3 INTRODUCTION Purpose To safeguard the health and safety of the mine workers and people affected by mining activities Objective To reduce mining-related deaths, injuries and ill health through the application of systems that monitor, audit and enforce compliance in the mining sector.

6 CIOM/PI Instructions The employers must respond to the instructions issued by the CIOM irrespective of whether the mine is complying or not complying with such instruction. e.g. women working in isolated areas, secondary escape. Mine must identify if there is any unknown open excavations in the mine premises and take corrective action to prevent any other person from falling into such excavation. All dangerous occurrences must be reported to the Principal Inspector of Mines. The employers must come up with different strategies to combat health and safety challenges in the region.
7 Challenges: Mining Mines fail to empower the Health and Safety Representatives. Copy and paste COP guidelines. Safe declaration is still a challenge in the region. Mines appointing incompetent people to declare the working place safe. Employers are not familiar with their mine procedures. Employees not adhering to the standoff distances.
8 Challenges: Engineering Compliance with regard to Regulation 8.10 of the MHSA: TMM s Pre use checklist or pre start checks Paper work exercises No understanding of the items or components Isolation and Lock out including key control procedure Usage of cell phones for the TMM operator and control measures in place. Usage of equipment not according to design By-pass of safety devices Poor maintenance Brake system not adequately and routinely tested
9 Challenges: Engineering Compliance with regard to Regulation 8.9 of the MHSA: Conveyor belts Pre start checks not done Pre start alarm manual Unauthorised entry Compliance with regard to Regulation 8.5 of the MHSA: Lifting equipment Storage Load test not done No control over issuing and receiving Unauthorised entry No inspection and maintenance
10 Challenges: Engineering Other challenges Over inspection not done Competent persons not conducting the task Over lapping of appointments
11 Challenges: Occupational Hygiene Compliance with regard to Regulation of 9.2(5) (c) of the MHSA: The employer must provide and maintain suitable and adequate, readily available latrine facilities, within a reasonable distance from each working place. - Some operations don t have ablution facilities at all working places. - Some operations that have the facilities, they are not maintained adequately.
12 Challenges: Occupational Hygiene Compliance with regard to Section 5.(1) of the MHSA: Employers to maintain healthy and safe mine environment, as far as reasonably practicable, every employer must provide and maintain a working environment that is safe without risk to the health of employees. - Implemented controls to mitigate the health risks are not being maintained at some mines.(e.g. airborne pollutants controls) - The effectiveness of implemented controls are not being measured frequently.
13 Challenges: Occupational Hygiene Compliance with regard to Regulation 9.2(4) of the MHSA: The employer to must ensure that the illumination at all working places is sufficient to enable employees, who conformed with the requirements of the vision tests conducted in terms of the Guideline for the Minimum Standards of Fitness to Perform Work at a Mine, to perform their work safely. - Illumination of all working places at the mines still a challenge. - Where illumination is provided, is not maintained. Lights that are out of order are being replaced.
14 Other Challenges Applications for Sunday Labour Permission are to be submitted on time with complete information. The employer to indicate if temporary permission is required. The letters of the concerned unions must have their contact details which includes telephone numbers and addresses.
15 CONCLUSION IT IS IMPORTANT THAT ALL THE STAKEHOLDERS SHOULD LIVE THE THEME OF STRIVING FOR ZERO HARM ENSURING EVERY MINE WORKER RETURNS FROM WORK UNHARMED EVERY DAY.
