What are the hours we can work? 12.) Is this a Union or a non-union job? 13.) Is all the replacement or relocation of low voltage wiring in the Villa basement part of this scope of work? 4/19/2016 4/25/2016 4/26/2016 1.) No, but all sealed bids will be open at the same time and documentation will record each bid as they are opened. 2.) No, that work is being handled separate from this project. 3.) Page: 3 of 5 PB-014 PB #014 1-On drawing # 4-400.00 d/d 4/5/16 ---Detail # 6 calls for the following items. Please provide additional details and information: A-Temp steel supports are shown-- what size, how much weight are they supporting, what is the on center spacing, and where do they go exactly? B-There calls for prefabricated housing vertical above grade who makes this, what is the material, are there more specific details and how many locations? I am not certain looking at the documents. C-Is all the work in the existing tunnel OK to be performed on regular time hours, 7AM till 4 PM? If not please define the hours and times we can work. D-How long is the existing tunnel? Do we need new tunnel construction drawings? I am not certain whether a new tunnel length is needed; or what is that length and connection details? E-How does the new manufactured housing tie into the new curb and tunnel construction I assume we are cutting the top of the existing tunnel off; and the housing is the new protection? How do these all tie in and are supported? F- Does the tunnel work stay as shown or until the demo contract is completed? We are not restoring or completing the existing tunnel on this contract. Please confirm all work is TEMP and stays in place. 1A. The provision of temporary steel posts here is to secure the top of the tunnel along the CMU curb during construction and this support is considered means and methods to be developed by the awarded bidder. 1B. This is only indicated in a single location. See Keynote F4 on sheet DM-303 for location. Basis of design is indicated on view 3/ M001. 1C. Refer to schedule 1/ DM-805 for the initial survey of electrical related services housed within the existing tunnel. Contractor is to confirm active versus inactive, and Services that are active or have been identified to be re-fed are to be scheduled with RUMC and contractor should assume Off-Hours for these services only. For the identified and confirmed abandoned services these are to be removed during regular hours. 1D. The existing tunnel runs from the Fitzpatrick building to the sub basement of the main hospital which makes it approximately 60'-70'. The demolition of the actual tunnel and potential new construction of a new tunnel is NOT to be considered within the scope of this project. 1E. The piping above ground should be supported via stanchions or rails coordinated the with temporary steam pressure reducing station. The piping within the tunnel should be supported either via hangers from the roof of the tunnel or from wall supports. All of these supports are means and methods and should be decided by the contractor in the field. 1F. All temporary work will remain in place for the extent of this early demolition project as well as during the construction of the new Emergency Department. Removal of the temporary services is NOT considered within the scope of this project.

Page: 4 of 5 PB-015 PB-#015 1. Provide, a project directory noting the design team disciplines, the client, the people who attended the pre-bid meeting and walk through. 2. Provide more detail on the Villa Building demo repair on where the demo meets the Mansion so we can price accordingly. 3. Confirm the bid was extended to May 4th at 3PM. Is it possible to push the bid due date to May 6th, at 3PM. We just received addendums #1 & #2 last week; and per the walk through other info will be provided, please confirm. 4. VERY IMPORTANT-The main thing to be sent out to all bidders was a confirming project schedule with proper key milestones and durations. This is very important because the dates noted are extremely tight and we must have the project schedule to bid properly. 5. Please confirm a 10% Bid Bond is required with Bidder's submissions. Please confirm Performance & Payment Bonds will be required from the successful bidder once awarded? (this cost must be in our bid price when submitting). 6. Confirm, all work including demo & shut downs will be priced on regular time hours for this project? If not please detail the O/T required and for what scope of work? 7. A copy of S already submitted for the project are in the folder. Please provide the final s numbers dating from 4/22/16. 8. Confirm the client abatement program starts on August 23rd 2016, and actual demo and GC work is scheduled to start September 9th 2016. This can be covered in the overall project schedule to be submitted. (These are key dates for current s and bidding). 9. If a new foundation is required for the Mansion where the Villa building is being demoed due to possible rubble foundations will it will be addressed after the bid award? Is it correct that we should exclude new foundation work in this bid? 10. Please provide additional information for the new generator so that we may bid properly. Please advise as the scope it not clear. 11. Please let me know if we are to copy any other team members on requests? (Never received direction on this procedure as promised. Please forward if needed and copy me). 12. Parking on site is not available. It was mentioned there would be offsite parking and transportation for our manpower to get to the site. If this is the case, how far away is offsite parking, will there be any cost for this service? Also, please confirm site deliveries and rubbish pick-up can be made from the hours 1 AM till 4 PM? 1. For a project directory refer to section 011000 Summary within the project manual. A scanned in attendee list is included on the Box Account link within the folder labeled Pre-Bid Meeting Materials. 2. Detail is included for repair at existing wall penetrations, and there is detail for the temporary partition that is to be constructed at the existing corridor that connects Garner Mansion with the Villa Building. Refer to demolition plans in the DM-300 series and architectural details on A-400 for additional information. 3. Bid due date has been extended to May 6th at 3PM. Addendum 3 was posted to the public link as of 4/21/2016, and responses to Pre-Bid Meeting s were posted in the public link as of 4/25/2016. 4. Intended project schedule was discussed during the Pre-Bid Meeting and dates will need to shift with the Bid Due Date as follows: May 6, 2016: Bid Due Date, 3PM May 20, 2016: Bid Awarded May 20-June 7: Awarded Bidder coordinate with Bohler Engineers to Finalize DOB Submission August 31: Anticipated DOB Approval Aug. 15-Sept. 9: Abatement Scope of Work (Not In Contract) Sept. 9-Nov. 18: Make Ready, Fitzpatrick Demolition, Villa Demolition Scope of Work 2018, 2nd Quarter: EMS Demolition Scope of Work to Occur, duration to be proposed by Bidders 5. Follow specifications. 6. Price shut downs to occur during off hours. Some may be able to occur during regular hours, but bids should assume O/T. 7. tal s issued to CannonDesign through April 27th is 16, note that all of the Pre-Bid Meeting questions were recapped as 11. 8. Refer to response to question 4. 9. Contractor shall ensure safety and maintain structural soundness of the adjacent building during construction. There is no new foundation required otherwise. 10. Information was include within 11 response. 11. Per form within the project manual, ya Ash of CannonDesign should have been issued the, and distribution would occur to owner and Bohler as required for input. 12. For bidding purposes, assume that Owner would provide area off site for parking, Contractor would provide shuttle.

