2 Invitation to Tender for Two Multi-Slice CT Scanners 1.0 General Provisions 1.1 Intent This invitation to Tender is intended to obtain Two Multi-Slice CT Scanners for the Western Regional Integrated Health Authority (Western Health). One unit will be located at the Western Memorial Regional Hospital, Corner Brook, Newfoundland and one unit will be located at the Sir Thomas Roddick Hospital, Stephenville, Newfoundland. This Tender is concerned with the acquisition of two Multi-Slice CT Scanners for the Western Memorial Regional Hospital and Sir Thomas Roddick Hospital, with consideration of the following: - Ongoing service and maintenance support. - All manuals, documents and initial supplies. - The right to reproduce any printed materials supplied with the product for the purpose of using the product. - Training and training manuals. - Future enhancement availability Western Health reserves the right to order additional units at the same price for a period up to and including 31 December Client Background Western Health was established in 2005 and is responsible for the delivery of Health and Community Services in the Western Region. 1.3 Vendor Response

3 1.3.1 Vendor s tender must contain an Executive Summary which shall contain: a. A brief description of the product being quoted. b. The name, title and address of the Vendor s representative responsible for the preparation of the Tender All prices quoted for goods and services must be specified in Canadian dollars, FOB Western Memorial Regional Hospital and Sir Thomas Roddick Hospital. All Tenders will be held to be valid for ninety (90) days following the Tender closing date Tenders must be received in full on or before the exact closing time and date indicated. TENDERS RECEIVED AFTER THAT TIME WILL NOT BE CONSIDERED All costs relating to the work and materials supplied by the Vendor in responding to this Invitation to Tender must be borne by the Vendor. 1.4 Release of Information While Tender is Open: The names of individuals or companies who have picked up the tender documents will not be released At Tender Opening: Only the names of the bidders will be read out After Tender Opening: 1. No further information will be released until after the contract is awarded. 2. After award, only the name and bid price of the successful bidder will be made available. 3. Information will be made available for a 90 day period only FYI, Statements that are included as part of our Tender calls: While bidders are welcome to attend the public opening, please be advised that it is not our policy to release bid information. Only the names of the bidders will be released.

This Authority will not be responsible for inhouse courier services if companies submit quotations by fax machine. The time stated on the fax Tender will become null and void since it is the responsibility of the company placing the Tender to have their Tender at the public opening, therefore, this Authority will not be responsible for any damages or liabilities In order to contribute to waste reduction and promote environmental protection, the Western Health will endeavour to acquire goods and services that support these principles, therefore, product(s) quoted should include:

5 maximum level of post-consumer waste and/or recyclable content minimal packaging minimal environmental hazards maximum energy efficiency potential for recycling disposal costs must not reduce the quality of the product required or affect the intended use of the product must not significantly impact the acquisition cost 1.6 Tender Acceptance Any acquisitions resultant from this invitation to Tender shall be subject to the Public Tendering Act The Tenders shall be opened in the Private Dining Room at The Western Memorial Regional Hospital on the scheduled date and time Any Tender may be accepted in whole or in part. The lowest Tender may not necessarily be accepted and Western Health reserves the right to cancel the Tender call. Western Health shall not be held responsible or liable for the payment of any costs that are incurred by the bidder in preparing a Tender in response to this invitation to Tender Awarding of tender will be based upon the following: Image quality (large and small patients) Price Functionality Dose reduction Software capabilities WIRHA value added benefits Vendor performance Turnkey Upgradability Patient comfort and safety Service and support Clinical applications Coronary Angiography (dose/heart rate, image quality, ease of processing) Please include descriptive literature with your bid. Please complete the attached Vendor s Checklist.

