2 We are now living in a borderless and interconnected world. A health threat anywhere is a health threat everywhere. Global and national health security is of the importance to prevent, detect, and respond to possible health threats that may occur. 3 Day One Session 1 Overview of Workforce Development for IHR/GHSA The main goal is to prepare countries to respond to outbreaks IHR provides framework for public health goals that links with GHSA goals (prevent, detect and respond) Workforce development is one of the Action Package under GHSA and includes broader groups of professionals such as clinician, statistician, lab technician and epidemiologists We must focus on every country s needs and strengthen capacity to respond to outbreaks before they become an epidemic GHSA Results from Bangkok to Yogyakarta Work will strengthen the health in the region Importance of IHR highlighted 2014 GHSA initiated to help countries increase IHR capacity Thailand > 30 year experience FETP took the lead for GHSA Workforce Development Action Package GHSA meeting in BKK (introduce the countries to GHSA activities) focused on how to work together in the region with 24 national organizations in the region represented Training workforce. Joint investigation and response. Share experience and resource in the region. Survey Results of Workforce Development of Epidemiology during Bangkok Meeting Self administered questionnaire during the Bangkok meeting related to PH workforce capacity building in the country. Only 14 respondents. Summary: Most counties have national plan for PH workforce development and the regional collaboration can be helpful for the training program Where program is existed, it includes physician, animal health and public health.
4 Day One Session 2: Country Strategic Plan Saudi Arabia Saudi FETP has already been established since 1989, graduated a number of 133 epidemiologists Other Programs in the Kingdom that provide postgraduate degrees in Epidemiology: King Saud University MPH (Field Epi Track); King AbdulAziz University MSc in Epidemiology; King Saud Bin AbdulAziz University National Guard MSc in Epidemiology. Workforce strategy: Increase number of candidates and trainers Establish collaboration with Ministry of Agriculture Establish 1 yr program for non-physician health care workers, in collaborating with universities Establish short courses (3-6 months) for health care workers With MOA to produce trained veterinarian Vietnam Epi training network in VN: Medical universities, regional institutes and Vietnam FETP (under MOH: 2 yr training program) Vietnam FETP Established 2007 by ADB, FAO, WHO support 2012 launch one month onehealth course Courses: 3-week, 3-month, and 2-yr courses Achievements: Pandemic influenza, dengue, Ebola, Zika Challenges: limited budget, part time fellows, limited multisectoral collaboration Workforce Strategic plan Target in 2020, to produce 1550 short term FETP (now 13% achieved) and 100 for long term trained epidemiologists (now 40% achieved)
5 Day One Session 3: Existing regional strategic frameworks Regional Strategic Framework for Veterinary Epidemiology Capacity Development and Networking in ASEAN: Workshop organized for member states. Workshop hosted in Thailand. 9 member states from ASEAN attended Develop a common vision. Most difficult one. Took 2 days. Members were asked what they want to see within 5 years. No words just make drawing. Epidemiology needs evidence policy support. Drawings translated into words. 1)Vision, 2) Analyses of drivers and obstacles (individual responses/ group responses), 3) Identify strategic goals (prioritaisation), 4) Process for endorsement and implementation (Asean process, key issues, strategic planning process) 2012 Framework initiated. Asean workshop to develop the Epi framework (Endorsed at ministerial level) ASEAN organized AVEG to implement strategies. AVEG draft ToR developed AVEG established. Epi Mapping tool developed elements: 1) structure & system, 2) Coordiantion and colalaboration 3) HR &management 4) Policy advocacy and support Opportunities: 1) Align animal and Human health strategies 2) Enhance multi sectoral collaboration
6 Day One Session 3: Existing regional strategic frameworks Asia Pacific Strategy for Emerging Diseases (APSED 2010): APSED is a bi-regional tool to help two WHO regions implement and strengthen IHR core capacities, and preparedness to respond to EIDs APSED is aligned with other existing strategies including GHSA, IHR, laboratories, etc. Key areas learned from this strategy includes: a unified WHO emergency programme, a global health emergency workforce, IHR core capacities and resilient national health systems, improvements to the IHR, accelerated R&D (e.g. vaccine development), and international financing/contingency fund Review was conducted and resulted in a recommendation to develop an updated strategy (next version of APSED) Now is developing APSED III (emphasis on M&E)
7 Day One Session 4 Draft Regional Strategic Framework Why we need regional strategic framework? ASEAN has endorsed Regional Strategic Framework for Veterinary Epidemiology Capacity Development and Networking in ASEAN GHSA is a mechanism that can enhance this collaboration. Public health workforce is a critical element. How draft is developed? Survey result Existing frameworks, for example, Regional Strategic Framework for Veterinary Epidemiology Capacity Development and Networking in ASEAN 2013, etc. Target Strategic goals Cross-cutting Strategy Implementation arrangement Monitoring and evaluation Group Work
