CLASSIFICATION OF ENDOSCOPIC EQUIPMENT STERILIZATION DEFINITION According to the Spaulding classification, most flexible endoscopes used in gastrointestinal endoscopy are classified as semicritical devices, as they come into contact with mucous membranes. Dr.Earle Spaulding Microbiologist of Temple University (Philadelphia) in 1939 proposed "a strategy for sterilization/disinfection based on a classification of medical reusable devices. Nowadays this classifications is recognized in National and International Semi critical devices require disinfection, but sterilization is not necessary. Accessories which come into contact only with the skin and mucosa do not need to be sterile, but must undergo high level disinfection. Endoscopy accessories which penetrate the mucosal barrier (e. g.biopsy forceps, guide wires, polypectomy snares, injection needles, etc) are classified as critical devices and therefore must be sterile at the point of use. Single use devices should not be reprocessed. National regulations should be followed. Sterilization is the complete elimination or destruction of all bacterial spores. The term sterilization is intended to convey an absolute meaning, not a relative one. Sterility Assurance Level (SAL) = SAL of 10-6 (1: ) This means that there is less than one chance in a million that an item remains contaminated or non sterile. To mantein sterility devices must be packed in pouches, rolls, TNT tissues or containers. DISINFECTION DEFINITION TRAINING MANUAL ARES PLATFORM MICROBIOLOGICAL & ENDOSCOPE ARCHITECTURE BACKGROUND Disinfection describes a process that eliminates many microorganisms, with the exception of bacterial spores. HLD is achieved using liquid chemicals by immersion/bath procedures. Remember that even if a device has been disinfected with Peracetic Acid (Sporicidal action/liquid sterilant), the device it s not packed so, as soon as you take out the item from the liquid you ll loose the claim! Share questions about the topics discussed during the training session Clarify all the unmet needs or doubts 5) With the duodenoscope you are able to see? a) The intestine b) The first part of the small intestine c) All the above 6) The Elevator Channels belongs to? a) Bronchoscope b) Colonoscope c) Duodenoscope 4) Which is the cell shape of a COCCI? a) Rectangular b) Spherical 5) Which is the most important enzyme in a detergent used in a GI lab? a) Cellulase c) Protease d) Amylase 6) Complete the phrase selecting the appropriate word. High level disinfection kills all [xx1], except for some bacterial [xx2]. Many countries measure high level disinfection performance by its ability to destroy [xx3]. a) xx1 = micro organisms / viruses / spores b) xx2 = infections / organisms / spores c) xx3 = Methicillin- resistant Staphylococcus aureus / Mycobacterium tuberculosis / Clostridium 7) What is the function of the channel separator? a) Maximum contact between scope and fluid b) Guarantee the unidirectional channels flow irrigation c) Prevent contamination 8) Which of the following features would be found in the ED200 System (more than one)? a) Vertical storage b) HEPA filter c) Display LCD d) Single scope position control 9) The requirements for high- level disinfection using washer- disinfectors are specified in which ISO a) IS b) IS c) IS d) IS ) According to the European Medical Device Directive which is the detergents class? a) Class IIb 4