6 Successful Company name(s) and price(s) will be posted on the website when tender awarded. 1.7 Service and Warranty System must be supplied with a full one year warranty guaranteeing the following services: - There must be a 12 hour or less on site response. - There must be a 30 minute or less field engineer phone response. - State number and frequency of PMs required yearly. - At least 98% uptime must be guaranteed. - Supplies and accessories discount must be included. % The warranty must be extended or a credit applied if any of the service guarantees are not met. Describe policy. 2.0 Product Specifications A full or shared service contract is to be negotiated. Please provide quotes on an annual service contract and a five-year contract. 7 2.1 CT SCANNER SYSTEM ( Western Memorial Regional Hospital ) One multi-slice 64 or greater helical volumetric CT scanner with the following specifications: NOTE: The vendor MUST indicate any and all components, software or features that are not included in the quoted price. SPECIFICATION COMPLIANT (yes /no) VENDOR RESPONSE Must have at least 0.5 sec full Rotation Available mas values at 120 kv and full 360 degree rotation should be in the range of 5 mas to 800 mas ( please indicate range) The detector must produce 64 slice (or greater) Indicate detector and cell size Indicate scan coverage Gantry aperture must be a minimum of 70 cm. 8 The system must have radiation dose optimization/reduction capabilities (describe) Include a dose profile for a scan using 120kV, 200 ma & 1 sec scan time for each available slice thickness The vendor s tender shall include a complete turnkey quotation. This tender shall include all equipment installation and construction requirements based upon current building codes. The tender shall also include, but not be limited to the following: Structural & access requirements All local building permits All removal of asbestos All lighting, electrical, and data wiring All ventilation/air conditioning requirements All matters/cost related to delivery of equipment All wall/ceiling/floor construction including finish work Vendors are requested to include any and all furniture that they deem necessary for the CAT Scan suite Vendors are required to provide sheet flooring throughout department (no carpet). Flooring to be Marley Eclipse or equal Mechanical (HVAC & pluming) Vendors are invited for site visits regarding turnkey installation. Vendors that have not previously visited the site at the Western Memorial Regional Hospital to review the proposed space and discuss their tender with the planning committee are required to do so before submitting quotation. A copy of the existing floor plan is attached for consideration by the vendor. All final engineering plans must be provided to WRIHA for final approval

10 Indicate spatial resolution in lp/cm at 2% MTF using 120kV, 200 ma & 1 sec scan time and a 2 mm slice thickness Indicate noise standard deviation in mm using 120kV, 200 ma & 1 sec scan time and a 5 mm slice thickness Indicate the maximum image resolution Indicate Radiation dose in msv on a 225 pound male undergoing CT Coronary Angiography with a heart rate of 60 beats per minute. Please provide technical exposure factors to produce radiation dose. 2.4 SYSTEM ARCHITECTURE SPECIFICATION COMPLIANT (yes /no) VENDOR RESPONSE Complete multi-slice helical CT scanner with all necessary software and hardware required to perform clinical protocol as indicated herein. With the capability of transferring image datasets via DICOM to the organization s G.E PACS. All cost to be included. The proposed system should include all software upgrades or system improvements Given the dynamic state of CT technology, the vendor s response should reflect any upgrades or technological advances which vendors will announce at RSNA It is expected that any upgrades announced at the RSNA 2007 will be included in the system s pricing Describe all Works In Progress

11 2.5 TUBE SPECIFICATIONS The scanner must be equipped with a x- ray tube that has a history of a low tube failure rate The tube must have the ability to handle rapid volume acquisition or in between successive acquisitions in a busy, high patient throughput facility. Indicate heat storage capacity and cooling rate of x-ray tube. Maximum ma must be 500 or greater. Please Indicate maximum ma. 2.6 CONSOLE SPECIFICATIONS Indicate size of monitor(s) Indicate screen resolution Indicate number of programmable protocols including paediatric protocols

12 2.7 WORKSTATION SPECIFICATIONS The proposed system must include one multi-modality, diagnostic reporting station with 3D and other advanced level post processing software Indicate size of monitor(s) Indicate screen resolution List workstation functions available The proposed system should include all workstation software upgrades Given the dynamic state of CT technology, the vendor s response should reflect any workstation upgrades or technological advances which vendors will announce at RSNA It is expected that any upgrades announced at the RSNA 2007 will be included in the system s pricing System must include a GE PACS diagnostic work station.