8 Day Two Session 5 Finalizing Draft Regional Strategic Framework Summary of key decisions Specific to ASEAN vs general regional framework document not specific to ASEAN Human Health vs Public Health Public Health Number vs Numbers + Systems Numbers + Systems Linkages refer to existing platforms rather than specific list Infectious Disease vs. Public Health Threats (Public Health Emergencies) should be facilitative but not too broad (core: IHR/GHSA) Implementation Arrangement: who owns this? Everyone public domain; in coordination with GHSA Workforce Development Action Package Resource mobilization leave open for now. For GHSA, resource mobilization has been at country level. At Regional level, what is the body that would receive resources? Regional level is focused on regional coordination and dialogue, initiating activities as preparedness effort
9 Day Two Session 6: Collaboration ASEAN ASEAN governance has changed from working group to clusters There are four clusters. Relevant to this strategic framework is cluster no 2 (responding to all hazard and emerging threats). Cluster 2 meeting Jul SOUMSD will be in August 2016 in Brunei The framework can be considered for FETN consideration for adoption FETN+3. Strategies: Promote capacity building for FETP Joint collaboration Ensure active and effective network Activities: Experience and information sharing, joint field investigation, joint training to address regional concern AVEG The strategic framework should be endorsed by AVEG governance Can use the strategic framework as guideline Look in detail of strategic framework and use the framework as reference for activities in future
10 Day Two Session 6: Collaboration WHO IHR 2005 and APSED framework is championed by WHO. The document is inline and fit into IHR and APSED. WHO delivered technical assistance to member states and GHSA funding are being directed through Regional Offices. FAO Regional program on FETPv has been started in several countries. Member state from ASIAN and China can send candidates to FETPv. Supported veterinary side with collaboration with multi sectors. Under EPT 2, there are initiatives from EPT 2 countries on work force development. This programme align with GHSA and support multi sectoral collaboration with epi capacity building and sensitize countries on the important for multi collaboration. FAO contribution will be supporting animal health sectors for contributing to the strategic framework. OIE PVS platform is sustainable improvement for countries on veterinary services include governmental and non governmental organization. Qualitative assessment of veterinary performance using the PVS tool composes four fundamental component. One of the components is assessment of human resources-> work force development. After evaluation, OIE conducts gap analysis for country s needs, include indicative budget, calculate 5 years budget for veterinary services. The treatment working on programme specific, such as training programmes (veterinary legislation, laboratory, etc). PVS follow up mission, to compare the country progress compared with previous evaluations. OIE and WHO -> pilot workshop IHR and PVS bridging. Objectives identification of synergies of PVS and IHR and identify areas to improve collaboration in human-animal interface. Operational framework for human-animal interface OIE will conduct National OIE and WHO bridging workshop at country level
11 Day Two Session 6: Collaboration INDOHUN 34 faculties are members of INDOHUN (School of medicine, veterinary, public health); also part of SEAHUN (South East Asian) Objectives of INDOHUN Establishment of epidemiology of public health and animal health work force To strengthen health system and multi sectoral coordination under coordination ministry of human development and agriculture The framework is opportunity to strengthen work force inline with IHR 2005 and GHSA to improve country capacity to detect, prevent and response to EIDs The framework can be adopted by SC meeting of FETN Ethiopia Ethiopia s contributed countries for public health workshop development. Believe advantages for sharing expertise between regional countries Saudi Arabia Should include multi sectors. Collaboration between countries s: any alliance will improve the work. Good opportunity for joint investigation, joint research and joint conferences.
12 Day Two Session 6: Collaboration USAID USAID supports Royal Thai government as lead countries on work force development GHSA action package to work collaboratively with the countries in the regional to develop regional strategic framework for human health work force development on epidemiology. Through the partnership with FAO, USAID continuously supports regional veterinary epidemiology capacity development and networking, Regional Epidemiology Training Programme for Veterinarian (FETPv) including short course in epidemiology in actions. Support future and current one health work force development through the regional and national one health university network (OHW). Through these networks, national coordination offices and their university partners are working with national government to define national one health workforce needs and strategies for their realization. USAID South East Asia network (Asia, Philippines, Malaysia, Thailand and Indonesia). USAID working with One health workforce, support for WHO and FAO. US CDC Mapping of FETP graduates. Standardize curricula FETP accreditation, establish accreditation process through TEPHINETS The FETPs contributed to EOC, the FETPs trained in emergency management (EOC).
