2 (3.0) Page 1 Project Name Gravelbourg High School Addition and Renovation Project Address st Avenue West Gravelbourg SK, S0H 1X0 Project Number Colliers Project Leaders, on behalf of Prairie South School Division No 210 is requesting proposals for the supply and installation of furniture components as outlined in this RFP for the location noted above. Respondents agree to hold pricing provided within this RFP for a period of ninety [90] days from close of RFP, should award not be made immediately. 1. General 1.1 All submittals shall be delivered and submitted to the following addressee before: June 21, :00:00 PM CST. Responses will be made in writing four (4) business days before close. 1.3 Evaluation criteria are outlined in section 5 of this RFP. The lowest price may not necessarily be awarded the project. 1.4 The client and Colliers Project Leaders reserve the right to reject any and all submissions. 1.5 The client and Colliers Project Leaders reserve the right to accept proposals in their entirety or portions thereof. 1.6 The proposal shall be prepared at the sole cost and expense of the manufacturer / dealer. 1.7 Schedule: : RFP issued June 21, 2016: RFP closed June 24, 2016: Award August, 2016: Installation of new furniture Attached appendices: Appendix A: Proposed floor plan, note furniture layout may vary.

Project Scope of Work 2.1 Project Coordination: All coordination shall be done directly with Colliers Project Leaders The Successful Proponent shall provide time schedule outlining furniture specification, manufacturing, delivery, installation, deficiency walk-through and correction of all deficiencies The Successful Proponent shall provide updates to the schedule as necessary It is the Successful Proponent s responsibility to track deliveries and to ensure all dates are met The Successful Proponent s Project Manager assigned to this project shall attend a minimum of two [2] site meetings prior to ordering/installation to ensure coordination of furniture issues with relevant trades. 2.2 Furniture Specification The Successful Proponent shall coordinate directly with Colliers Project Leaders for all component and finishes selections Plans, elevations and 3D line drawing of each typical shall be submitted To Colliers Project Leaders for approval prior to commencement of specification Finish samples of all selections shall be submitted to Colliers Project Leaders for final approval prior to order placement Final specification shall be reviewed by Colliers Project Leaders and signed-off by client representative prior to order placement. 2.3 Delivery and Installation All deliveries shall be made directly to the site: Gravelbourg School st Avenue West Gravelbourg SK, S0H 1X Any damage to the site as a result of furniture delivery and/or installation shall be repaired by the owner s contractor at the expense of the furniture dealer. 2.4 Final Clean Upon completion of work, remove all excess products, packing materials Upon completion of work, all furniture shall be cleaned and dusted; surrounding areas affected by installation crew shall also be cleaned and vacuumed. 2.5 Deficiencies and Client Sign-off Upon completion of installation and final clean, the project manager assigned to this project shall coordinate a deficiency walk-through with Colliers Project Leaders, the client representative and installation coordinator A written deficiency report shall be produced by the furniture Project Manager and distributed to the team in a timely manner. Include all dates for rectification of all issues outlined in the deficiency report When the client representative deems that all deficiencies have been rectified, and that all conditions of this document have been met, final invoice may be issued.