Page: 5 of 5 PB-016 PB #016 1. Addendum #2 DWG #X-001.00 d/d 5/13 is to be worked in with the MEP drawings specifically DWG #M-002.00 d/d 4/5/16. It is our understanding certain details are superseded from M-002 by Addendum DG X-001. The route for running the new TEMP services for steam and electric runs through the main roadway, and ER & Ambulance's main road. What is the plan and direction for us to conduct this work. Roadways may be workable; the ambulance ER entrance is not clear or workable without shutting down for a duration of time. What is the hospital's overall logistic plan for access and relocation? We need the guidelines so we can bid properly and also develop our logistic process for construction and demo work. Right now we do not see how the work can be completed while the ER is running and ambulances are moving throughout. Also, the sidewalk bridge cannot be erected because we need to excavate, install and fill the bridge would be in the way. Solution bridge goes up after new temp work, does not work overall with the projected schedule. 2. Please advise do we cover with all steel plates; or after specified back-fill process do we cover with temp asphalt? 3. Does the sidewalk bridge stay after our demo contract is completed? Please advise. We are renting. Does rental cost turnover to RU? Please advise because we are just costing for the duration of our work and plan on removing it as soon as the building is low enough to avoid pedestrian risk please. 4. The info on a notes we should carry one R & R of the TEMP generator? Is the R & R within the demo contract time frame or expected later down the road? Should it be alternate price? NEW 4/27/2016 4/29/2016 5/3/2016 1. Follow M002 for suggested temp utility route. Intent of pathway shown on M-002 is to avoid ambulance traffic as well as new E.D. retaining walls and building construction. Sidewalk Bridge to be installed following temporary trench work. 2. Bidders to price asphalt at temporary trench. 3. Remove sidewalk at suggested time once demolished building is low enough. 4. This allowance to relocate once is reserved to occur during this project. 5. Site element demolition is not in contract and will occur as part of a separate scope of work. 6. Bohler to respond. 7. Refer to Detail 3/M001 for housing to be basis of design. 8. All MEP services within EMS cottage are to be cut and capped and do not affect remainder of campus services. 9. Stained glass will be returned to Owner. 10. Price shut downs to occur during off-hours. 11. Extension to 5/6/2016 Bid Date has been approved. Refer to 15 Question 4 for milestone dates. 5. Do the existing parking lots, sidewalks, curbs ETC within the construction fence foot print off of Fitzpatrick & Villa get demolished and removed, it is not clear? Please advise there is 24x 50 space calling for 8 of gravel and the "payment(pavement?)" below is demolished? (We plan on using this for our truck was stations and deliveries). 6. Should we assume we can use fire hydrant water for dust control? Please advise that will be a RU cost, and we will run hoses and pipe as needed and overhead for roadways in our prices. 7. Drawing M-002 note 2, calls for a lockable waterproof enclosure is there a specified product or is it to be made of plywood? What size is needed? Provide more detail and specs on this. Please advise whether you need stand up sheds for service? 8. We do not see MEP demo info for the EMS cottage? Is this a self-contained building for MEP S? Please advise. 9. We are planning on saving the 2 stained glass units (with professional stained glass companies). Where is it to be delivered? Does it need to be crated? We cannot guarantee removal in one piece at this time? TBD after award, other logistics and "how." 10. What is the shut off plan for MEP R & R work for the main building / Fitzpatrick & Villa? Are they on REG time or off hours? Please advise. 11. A client / project schedule was to be sent out 4/20 showing the dates and milestones. Please provide, the information is critical to our bid pricing. CAN BID TIME FRAME BE EXTENDED TILL 5/6 /16 Friday?
