4 Food Sample Form The Toronto Congress Centre (TCC) is a practicing HACCP facility adhering to the highest food safety standards. In order to uphold our HACCP accreditation, it is imperative that we monitor and control all food products being served onsite. All exhibitors who wish to dispense food samples at TCC must submit this form in advance of the Event. 5 Food Sample Form TCC Guidelines: 1. TCC holds exclusivity to F & B within the facility. 2. Written approval is required by TCC if looking to distribute food and beverage samples prior to the Event. 3. Distribution of alcohol samples are not permitted without TCC s prior written approval. Event Management must have required permits, insurance coverage and comply with regulations set forth by the AGCO, LLBO and any other necessary authorities. 4. Event Management agrees to indemnify TCC with respect to any liability regarding sampling. 5. Costs or services associated with setup/teardown of equipment, labour, furniture, transportation, power, cleaning, etc. are the responsibility of the distributor of samples. The undersigned agrees to prepare and serve all samples in accordance with the guidelines outlined in the City of Toronto Special Event Temporary Food Establishment package culture/special_events_office/food_vendors/files/pdf/temporar yfood_vendor-package.pdf including, but not limited to, the providing of portable hand washing facilities where appropriate. 7 Live Animal Agreement Here are the guidelines for the request for the presence of (Animal(s)) at (Booth #) booth for the 20th Annual Joint CCI-T/ACMO Condominium Conference November 10, 2016 to November 12, 2016 at The Toronto Congress Centre. The animal must be self-contained and humanly treated within the booth space. This includes provision of water and food; as well the exhibitor is responsible for maintaining the (Animal(s)) in a sanitary fashion, and disposing of the waste appropriately. Any forms of neglect or failure to keep control over the animal(s) will not be tolerated. Failure to meet these guidelines will result in the removal of the animal(s) from the premises. 8 Live Animal Agreement Live animals meeting the following criteria may be permitted in the facility at The Toronto Congress Centre s sole discretion, and by proper execution of this waiver. Certified Service Animals ie: guide dogs, either working or under directed training Animals which are part of an approved Exhibit or professional seminar and licensed for the purpose under applicable Ontario Law Pets, companion animals and service animals not in current service or under directed training are not permitted in the Centre. Permitted animals will be under leash control at all times while in or on Centre grounds, or secured in a cage/kennel or other secure enclosure. Ontario Code applies. The Party executing this waiver accepts full responsibility to determine and obtain any permit or license required under City, Province or Federal Regulation and to bear the full cost of, and hold harmless and indemnify TCC and its related parties from, and loss or damage or injury, or any action or suit arising there from, caused by any permitted animal, to any person or thing in or on Centre grounds, arising from whatever cause or provocation. TCC may in its sole discretion deny entry to, or cause to be removed, any animal; which it finds to be maintained or otherwise treated or behaving in an unsafe, inhumane or unsanitary manner.
