It is the responsibility of importers to inform exporters of the existence of the present procedure. Verifications are executed at origin on seller or exporter s request; based on Verification Order applied by the importers in Indonesia. 1. IMPORT VERIFICATION PROGRAM (IVP) FOR NON-B3 WASTE TO INDONESIA PROGRAM MANDATED BY The Ministry of Trade of the Republic of Indonesia APPLICABLE REGUALTION(S) The Regulation of Minister of Trade of the Republic of Indonesia. 31/M- DAG/PER/5/2016 dated 09 May 2016. STARTING DATE For Verification Orders (VO) issued from 17 th Jul 2012 APPOINTED SURVEYOR SCOPE OF INDONESIA IVP COTECNA on behalf of KSO SCISI (refer Section 7 for countries covered) - Conformity to Quantity and Specification. - Import eligibility checks - Recording the related shipping data and information - Goods are clean and dry and free from toxic materials and/or impurities. - Country of Origin 2. (2) Exporter should give minimum 3 working days notice for inspection arrangement and all relevant documents should be submitted, ie. completed RFI form and Packing List. (3) Goods presented for inspection must be as per goods stated in the VO. (4) Country of origin and supply should also be the same as per VO. (5) Inspected quantities must be within the quantity stated as per VO. Otherwise, VO quantity must be amended or new IO presented before inspection. (6) Exporter must ensure that goods arrival date in Indonesia is within the VO validity date. Page 1 / 5

To seller: Security Label, if required by the seller To KSO SCISI: Draft Laporan Survey (LS) For KSO SCISI to issue Final LS to importer for Customs clearance. (1) Exporter s Statement Letter (ESL) is a required document to be submitted before issuance of the LS. This must be prepared with Exporter s letterhead. The template is available from Cotecna offices. (2) 1 LS will be issued for 1 inspection done at 1 location. (3) Exporter should submit the Final Documents as soon as available to facilitate issuance of Report of Findings. 5. INSPECTION FEES Verification fees are paid in advance by importers to KSO SCISI at the time of VO application. Nevertheless, Cotecna may invoice the seller in the event of supplementary inspection visits (in vain or unsatisfactory results), inspections beyond normal hours and/or on non-working days. The costs incurred by the seller for presenting the goods for inspection, such as unpacking, handling, repackaging are for the account of the seller. In case if exporter/importer request for additional commercial scope of intervention, Cotecna also reserves the right to charge for these additional scopes.
