Sufficient Employees or Subcontractors Per zone To Be Trained In CITB Safe Support To Excavations Sufficient Employees or Subcontractors per zone to be Confined Space Trained to CP1 & CP2 level Mechanical Excavation Equipment may only be operated by personnel in possession of a CITB CPCS or NPORS card. Steel toe capped boots, gloves, hi-visibility vest, safety specs and flame retardant overalls to be worn at all times. Service Avoidance Permit/Safe to Proceed form to be in place prior to any excavation being undertaken All plant must be in go service and inspected daily with records kept weekly. Head Protection, High Visibility Clothing, Steel Toe Capped Boots, FR Clothing/Overalls, Hand Protection & Safety Specs To Be Worn At All Times Site Specific Assessment must be undertaken prior to the commencement of any work activity and reviewed daily or upon change to working environment / working conditions. Daily H & S briefing records to be completed and briefed to teams prior to daily start work. Gas, Water, Sewer, BT and Electric Plans Must Be Available As A Minimum A Line search and Grey Area check must be carried out to determine the presence of Major Accident Hazardous Pipelines (MAHP) prior to excavation commencing Monitoring

All traces shall be marked up with Non-permanent spray paint Encroachment lines must be marked 0.5m outside the area to be excavated near traces to show where hand digging must be undertaken A Service Avoidance Permit/Safe to Proceed form must be completed and signed off by Team Leader and Supervisor prior to excavation work commencing Existing Services Shall Be Exposed Using hand dug Trial Holes Prior To Mechanical Excavation. Unidentified cables/services must be reported to Line Manager for further investigation. Pneumatic Breakers Or Other Power Tools Shall Be Used To Remove The Road Surface Only Mechanical Excavators Shall Not Be Used Within 0.5m Of A Buried Service Utilities Shall Not Be Used As A Foothold Measures Residual risk columns 5 6 All Utilities Shall Be Treated As Live Where excavation is required within the vicinity of overhead lines the electricity supplier should be contacted to identify the minimum safe working distance from the overhead line (in accordance with GS6. Suitable and sufficient welfare provision must be available, including Drinking Water, Hot and Cold Water, Soap, Towel and Resting Facilities All excavations shall be assessed for support requirements regardless of depth. Protection Shall Be Given To Tree and plant Roots During Excavation All Excavations Deeper Than 1.2m Must Be Supported and a Permit to Work issued Monitoring Page 3 of 6

When breaking ground with a mini-excavator pecker where a windscreen is not fitted, operators are to ensure that there is sufficient distance between the driver and the activity to ensure they are not at risk of ejecting debris. Excavation 1.5m Deep - Spoil 1.5m From Edge) All Excavations Deeper Than 1.2m Must Have Suitable Edge Protection And Guarding With Heras Fencing Access Into Deep Excavations Shall Be Via A Tied & Secured Ladder Where Necessary Gasco seekers Shall Be Used Within Deep Excavations Excavations Shall Be Guarded At All Times (NRSWA) and SLG Matrix in job pack. Do Not Break Away Bricks Or Concrete. Cables in concrete must be made dead or an alternative route for the excavation shall be found. Process for Services Encased in Concrete must be followed. Always Keep Plant Well Away From The Edges Of The Excavation To avoid surcharging the sides of the excavation Switch Vehicles Engines Off Or Park To Prevent Ingress Of Exhaust Fumes Stop blocks to be placed 1metre from the edge excavations where close approach by plant and vehicles is likely. Surface Water Shall Be Channelled Away From Excavations Using Sandbags Where Necessary Water Pumps Shall Be Supplied To Remove Water From Excavations Water removed from excavations shall be pumped to a settlement tank, tanker or grass verge where necessary Ensure That Excavations Are Reinstated As Soon As Possible All Work Shall Be Stopped In The Event Of Measures Residual risk columns Monitoring
