evaluation and other submitted materials such as seminar participation and performance on assignments. Competency #1: Intern identifies as a professional social worker and conducts himself/herself accordingly. 1.1 Advocates for client access to the services of social work 1.2 Practices personal reflections and self-correction to assure continued professional development 1.3 Attends to professional roles and boundaries 1.4 Demonstrates professional demeanor in behavior, appearance and communication 1.5 Engages in career-long learning (i.e. staff training, conferences, workshops) that has helped to establish a pattern of career-long learning 1.6 Uses supervision and consultation effectively. Competency #2: Intern can apply social work ethical principles to guide his or her professional practice. 2.1 Recognizes and manages values in a way that allows professional values to guide practice 2.2 Makes ethical decisions by applying the NASW code of ethics and, as applicable, of the International Federation of Social Workers/International Association of Schools of Social Work Ethics in Social Work, Statement of Principles 2.3 Tolerates ambiguity in resolving ethical conflicts 2.4 Applies strategies of ethical reasoning to arrive at principled decisions Competency #3: Intern applies critical thinking to inform and communicate professional judgments. 3.1 Is able to distinguish, appraise, and integrate multiple sources of knowledge, including research-based knowledge, and practice wisdom to practice 3.2 Analyzes models of assessment, prevention, intervention, and evaluation 3.3 Demonstrates effective oral communication and written communication in working with individuals, families, groups, organizations, communities, and colleagues WTAMU Social Work Program Graduate Field Evaluation 2 P a g e
