4 1.0 INTRODUCTION On behalf of the Beverly Hills Unified School District (BHUSD), Environmental Audit, Inc. (EAI) is submitting this Monitoring and Maintenance Plan (M&M Plan) for hardscape and methane and hydrogen sulfide (H2S) monitoring probes in Area of Interest 5 (AOI 5) at the Beverly Hills High School (BHHS) located at 241 S. Moreno Drive, Beverly Hills, Los Angeles County, California (Site) (see Figure 1 and Figure 2) to the Department of Toxic Substances Control (DTSC) for review and approval. The BHUSD is proposing improvements and code upgrades to structural, mechanical, electrical, and plumbing systems at the BHHS, as well as demolition of certain facilities, and construction of new facilities, including an underground parking structure. The purpose of the M&M Plan is to describe the requirements necessary to effectively monitor and properly maintain hardscape areas, which function as an effective barrier between contaminants detected in soil and students, faculty and staff, and methane and H2S probes located in AOI 5 of the BHHS. The M&M Plan also outlines notification policies and procedures for discovery of damage or deterioration from use, weather, seismic activities, as well as any planned alternation from digging or excavation. 5 2.0 SITE DESCRIPTION The Site is located on the northwest corner of the intersection of West Olympic Boulevard and Spalding Drive in Beverly Hills with an address of 241 S. Moreno Drive (see Figure 1). The Site is comprised of approximately 25 acres of real property that is presently identified as the BHHS. The BHHS is covered by buildings, asphalt and concrete pavement, artificial turf, grass fields and landscaping (see Figure 2). Since 1979, all oil production activities have been consolidated and limited to an approximately 0.6 acre oil production facility located at the southwest corner of the BHHS that currently is and has been operated by Venoco Inc. since ASSESSOR'S PARCEL NUMBER The Site is identified by the County of Los Angeles by the following Assessor's Parcel Numbers: 2.2 LAND USE Size Assessor's Parcel Number (acres) ± ± ±5.32 Total ±25.11 The BHHS, which teaches ninth through twelfth graders (9 12), was originally built in 1927 by the Los Angeles High School District and became part of the BHUSD in Prior to development, the real property on which the school is located was primarily vacant land, but did include oil exploration activities. Land use surrounding the Site is a mixture of residential and commercial development (see Figure 2). Land use west of the BHHS is commercial, with residential development surrounding the campus in all other directions. SAB:2751:M&M:PLAN JUN EAI Project No. 2751

6 3.0 BACKGROUND INFORMATION 3.1 AREA DESIGNATIONS For purposes of assessment and reporting, the BHHS has been divided into eight areas of interest identified as follows (see Figure 3): Area of Interest Acres AOI 1 ±2.2 AOI 2 ±0.9 AOI 2A ±1.4 AOI 3 ±0.22 AOI 4 ±3.6 AOI 5 ±4.7 AOI 6 ±7.0 AOI 7 ±4.6 An approximately 0.6 acre oil production facility is located at the southwest corner of the BHHS that currently is and has been operated by Venoco Inc. since 1995 (see Figure 3), which coupled with the areas of interest comprise the school site. 3.2 INVESTIGATIONS Investigations have been conducted for all areas of interest at the BHHS under the oversight of the DTSC (see Section 4.0). These investigations identified chemicals in soil at all areas of interest at the BHHS, except AOI 3, at levels that require mitigation or remediation. A vapor intrusion human health risk assessment conducted for the BHHS using the maximum concentrations of all volatile organic compounds (VOCs) detected in soil gas and school exposure scenarios, estimated the incremental lifetime cancer risk (ILCR) to: (1) students at less than one per million for all areas of interest, except AOI 6. The area of AOI 6 where the estimated risks were greater than one per million is the location of the proposed underground parking structure; and (2) staff at less than one per million for all areas of interest, except AOI 6 and AOI 7. The ILCR for AOI 7 was estimated at 1.3 per million. AOI 7 currently is and will remain a play field with no habitable structures upon completion of the improvement project (see EAI, 2016C). Methane and H2S were detected in some areas of the BHHS at levels that require mitigation. 3.3 REMOVAL ACTION FOR AOI 1 A removal action (excavation and off site disposal of soil impacted with arsenic, total petroleum hydrocarbons as diesel [TPH D] and dieldrin) was completed in November 2015 for AOI 1 (see EAI, 2015H) and approved by DTSC in a letter dated January 7, Portable trailers have SAB:2751:M&M:PLAN JUN EAI Project No. 2751

