(Cache County EMS Authority) Request for Bids CARDIAC / DEFIBRILLATOR MONITORS Introduction In order to carry out the responsibility of providing its citizens with the highest practical level of emergency medical services, Cache County EMS Authority (CCEMS) is soliciting bids from qualified providers for portable cardiac Defibrillator Monitors, meeting the specifications contained herein. Proposal Requirements Bids shall be submitted no later than 1700 hrs. On Thursday September 17 th, Sealed bids shall be delivered to: Cache County Executives Office (199 North Main Logan Utah ) C / O Jay Downs Bids shall reflect the best and most current information and offers. Vendors shall provide two (2) copies of each bid. Cache County EMS Authority reserves the right to negotiate with selected bidder prior to the final selection. 2

3 Bids received after the deadline shall not be considered, and will be returned unopened to the proposers. Bids must be submitted in sealed envelopes that are clearly marked to identify the nature of the bid, and the organization or individual submitting the bid. CCEMS Authority reserves the right to reject any or all proposals or bids, to waive any formality or technicality, or to accept proposals or bids deemed to be in the best interest of CCEMS Authority. CCEMS Authority reserves the right to consider all bids based on a best value plan. Lowest price is one factor being considered. All other factors will be considered as well, including but not limited to service, local vendor reliability, demonstration of the working product for such things as twelve lead transmissions, and other factors that may apply. Requirements This request is for bids for fourteen (14) Identical Cardiac / Defibrillator Monitors and all related equipment as outlined herein for the Cache County EMS authority. Please provide a detailed description of the proposal for providing 14 Cardiac / Defibrillator Monitors needed by CCEMS Authority. The vendor shall supply, as a part of their bid package, a copy of the warranty or warranties they propose to provide for each item and in no case shall it be less than twelve (12) months. The vendor shall furnish, as a part of their bid, equipment specifications brochures clearly describing the equipment they propose to furnish. Please provide the unit price, including all options included or required to be included by the bid specifications. The unit price shall reflect every cost to be incurred by the purchaser. 1. PROVIDER INFORMATION a. General Information i. Name and address of the company 3

4 ii. Where the company is incorporated, how it is organized and its reach (i.e., Local, Regional, National) 2. HISTORY a. The responder shall provide a narrative discussing its history as a provider of Cardiac Defibrillators Monitors. 3. REFERENCES a. The responder shall provide a list of three references. 4. ADDITIONAL REQUIREMENTS Scope of Work / Specification Parameters: 5. All equipment and supplies must be new / unused lead, and 12 lead monitoring capability 7. Bi-phasic defibrillation with adult and child defibrillation pads. 8. Capable of being programmed to start in Automated External Defibrillator mode. 9. Non-invasive pacing capabilities both demand and nondemand. 10. Synchronized Cardio version with EKG markers 11. End Tidal carbon dioxide monitoring (EtCO2) 12. Pulse Oximetry (Sp02) 13. Non-Invasive Blood Pressure (NIBP) 14. Batteries- as recommended by provider. A minimum of 2 batteries per unit are required. 15. A total of 4 Multi-battery charging stations shall be included in the bid. 16. Each defibrillator must be capable, and include equipment for 110 AC charger and Mobile 12 V DC charger.(if that option is available-not a requirement). 17. Cases as recommended by provider 18. Supplies and cable to stock each defibrillator, plus a full back up restock- as recommended by provider. 19. Data storage capabilities detail capacity and data management including hardware and software as applicable. 20. Data Retrieval event summary printout 21. Data transmission (scene to hospital) via blue tooth or other comparable technology. Including description of needed 4

5 equipment and or services. This bid shall include ALL costs of ALL equipment required to transmit 12 lead data to receiving medical facilities, except for the cell phones and phone service (if applicable) which shall be the responsibility of CCEMS Authority. 22. Data reception (at hospital) capabilities Provide detailed description of ability to provide data to receiving medical facility including all hardware and software required. All hardware, software and subscription costs shall be included in the bid. If CCEMS Authority is to be responsible for the cost. 23. Data management software package. Customer Service 24. Repair Service specify service plan and response time 25. Extended Warranty in addition to original manufacturer warranty for a total of 5 years coverage (List as deductive bid item). 26. Onsite training; initially; and as upgrades are provided; and as requested. Trade In 27. List Trade in value or the trade in methodology that will be used for trade in for 10 Zoll Defibrillator Cardiac Monitors.. 9 M series and 1 E series. Delivery 28. Delivery shall be made within 60 days of order 5

6 Options to purchase as funds are available (List the cost for each item below as a deductive bid item.) 29 Accessory packs as recommended by Provider 30 Reserve cables and packs 31 CO Detection 31 Invasive Blood Pressure Monitoring SIGNATURE ON PROPOSAL Proposals must be signed by an authorized representative of the company named therein. The signature on the proposal shall be interpreted to signify the provider s intent to comply with all of the terms, conditions, and specifications set forth in the Request for Proposal, unless specific exceptions are clearly noted within the proposal. CRITERIA FOR PROPOSAL EVALUATION Proposals will be judged on the ability of the responder to meet the specific needs of CCEMS Authority and other basis as CCEMS Authority deems necessary. CCEMS Authority will be the sole judge as to which proposal, if any, is accepted. Following the review of proposals, CCEMS Authority, at its sole discretion, may request interviews with, or presentations from any or all responders. CCEMS Authority reserves the right to reject any and all proposals.. CCEMS Authority appreciates in advance the efforts that responders will make and looks forward to participating in the selection process. The selected responder selected will be required to enter into an agreement with CCEMS Authority regarding the terms of the services provided. 6
