1 Form 470 Application # Infrastructure Upgrade Request for Proposals Be it known that the Manteno Community School District No 5 (herein called District ) with the main office located at 84 N Oak Street, Manteno, IL 60950, is soliciting proposals for Infrastructure Upgrade subject to the following terms. All proposals should be submitted in electronic format as PDF files. Delivery can be made via E mail*, CD ROM, USB Flash, etc. and must be received by the District Office no later than 2:00PM, January 2, 205. Upon notification of award, the successful vendor shall provide two (2) sets of paper hard copy of the proposal and must include all required supporting documentation, exhibits, addenda, assurances, specifications, etc. Incomplete proposals or late proposals will not be accepted. Per Illinois School Code, 05 ILCS 5/ (the Competitive Bidding Statute"), computers and related technologies are specifically set forth as items not subject to the competitive bidding. Submitted proposals are NOT considered sealed bids, but are legally binding upon the presenter. The District reserves the right to reject any and/or all proposals, to accept any proposal of its choice or to enter into additional negotiations regarding price, scope of services or other terms. The District does not obligate itself to accept the lowest proposal or any other proposal. The District reserves the right to waive any informalities in any or all proposals and to accept any one or a combination of alternative proposals in the interest of the District. No proposal may be withdrawn or modified within the 90 day period from the time of its presentation and opening. Absolutely no phone calls will be accepted. *E mailed proposals must NOT include any attachments in.zip format. Use of a read receipt is strongly encouraged to verify receipt. Special E rate Consideration: Manteno Community School District No 5 intends to use E Rate discounts for these services and plans to submit USAC SLD form 47 within the applicable filing window in

2 response to the RFP for funds based upon the pricing submitted by the successful vendor. The SLD funding cycle may take as long as 2 8 months. In the event funding is not available this RFP and any subsequently signed agreements will become void. Specifications and details for Infrastructure Upgrade This represents multiple projects. Pricing for each project must be broken out independently. The district reserves the right to move forward with any, all or none of the projects. However, only one vendor will be awarded a contract, regardless how many of the projects are actually pursued. While specifications list specific Cisco part numbers, we welcome equivalent solutions that are functionally equivalent or superior to the specified product. All product must be NEW and provided through authorized manufacturer channels (no refurbished or gray market ). Any equivalent product must be 00% compatible with what is specified to ensure functionality with the existing infrastructure. If providing an equivalent solution or equivalent parts, it is up to the vendor to include all parts, labor, licensing, etc. to ensure the solution is 00% compatible and to provide documentation explaining how the proposed solution is equivalent. The included line item bill of materials should be used for submitting pricing. For equivalent solutions, a comparable bill of materials with line item pricing should be used. The column should include a percentage reflecting the percentage of the line item that is part of the USAC ible Services list. It is anticipated most line items would be 00% but some may include equipment/service that is ineligible which should be reflected in that column. The District reserves the right to remove any line item(s) from consideration if it is deemed to be cost prohibitive or not cost effective. (This would only involve full line item sections i.e. item would include.0 through.9) Project : High School Core Switch Replacement This is a replacement for an existing Cisco WS 4506 switch. The scope of this project includes providing replacement gear as specified, labor to remove existing equipment and install, configure and test the new equipment. This will supplement or replace existing gear in each network closet, providing a 0Gbps path from the edge switch to the core. The scope of this project includes providing equipment as specified. The criteria below will be used to evaluate proposals. If scores are approximately equal, the incumbent vendor will be given preference. Category Criteria Weight / Reputation When all proposals have been received the district will make a determination of what Projects/ items to move forward with. The Total (TC) of those items will be summarized for each response. A Total Average (TAC) will be calculated by summarizing the total cost of all responses divided by the number of responses. Reference checks and similar tools indicate the vendor is reputable and able to complete projects on time. TC >= 20% less than TAC = 50 points TC >= 0% less than TAC = 40 points TC within 0% of TAC = 30 points TC more than 0% higher than TAC = 0 points 0 points

7 Quality of Proposal Accuracy No prior negative experiences with District. Response must be complete, leaving no room for misunderstanding. It must be easy to read with terms clearly defined and explained. There should be no question what the cost will be. Response ensures specifications are met. 8 It is anticipated that the successful responder will be notified by January 3, 205. All final contracts and related documents requiring signature must be provided to the District by February 0, 205. The contract will be submitted to the Board of Education at the regular February 205 meeting for conditional approval. Though approved and signed, the contract will be contingent on receiving USAC approval for the anticipated discount amount. Service Commencement It is the responsibility of the responder to ensure all necessary preparatory work is complete in order to ensure a service start date, the latter of, July or within 30 days of issuance of a USAC Funding Commitment Decision Letter for the requested amount. The District may opt to place a full or partial order prior to receiving a FCDL.