7 been installed in AOI 1 and will be used to temporary house students, faculty and staff during the improvement project, which will be conducted in phases over several years. 3.4 REMEDIAL ACTION PLAN The BHUSD is in the process of preparing a Remedial Action Plan (RAP) to address contamination in other areas of the BHHS. 3.5 LAND USE COVENANT AND SOIL MANAGEMENT PLAN Because the risk assessment assumed exposure scenarios based on school use and soil above screening levels will remain at depths deeper than 5 feet, and existing hardscape areas will continue to be used as a barrier to prevent exposure to soil contamination, a deed restriction/land use covenant (LUC) will be recorded that prevents residential use of all real property that comprises the BHHS. A Soil Management Plan (SMP) has been submitted to DTSC providing guidance and protocols for conducting non remedial excavation work at the BHHS, such as excavation for footing extensions and micropile installation for seismic retrofitting of existing buildings, and trenching for the installation of new utility lines (see EAI, 2016D). 8 4.0 CHEMICALS OF POTENTIAL CONCERN The results of assessment work conducted to date for AOI 5 are provided primarily in three EAI reports for the BHHS titled "Draft Preliminary Endangerment Assessment Report, AOI 2A, AOI 4, AOI 5, AOI 6 and AOI 7," dated December 14, 2015, "Draft Vapor Intrusion Human Health Risk Assessment Addendum," dated February 2, 2016, and "Draft Revised Preliminary Endangerment Assessment Report AOI 5," dated June 8, The results of background research conduct for the BHHS and surrounding properties identified historical (see Figure 4) and current oil field activities as well as historical and current school buildings as sources of potential contamination that requires evaluation. The chemicals of potential concern (COPC) in soil by suspected sources are identified as follows: Oil Field Activities Total petroleum hydrocarbons as gasoline (TPH G), diesel (TPH D) and oil (TPH O). Volatile organic compounds (VOCs). Semi volatile organic compounds (SVOCs). Title 22 Metals. Historical/Current School Buildings Lead. Arsenic. Organochlorine pesticides (OCPs). Polychlorinated biphenyls (PCBs). 10 5.0 BASELINE INSPECTION, MONITORING AND REPORTING Funding is presently available only for upgrading Buildings B1, B2 and B3 and all three buildings are currently vacant and will remain vacant until completion of the improvements. Improvements to Building B4 will not be initiated until funding is available. Interior upgrades for Buildings B1, B2 and B3 are scheduled to commence in June 2016 and do not require DTSC approval. Structural/seismic upgrades for Buildings B1, B2 and B3 are scheduled to begin in October 2016 and do require DTSC approval since soil will be excavated to expose and expand the footings. Improvements to Buildings B1, B2 and B3 are anticipated to be completed by the end of During this time period, students, faculty and staff normally located in Buildings B1, B2 and B3 will be housed in the temporary trailers located in AOI 1 (see Figure 3) and/or other areas of the campus. All monitoring, inspection and reporting will be conducted under the supervision of properly trained and certified personnel, e.g., registered geologist, registered engineer or certified industrial hygienist. 5.1 BASELINE INSPECTION Hardscape Areas Prior to implementation of the M&M Plan, an inspection of the hardscape areas will be conducted to identify each hardscape area, e.g., Building B1, Hardscape Area A, and to document the baseline condition of each hardscape area. It should be noted that excavation for footing extensions and micropile installation for seismic retrofitting of Building B1, Building B2, Building B3 and Building B4 will be conducted first and will require excavation in hardscape areas to expose and expand the footings, followed by repaving of the excavated areas. This excavation work will be conducted in accordance with the requirements contained in the SMP (see EAI, 2016D). Once this work has been completed, hardscape areas and the baseline condition of each hardscape area will be identified, including photographs, and a figure provided to DTSC documenting hardscape area locations Monitoring Probes For the reasons discussed in Section 5.1.1, the six additional probes (A5 SG7 through A5 SG12) to be installed for methane and H2S monitoring (see Figure 4 and EAI, 2016E) will be installed following completion of the seismic retrofitting of Building B1, Building B2 and Building B3. In the event any of the existing probes (A5 SG1 through A5 SG6) are destroyed or damaged during implementation of the improvement program, they will be replaced at the time the new probes are installed. Once this work has been completed, a figure and survey data will be provided to DTSC documenting probe locations. SAB:2751:M&M:PLAN JUN EAI Project No. 2751

