1 Two Park Avenue New York, NY U.S.A. tel fax www. asme.org INFORMATION AND PROCEDURES FOR OBTAINING N, NA, NPT, NV, N3 & NS NUCLEAR CERTIFICATES AND CERTIFICATION MARK CLASS l, 2, 3, MC, CS, CC &TC The ASME Certification Mark is used to indicate that items stamped comply with the Quality Assurance Requirements of Section III, Subsection NCA, General Requirements for Divisions 1, 2 and 3, as well as the specific requirements of other applicable Subsections. Code Books required are shown on the Code Book Order Form, which can be downloaded from our website. The Applicant is required to meet the Quality Assurance requirements of the current Code. Alternatively, if the Applicant wishes to limit its Quality Assurance Program to the requirements of a Code edition and addenda earlier than the latest published, that Code edition and addenda must be shown in the application (Scope of Work Requested by Applicant) and the Quality Assurance Requirements of that Code edition and addenda must be met. The requirement for possession of an official Certification Mark and Certificate is for the purpose of safeguarding the use of the Certification Mark when the Applicant certifies that the equipment is built to the requirements of the ASME Code. It is not the intention of ASME to unduly restrict the use of the Certification Mark, but rather to prevent unauthorized use under all circumstances. ASME Certificates and Certification Mark are furnished from this office only. The Quality Assurance Manual and documentation which support understanding of the program must be in English. ASME, by the use of Survey Teams, reviews the implementation of the Applicant's Quality Assurance Program. Any findings are discussed between the Team and the Applicant, and a report is submitted to ASME, which either authorizes the issuance of the Certificate(s) or requests additional action by the Applicant. The Survey Team consists of ASME Consultants, the Accredited Authorized Inspection Agency and a representative of the Enforcement Authority in Jurisdictions. The "Guide for ASME Nuclear Survey Teams for Certificate Holders" provides a checklist of the elements of a Quality Control System. The Applicant is required to indicate the corresponding cross references to the provisions in the Quality Control Manual which address these elements. The completed Guide must be provided by the Applicant to the ASME Survey Team. Surveys for a Quality Assurance Program Certificate, are also available. It recognizes that an organization has an acceptable Quality Assurance program and the capability to staff, equip, and otherwise implement the program. There is no stamp issued with a Quality Assurance Program Certificate until ASME conducts an implementation audit for conversion to a Certificate of Authorization. Payment of charges for the Survey will be as follows: 1. ASME will submit an invoice to the Applicant for the fees and expenses of the ASME Consultants and the National Board Representative (when the National Board acts on behalf of the Enforcement Authority), administrative charges, the $3,000 three year fee per Certificate and the $300 fee for use of the Certification Mark on initial issuance. 2 2. The Applicant pays their Accredited Authorized Inspection Agency directly. 3. The Applicant pays their Enforcement Authority directly, if applicable. Quality Assurance Systems vary in complexity, detail and structure as do an Applicant's size, scope and volume of work. Because of the many variables, it is impossible to project the length of time required for the Survey Team's visit. The cost could range from $35,000 to $75,000 for Domestic and from $40,000 to $90,000 for International. Price ranges on applications for multiple sites may vary Please bear in mind that the fee ranges quoted are based on typical average surveys and that your survey could require more or less time. These estimates are not firm quotations. If your facility requires an extended survey, because of its complexity, the ranges could be exceeded. Our Survey Team will make every effort to conclude their visit in a minimum length of time. ASME requires an advance deposit of $10,000 for surveys prior to establishment of survey dates. This will be applied to the cost of the survey. If the Applicant withdraws their application after the remittance is received by ASME, there is an administrative fee of $500 which will be deducted from the original remittance. In addition, it is requested that the Accreditation/Certification Agreement and Form A2.70 be ed to ca@asme.org. To avoid unnecessary expense and delay, the Applicant should consult with its Accredited Authorized Nuclear Inspection Agency for advice as to the probable acceptability of the Quality Assurance Program. A complete list of Accredited Organizations holding Authorized Inspection Agency Certificates, and the Code Sections for which they are accredited is shown as a link on the website. The program demonstration/implementation being presented to the Survey Team must be able to demonstrate/implement all aspects of the Quality Assurance Program which will include: 1. Material Organization activities for all Applicants applying for this additional scope to their N-Type Certificates of Authorization. 2. Material Organization activities for Applicants applying for N-Type Certificates of Accreditation with material organization activities appearing in their Certificate scope. 