13 2.8 CONSOLE AND WORKSTATION ACTIVITIES The system must be able to perform the following functions while scanning without affecting response time: Pre-register next patient Set up next scan parameters Access patient and image directory, & display previously recorded images Create/modify scan protocols in protocol directory Archive previously recorded images Perform reformatting of planar images Archive and retrieve disc files Perform scanning of the next sequence start prior to the complete reconstruction of the previous scan The console should be ergonomically designed for efficient use by the operator Describe features. The operator s console must permit access to all system functions, including data acquisition, reconstruction, postprocessing, display, analysis, archiving and filming capabilities

14 2.9 NETWORK The System and workstation(s) must be GE PACS/DICOM compatible / compliant. It must include the following DICOM interfaces (describe in detail). The system must have DICOM Verification Service Class (VSC) The system must have DICOM Query/Retrieve Service Class (Q/R SC) The system must have DICOM Storage Service Class (Both SCU & SCP) The system must have DICOM Storage Commitment Service Class (Storage Commitment SC) The system must have DICOM Print Management Service Class (PM SC) The system must have DICOM Modality Worklist Management Service Class (MWM SC) The system must be capable of interfacing with Meditech HIS/RIS The system must be able to fully integrate with GE PACS Vendor must provide DICOM conformance statement upon request All costs associated with connectivity of system to the GE PACS and HIS /RIS is the responsibility of the vendor (interface, licenses, software and hardware) 2.10 INTEGRATED HEALTH ENTERPRISE STANDARDS Provide IHE integration Statement for the quoted system. Provide time line for implementation of IHE integration profiles

15 2.11 UPGRADEABILITY Software upgrades that enhance existing applications must be provided by the vendor indefinitely at no cost to the purchaser. These no charge upgrades shall include any circuit boards or other parts if software is added to enhance existing capabilities Upgrades and/or enhancements that add new capabilities or parameters to the equipment included in this document shall be made available to the purchaser at a discounted cost of 70% of the actual list price of the items for a period of ten years after the purchase of the system System should have capability to being upgraded as new technology emerges for at least 7 10 years Additional or new software must have the capability of being downloaded by remote computer access. Software must include a free trial period before purchase 2.12 REMOTE DIAGNOSTICS The system must possess remote diagnostic capabilities via high-speed internet access. Please provide details The remote diagnostic capabilities must include the ability to remotely connect to the system on a regular basis to retrieve information about the system The system must possess the ability to access applications and service support through the operator s console without the need to place a phone call The remote diagnostics must be proactive, reactive and interactive

The Vendor shall include a ceiling mounted dual injector compatible and interfaced with the CT system. It is suggested that more than 1 model be quoted, also quote upgrade for existing CT injector for WMRH. The Vendor shall supply an insulflation device used to perform CT colonography. Please quote more than 1 model. The Vendor shall include a phlebotomy chair without armrest Part #A1202-CO Shall include phlebotomy tray Part # A1201-CO and Formica armrests. The Vendor shall include blanket warmer Olympic Warmette model # 10. The Vendor shall include two Stryker Stretchers with IV poles at head and foot. 17 The Vendor shall include a contrast media warmer. The Vendor shall include a professional weigh scale with a 450 pound weight capacity. The scale should include height measurement capability. The Vendor shall include three chairs (Granada or Obus Forme) for operators as per Western Health s specifications. Vendor must include all necessary equipment and software for EKG monitoring for Cardiac Imaging. 3.0 CT SCANNER SYSTEM ( Sir Thomas Roddick Hospital ) One multi-slice 16 or greater helical volumetric CT scanner with the following specifications: NOTE: The vendor MUST indicate any and all components, software or features that are not included in the quoted price. Invacare SX5, 22 seat with Oxygen Cylnider Carrier and IV Pole, swing away leg rests. 4.0 Presentation / Training / Service 4.1 Presentation A presentation of the Tender and / or a demonstration of the product / system shall be provided, if requested, at the Vendor s expense. 4.2 Training and Education Training for technologists and radiologists must be included in quoted price Two week training for 2 technologists for each unit at a similar installation (all expenses included) Dates are to be determined by purchaser The vendor must provide full service tuition for one technologist at the vendor s education centre for each unit (all expenses included). The vendor must provide ten days of on site applications training for technologists immediately following installation at both sites.( Western Memorial Regional Hospital and The Sir Thomas Roddick Hospital.)