13 Day Two Session 7: Exercise for the National Strategic Plan Note for additional feedback for framework document: need to add achievements; recommend SWOT analysis Regional Strategic Framework helpful for countries as they are developing and/or strengthening programs Important to make sure it is inter-sectoral and that Regional Strategic Framework should be flexible Gaps identified: How do some countries start programs as that is a challenge Effectiveness of GHSA coordination For countries not part of GHSA, how to get involved with what is being produced Ensure buy-in up to high-level from the beginning Ensure identified focal point Ensure adequate communication Identify desired outcomes at the beginning M&E key component How being implemented? How to determine if implementation is successful? Ensure budget is in place Ensure there are incentives for trainees and clear career pathways for graduates
14 Day Two Session 8: Implementation Framework or guidance voluntary, in support of existing international and national frameworks and plans Add value to existing frameworks and plans To IHR: strengthen workforce capacity, focus on EID/public health threats To Other: supports workforce development, recognizes essence of multi-disciplinary professionals, embraces OneHealth approach, provides guidance Principle implementers are countries Equally important implementers are regional organizations and their international partners how to get their support? Thailand is lead country for GHSA Workforce Development Action Package how will Thailand serve to move this framework forward? Depends on collaboration and coordination that we re going to have From beginning, get right support from policy makers Build on existing structures, stepping-stones of what IHR already built Collaboration with existing partners key, ask that they help own with Thailand as leading country and help to move Regional Strategic Framework forward Main counterpart that will help workforce development agenda forward is FETP workforce not only epidemiology but epidemiology is cornerstone and starting point; then will need to work together on other aspects as well Good coordination with TEPHINET key Upcoming opportunity to coordinate with GHSA US CDC long-standing FETP support key linkage to programs globally USAID Office in Thailand another key linkage/strength Close coordination with Indonesia as Chair Main activity that we can take advantage from each other is Training of Trainers (plan for 2016/2017) Collaboration and coordination between animal health and human health sectors (FAO and OIE) Building on existing frameworks and platforms in ASEAN Joint activities, work with partners, expedite through established GHSA coordination mechanisms Build on findings, activities and platforms coming from PVS pathway; identify initiatives to support countries
15 Day Two Session 8: Implementation Indonesia: Don t yet have national strategic framework for workforce for epidemiology As such, this Framework can be helpful Need data first to guide development of national strategic Map FETP graduates, goal of developing network of FETP graduates (to date, 600 alumni) Review existing policy and strategy and ensure aligned across key areas (eg. laboratory) Collaboration with Epidemiology Professional Association Improve capacity of all epidemiologists in field Collaborate with University, improve level of trainees Challenge to get to target (1 per 200,000) also focus on improving health worker capacity with training, how to spread the number of epidemiologists to reach population Saudi Arabia How can you ask FETP graduates to participate as mentors? What incentives to encourage them? Vietnam Three model to training FETP (short courses up to long course) How is OneHealth incorporated into FETP? Who will conduct training course? Ans: Need to collaborate; examples of extending FETP training to welcome trainees from other sectors (eg. animal health, wildlife); some countries developed FETP-V and FETP-W programs; many managerial issues to solve (eg. how to fund)
16 Day Two Session 8: Implementation Myanmar: This Strategic Framework is very strong and very important resource to every country Need to standardize training Need to include other sectors and ensure good coordination, cooperation, collaboration from high level to field (can be challenging) Need to develop human resource, this framework can help be push/help drive national authorities to adopt or implement concept Technical sharing also important Malaysia: Timely, lining up with existing planning for health workforce (clinical component already endorsed) Challenge is epidemiology training is siloed, look at curriculum and how to strengthen collaboration between educational, training side and ministry/public health program side Good alumni support (EIP Gives Back) Strong FETP-V program, need to strengthen the marriage Already have joint exercise and regional collaboration that can be built upon and share good practices Laos: How to go forward? Look to Thailand as lead country Key that Strategic Framework is voluntary, get support at policy level (eg. DG/FETP Dir) Ask Thailand to support assessment, see the capacity feedback the result to the country, partners (eg. WHO, FAO, OIE)
17 Day Two Session 8: Implementation Brunei Darussalam: Not member of GHSA, do not have national strategic plan may be challenging to get buy-in but will look into it, will be helpful if adopted by ASEAN This Strategic Framework as a good way forward but will need to tease out technical details Cambodia: Have a lot of projects, key is how to help each other, how to know each other Have epidemiology training but only 6 months, has qualified, degreed epidemiologists through collaboration in the region Have strategic plan for all IHR areas See gap and look at how to collaborate in region, tap into knowledge and experience
18 Day Two Session 8: Implementation Q&A Regional strategy needs to support building national capacity, catalyst that helps each country reach IHR compliance More than 35 years of FETP experience among lead countries (Indonesia and Thailand), immense experience that can help inform and support implementation regionally and globally Utilize shared knowledge and experience of FETP with participants from other countries build on regional networking and collaboration through FETP graduates going to back to their home countries For small countries, potential opportunities through regional training Opportunities through Training of Trainers (TOT) program Lead countries concerned about financial and technical support Address burden especially for developing or small countries Ans: GHSA catalyst for IHR compliance; funding focused on country level; JEE will help to identify needs that can help guide donor financial contributions; funding support for regional level could be through country contributions and/or be brought to GHSA Steering Group for identifying ways to mobilize this support Participants requested to review and update lists of focal points
19 Day Two Session 9 Aligning GHSA Plan of Action to Regional Strategic Framework 2016: Plan of Action proposed before Regional Strategic Framework Activity 1: Establish and maintain a functional regional coordinating mechanism and office Activity 2: Develop Regional Strategic Framework Activity 3: Strengthen regional networks on workforce development 2017: Strategic Framework as mobilizing tool for resource mobilization Goal 1: Field Epidemiology Training Develop curriculum together to provide field training for academic epidemiology graduates Forum to bring together producers (training institutions) with users (Ministry of Health) Goal 2: Networking, international and regional Goal 3: Multi-sectoral cooperation, OneHealth Goal 4: Knowledge management