3. A separate demonstration/implementation item which is representative of a pressure relief device (achievement of capacity certification is not required) for all Applicants applying for multiple Certificates of Authorization with an NV Certificate of Authorization being one of them. 4. Class 3 PE Plastic Piping activities for Applicants applying for N-Type Certificates of Authorization whose scope of activity appearing on their Certificate will permit them to fabricate, assembly, install, manufacture, and/or supply Class 3 PE Plastic Pipe. 3 Upon receipt of completed application and deposit, and notification that the Applicant is ready for a survey of the Quality Assurance Program and its implementation, a mutually acceptable date for the survey will be arranged between the Applicant and ASME. The Applicant is responsible for obtaining the concurrence of the Accredited Authorized Inspection Agency and Enforcement Authority, where applicable, for the survey dates. Cancellation of survey dates is subject to a monetary penalty, deducted from the advance deposit, as follows: $2,000 for cancellation after acceptance of the survey dates. $3,500 for cancellation within 2 months of the survey dates. $6,000 for cancellation within l month of the survey dates. In addition to the above, the Applicant will be charged for any committed travel and miscellaneous expenses associated with the cancelled survey. Note that one complete application set must be submitted for each different plant location and field site. Multiple Certificate and Certification Mark requests at a single location require only one copy of each form. For Applicants for NV Certificates, a list of Testing Laboratories Holding ASME Certificates of Acceptance for Conducting Tests of Pressure Relief Devices is shown as a link on the website. ASME will make every effort to conduct an evaluation of the Applicant's program at the earliest possible date after receipt of the application forms and deposit. However, it should be expected that a lead time of four to six months is usual because of scheduled surveys of other companies. ASME can accept no liability for consequences to the Applicant which may be the result of the survey or subsequent ASME actions. A renewal survey will be scheduled approximately eight to twelve weeks prior to the current expiration date to assure continuity of the Certificates. Certificate Holders have the option to request an extension to their current Certificate expiration date to complete current work. Such a request for extension to complete work in progress must be received, by ASME, at least ninety days prior to the current expiration date. A check for $2,000 per Certificate must accompany the request. The request must contain information concerning order entry dates, order numbers, quantity of each order, item descriptions and the completion dates. A copy of the request must be sent to the Enforcement Agency for the location of the work in progress, and your Accredited Authorized Inspection Agency. The Certificate Holder shall also request their Accredited Authorized Inspection Agency to evaluate their present activity and provide a report to ASME. The evaluation shall include the current effectiveness of the management and quality program relative to what was in place and presented to the ASME Survey Team at the last survey. An extension, if granted, will be for six months, with no further extension. The extension will only cover work listed in the request. A Certificate Holder who voluntarily terminates a survey in progress is not eligible for this type of extension of expiration date. A brochure listing the various sections of the Boiler and Pressure Vessel Code and other ASME publications required for obtaining ASME Certificates is available on our website. Subscription to the indicated sections of the ASME Boiler and Pressure Vessel Code is required for certification to assure that Code users have the latest applicable Code rules. Effective with the issuance of the 2007 Edition of the Boiler and Pressure Vessel Code, the electronic version, purchased from a licensed reseller is acceptable, as well as the published version by ASME. The rules published in the Code are subject to revision from time to time. These revisions are published annually in addenda to the BPV Code. The effective date for which the latest published Code edition and addenda become the current Code edition and addenda is six months after the date of publication of each such edition and addenda. The Applicant or Certificate Holder is not precluded from implementing the latest published Code edition and addenda earlier than the effective date. Additionally, interpretations of the rules are published in supplements to the Code. These addenda and supplements are provided to subscribers, as part of their basic subscription, to assure their awareness of the latest rules, revisions and interpretations. A2.10-2/14(3/4)

12 Two Park Avenue New York, NY U.S.A. tel fax SCHEDULING INFORMATION NEW/RENEWAL APPLICANTS ASME will make every effort to conduct an evaluation of an Applicant's program at the earliest possible date after receipt of the application forms and fees. However, there is an average lead time to schedule surveys, from receipt of completed application and fees of four months due to scheduled surveys of other companies. To minimize costs for Applicants, ASME tries to schedule a series of surveys in the same area of the world. Therefore, in order for ASME to schedule your survey, please answer the following two questions. Please note that if you check as soon as possible for Question 1, or a date much earlier than four months, we will in general only be able to schedule at those times, if another company in your area cancels their date. Question 1 What is the earliest date that you are available for a survey? {Note: Renewal surveys are required to be scheduled approximately 8-12 weeks prior to the current expiration date.} As soon as possible, or Question 2 What weeks or dates are not acceptable to you (due to Holidays, plant shutdowns, etc.) between your first available date and eight months from the date of your application? 15 Two Park Avenue New York, NY U.S.A. tel fax www. asme.org ADDITIONAL INFORMATION TO BE PROVIDED WITH APPLICATION FOR RENEWAL SURVEY The procedures of the Committee on Nuclear Certification (CNC) have been revised to provide some flexibility in establishing the size of Teams for renewal of N-Type Certificates. The procedures state that at the time of application for renewal, the Certificate Holder shall identify to ASME the programmatic changes that have been made and accepted by the ANIS since the issuance or last renewal of the Certificate. We would therefore like a report from you providing information on the programmatic changes that have occurred since your last survey. The report should also cover the following: 1. Significant changes in the organization from those in effect at the time of issuance or last renewal of the Certificate. This could include reorganization, a merger or acquisition, or change in key personnel. 2. Any changes in the QA Manager/Director, including the frequency of change in this position and the experience level of personnel filling this position. 3.