28 The vendor must provide five days of on site applications training for technologists after three months of clinical use at each site. Radiologist s training must include four hours of applications training for each radiologist, supplemented with an additional two hours of applications training for each Radiologist three months post the first clinical application of the system. Radiologist training must include educational programs and hands on clinics at a clinical site for three Radiologists. All expenses paid. Quote the cost of training for each additional Radiologist (above three) Describe and quote cost of additional educational /training opportunities or programs On-line and CD Rom tutorial provided on operator s console A detailed description of the in-service training to be provided for the radiologist and technologist personnel. This should include a description of the following; -program duration and format - location - content - qualifications of instructors written materials Include the time frame and/or proposed schedule required to complete training of all applicable staff 4.3 Service The Vendor shall confirm in writing that Parts and Labour will be available for the quoted system for not less than nine (9) years after the warranty period.

29 4.3.2 The Vendor shall provide as an option, pricing for a one-year Service Contract including all parts and labour The Vendor shall provide all Service and Parts manuals required to service the equipment The Vendor shall agree to provide factory training for One in-house Biomedical Technologist, employed by Western Health, for the purpose of maintaining the Multi-Slice CT Scanners. Such training shall be equal to the training provided to the Vendors own service staff. All costs associated with this training, including travel, accommodations, meals and tuition shall be included in the Tender price. 5.0 Product History and Vendor Reputation 5.1 The Vendor shall provide a list of three (3) organizations where a similar Unit has been installed. Include a contact person for each organization. 6.0 Financial Considerations 6.1 All applicable taxes shall be indicated in the Tender. 6.2 The cost for installation, initial set-up and programming shall be included in the Tender price. 6.3 All costs for training shall be included in the Tender. This includes any travel, meals and accommodation. 7.0 Terms of Payment The Authority agrees to pay the full invoiced amount within 30 days following acceptance of the installed system by Western Health. Acceptance testing will be completed within 30 days following the complete installation of the system. 8.0 Terms and Conditions 1. Vendor must state the first date of release of the quoted item. 2. Vendor must indicate upgrades or modifications since first release. 3. Vendor must indicate upgradeability if applicable. 4. Vendor must provide one set of service repair manuals. 5. Vendor must provide optional services packages i.e. Full Service, Shared Service.

30 6. Vendor must have service personnel with a 12 hour or less on-site response. 7. Vendor must be fully compatible and compliant with all DICOM standards and fully GE PACS ready. 8. Vendor must supply expert applications training satisfactory to Western Health. 9. Acceptance testing will be at the expense of the vendor. 10. Western Health will consider ergonomics, ease of use and specifications supplied by the vendor which enhance their quoted products in comparison to other vendors. 11. All equipment quoted must have uninterrupted power supply. 12 All equipment quoted must be CSA approved. 13 The Vendor should inspect the existing facility and completely familiarize itself with all conditions affecting the equipment specified in this document. Failure to do so will not relieve the seller of any of the requirements contained herein. 14. There shall be no separate licensing fee for the use of the vendor s software. In addition, no software licensing agreements shall be interpreted to require a written non-disclosure agreement from the purchaser or its employees or other persons authorized by the purchaser to use the vendor s software. 15. All equipment provided according to the terms of this document shall be of current production, new and of first-rate quality. Reconditioned or previously used equipment shall not be accepted. 16. If the vendor plans to halt the production of the equipment referenced in this document and to produce improved models before the delivery date, vendor shall notify the purchaser of this and provide the option of upgrading the system. 17. A proposed delivery and installation schedule, including time required for installation, start-up, acceptance testing, and removal of existing equipment will be provided by the vendor. 18. Any proposals for equipment that does not meet the requirements of the Medical Devices Regulations or CSA medical standards will not be considered in the tendering process. 19. It should be understood that the warranty period shall not begin until acceptance testing of the system has been completed and approved by Western Health. 20. The vendor shall identify all consumable items required to operate the CT Scanner and provide guaranteed prices for these items for a period of five years. Prices for consumable items shall be subject to negotiation with the purchaser.
