CLASSIFICATION OF ENDOSCOPIC EQUIPMENT STERILIZATION DEFINITION According to the Spaulding classification, most flexible endoscopes used in gastrointestinal endoscopy are classified as semicritical devices, as they come into contact with mucous membranes. Dr.Earle Spaulding Microbiologist of Temple University (Philadelphia) in 1939 proposed "a strategy for sterilization/disinfection based on a classification of medical reusable devices. Nowadays this classifications is recognized in National and International Semi critical devices require disinfection, but sterilization is not necessary. Accessories which come into contact only with the skin and mucosa do not need to be sterile, but must undergo high level disinfection. Endoscopy accessories which penetrate the mucosal barrier (e. g.biopsy forceps, guide wires, polypectomy snares, injection needles, etc) are classified as critical devices and therefore must be sterile at the point of use. Single use devices should not be reprocessed. National regulations should be followed. Sterilization is the complete elimination or destruction of all bacterial spores. The term sterilization is intended to convey an absolute meaning, not a relative one. Sterility Assurance Level (SAL) = SAL of 10-6 (1: ) This means that there is less than one chance in a million that an item remains contaminated or non sterile. To mantein sterility devices must be packed in pouches, rolls, TNT tissues or containers. HLD is achieved using liquid chemicals by immersion/bath procedures. Remember that even if a device has been disinfected with Peracetic Acid (Sporicidal action/liquid sterilant), the device it s not packed so, as soon as you take out the item from the liquid you ll loose the claim! 7 Operator and technical courses how to get the best performance for your Steelco equipment Equipment user courses 1 day course Chemistries and process monitoring products 2 day courses These courses cover user training for new or previously purchased Steelco equipment and are designed for those that will be operating equipment. Courses can be held at either Steelco or customer premises. Optimal outcomes are achieved using the most appropriate consumables with equipment and monitoring processes. Day 1: analysis of which chemistries to use for thermal or chemical disinfection as part of manual and automated cleaning processes based on the type of load and soils Day 2: review of process-monitoring devices for use with washer disinfectors, ultrasonic units and sterilizers, based on relevant international standards. Technical training from 1 day up to 1 week course depending on the aim and complexity of the systems To obtain optimal performance over many years and meet international and local standards, maintaining equipment according to the supplier s recommendations is important.