11 5.1.3 Interior Building Areas Building upgrades may require modification of the interior layout of Building B1, Building B2, Building B3 and/or Building B4. Since selected interior areas of Building B1, Building B2, Building B3 and Building B4 will be monitored for methane and H2S, a figure depicting proposed interior building monitoring areas will be provided to DTSC for review and approval once interior upgrades are completed. 5.2 MONITORING Quarterly monitoring is proposed to be performed for the first three years, after which EAI is recommending that the monitoring data be reviewed and evaluated by DTSC to determine if the frequency of monitoring can be reduced to semi annual or annual. Regardless of the monitoring frequency, in the event of a significant seismic event (see Section 6.2), monitoring will be conducted within 24 hours of the event. Prior to use in the field, all instruments utilized for monitoring purposes will be properly calibrated, prior to each monitoring event, in accordance with the manufacturer's specifications. If a monitoring event identifies a hardscape area damaged or degraded to where underlying soil could be exposed or elevated methane or H2S readings, DTSC will be notified with 24 hours of the discovery and response actions implemented as outlined in Section Hardscape Areas Visual monitoring of the integrity of hardscape areas will be conducted to document the condition of each area and the results recorded on the Hardscape Monitoring Log (see Appendix A). Areas that are identified as being damaged or degraded to where underlying soil could be exposed, if not repaired, will be repaired immediately. The monitoring logs will create a chronological record of the integrity of each hardscape area to function as a barrier between contaminants detected in soil and students, faculty and staff Monitoring Probes Methane monitoring will be conducted for the vapor probes (see Figure 5) utilizing a Landtec GEM 2000 gas monitor and H2S a Jerome 631X analyzer. Three purge volumes of air will be removed from the probes prior to sampling (methane first followed by H2S) and the results recorded on the Vapor Probe Monitoring Log Methane and Hydrogen Sulfide (see Appendix A). In addition to sampling, the integrity of the probes will be assessed and repairs or replacement completed as necessary and documented in the monitoring report. 12 Existing probes A5 SG1 through A5 SG6 are triple nested, except A5 SG5, all containing probes at 5 and 15 feet, and 24 or 30 feet. Existing probe A5 SG5 is double nested at 5 and 15 feet and all the new probes (A5 SG7 through A5 SG12) will be double nested at 5 and 15 feet. Appendix B contains the soil vapor probe construction details for the existing probes, and the new probes will be constructed identical to A5 SG5. Monitoring will be conducted for the probes at depths of 5 and 15 feet Interior Building Areas Methane monitoring will be conducted for interior building areas utilizing a Landtec GEM 2000 gas monitor and H2S a Jerome 631X analyzer. The results will be recorded on the Building Monitoring Log Methane and Hydrogen Sulfide (see Appendix A). 5.3 REPORTING Quarterly monitoring reports are proposed to be submitted electronically to DTSC (Scarlett Xihong Zhai, PhD Xihong.Zhai@dtsc.ca.gov) in accordance with the following schedule: Reporting Period Report Due Date January March April 30 April June July 31 July September October 31 October December January 31 The reports will include the monitoring logs, figures and photographs and describe: Results of visual inspection of hardscape areas and probes. Comparison and discussion of inspection results against baseline data. Description of any repairs or improvements to hardscape areas and/or probes. Description of any significant changes in Site conditions or usage. Description of any response/contingency actions implemented. 13 6.0 RESPONSE ACTIONS 6.1 DAMAGED/DETERIORATED HARDSCAPE Hardscape areas that are identified as being damaged or degraded to where underlying soil could be exposed, if not repaired, will be repaired immediately. An initial response action, prior to repaving, may be required. The initial action will include closure of the damaged area using barricades or other means, and may require the damaged area to be covered with heavy plastic sheeting until the area is repaved. Air monitoring during repaving work will be conducted in accordance with requirements of the SMP (see EAI, 2016D). 6.2 RESPONSE TO SEISMIC EVENT For the purposes of response planning, a significant seismic event is defined herein as the following "an earthquake of magnitude 5.0 or greater on the Richter Scale within 10 horizontal miles of the Site, recorded at a seismometer station." The following outlines the actions to be implemented within 24 hours of a significant seismic event: Conduct an inspection of hardscaped areas for cracking or other damage. Conduct an inspection of interior and exterior building spaces. Conduct a monitoring event as described in Section 5.2. If significant methane or H2S concentrations are detected in monitoring probes or inside the buildings pursuant to the guidelines contained in Section 6.3, implemented response actions and immediately notify DTSC. Prepare and submit an Incident Report to DTSC as soon as possible. 6.3 RESPONSE TO ELEVATED METHANE OR H2S READINGS Methane has not been detected in soil gas samples collected from any depth, i.e., 5, 15, 24 and 30 feet, in AOI 5 at or above the 5,000 parts per million volume (ppmv) guidance provided by DTSC for school sites (see DTSC, 2005) or the H2S guidance of ppmv. For purposes of determining the need for a response action, the following criteria are proposed for probe and indoor air readings: Methane H2S Source (ppmv) (ppmv) Response Probe 5, Check Indoor Air Readings. (1) Increase in air exchange rate to be completed/certified by a licensed HVAC contractor and certification include in the Incident Report.
