14 Documenting an Incident Incident Log (diary of events) What happened? How was the incident addressed? When did the incident occur? Who was involved? Documentation is Good Even when you are documenting a mistake Mistakes will be made When in doubt, document 14

18 Incidents at Poll Book Table Voter numbers that appear on both the poll book and the new registration poll list Cross out and initial number on poll book 10 After assigning voter number and getting signature, we learned Breese Stevens had moved. Crossed out voter number on poll book and sent voter to registration table. 10:00 ZDE BAB Incidents at Poll Book Table Voter ID issues 11 Voter presented UW Faculty ID as voter ID. Reviewed list of acceptable IDs and offered provisional ballot. They left without voting. 10:05 LWW 18
19 Incidents at Registration Table Abandoned registration form Voter registration issues 12 Harlow Orton began filling out a registration form but changed his mind. Sending partially completed registration form to Clerk s Office in tote. 10:15 SAH Incidents at Registration Table Proof of residence issues 13 10:30 A potential voter unable to register because SAH they only had a passport to present as proof of address. Explained that passport doesn t list an address. Reviewed proof of address options with them. They said they might return to polls later. 19
20 Incidents at Ballot Table Things that come up while issuing ballots 14 10:38 Mistakenly handed two ballots to a voter. LAD Voter returned extra ballot. Tore ballot and placed in Discarded envelope. 15 Realized we had issued some ballots with 10:45 LAD only one set of initials this morning. Now following correct procedure. Incidents at Ballot Box Jammed ballots Any time you open ballot box 16 Ballot jammed. Danced with tabulator 11 ABW and ballot dropped. Screen indicated ballot had been counted. 17 Another ballot jammed. Tabulator dance 11:05 ABW didn t work. Opened ballot box and pulled ballot into bin. Relocked ballot box. 20
21 Incidents at Ballot Box Use of emergency bin 18 Unable to clear ballot jam. Called Clerk s 11:20 ABW Office and started using emergency bin. Posted emergency bin signage. 19 Clerk s Office cleared ballot jam. Feeding 11:50 ABW emergency bin ballots into tabulator. KSE 21
