1 Purpose: Policy: Background: The purpose of this policy is to provide a standardized system that is utilized by all fire agencies to track the location, movement and identity of firefighters and companies assigned to a full alarm response or greater. The Personnel Accountability System gives the Incident Commander a fast and efficient means to account for fire/rescue personnel, their assignment, and their status at the scene of an emergency. It is the policy of the San Mateo County Fire Chiefs that all companies involved in joint operations with other agencies at the level of a full alarm or higher will utilize the approved Personnel Accountability System described in this policy. The tracking of fire personnel and companies at an emergency scene is one of the key elements in managing successful operations. As the ICS expands, the need for a personnel and equipment accounting system increases rapidly. The T-Card system is a proven, simple system of personnel and equipment accountability. Procedures: 1. General A. The Company Officer is responsible for the safety and tracking of personnel assigned to their Company. Company Officers shall exert control over all personnel assigned and be prepared to account for their personnel at all times. B. The Incident Commander is responsible for the implementation of the Personnel Accountability System. C. The scene Safety Officer will insure the personnel Accountability System is conducted properly. 2. T-Card System A. Two pink T-Cards are required to be filled out daily (in pencil) at the beginning of each shift. The T-cards shall state the type of apparatus (truck, engine, etc.) apparatus number and names of personnel assigned. The T- Page 1 of 16
2 Cards shall be attached and carried in the holder on the officer s side door. All personnel changes require the T-Cards be updated. B. One T-Card shall be collected at the incident check-in: staging, base, or any other check-in point. The T-Cards collected at the check-in point shall be taken to the command post. Units arriving before establishment of check-in shall have their T-Cards picked up by the command post staff. C. One T-Card is retained by the company officer; the radio pocket is a good place, and given to the Out Team at the entry point. If a rescue is in progress or for any other reason entry is made before the Out Team is set up, the T-Card is left with the engineer of that company. D. T-Card given to the Out Team shall be retrieved and taken with the company if given another assignment (i.e. Rehab or told to use a different point of entry). The Out Team shall report any change in assignments to the command post. E. White T-Cards shall be filled out by all overhead personnel and used in same manner as the pink company T-Cards. F. Extra T-Cards shall be carried in all command vehicles and can be used for tracking other than fire department personnel, i.e., PG&E, AMR, outside contractors, etc. G. For anyone reporting to an incident without a T-Card, a report shall be made to that respective officer s company s fire chief by the incident commander. General Information/Instructions: Resource Status Cards (T-Cards, ICS 219) are used by the Resources Unit to record the status and location of resources, transportation and support vehicles and personnel. When filled-in the T-card rack, the T-cards provides a visual display of the status and location of the resources assigned to the incident. There are eight different Resource Status Cards. Each T-card is a different color and used for a different purpose. Page 2 of 16

Individual Personnel T-Card (ICS 219-5): Initial data concerning personnel resources comes largely from the ICS 211 check-in list. The data to be obtained from the check-in list includes: Three-letter designator Person s name Order and/or request number (if used) Whether or not on manifest If on manifest, person s weight Home base Departure point Method of travel Incident assignment Person s other ICS qualifications Page 3 of 16
4 Record or Display Data Record and display data personnel resources data is recorded and displayed using the white T-cards (ICS 219-5). The data elements include the following information: Agency the three letter designator for a person s home agency. Name the person s name Incident Assignment record the person s current ICS assignment Order/Request No. if used by the person s home agency. Record the order and/or request number assigned to that individual. Date/Time Check-in the date and time the person checked-in to the incident. Home base the location of the person s home base. Departure Point for use primarily in recording information on personnel resources that are enroute. Record the location from which the person is dispatched and the approximate time of dispatch. Method of Travel record the method of travel used by the person to get to the incident. On Manifest check whether the person is on manifest. Weight If a person is on a manifest, record their weight. Transportation Needs leave this space blank for use by the Demobilization Unit. Date/Time Ordered leave this space blank for use by the Demobilization Unit. Date/Time Confirmed leave this space blank for use by the Demobilization Unit. Destination Point leave this space blank for use by the Demobilization Unit. Remarks record the person s other ICS qualifications in this space. Incident Location the ICS location of the individual (e.g., Division A, Base, Staging Area, etc.). Page 4 of 16
5 Time the time of day and date, which the current status was recorded. Status check the appropriate status for the individual as follows: Assigned Available O/S rest O/S mech. O/S pers. ETR Currently working Ready for assignment, can respond within three minutes. Out of service for rest. Out of service for mechanical reasons, (i.e., stranded with a broken down vehicle, out of service with a broken radio, etc.). Not applicable to personnel resources. (Use for equipment when there are no personnel to operate the equipment.) If the individual is out of service, enter the time they are expected to return to service. Note This space is for additional information pertinent to the individual s status. It should be noted that the other side of the T-card is used to record changes in the status of an individual. The date elements are recorded as described above. Each time the individual changes status, that change is to be recorded on the individual s T-card. If you do not have complete personnel information, fill out a T-card with the individual s name, home agency, and incident assignment. Keep a list of personnel for whom the Resources Unit does not have complete information as a reminder to obtain the required data later, as incident activity decreases and attention can be shifted to obtaining missing information. 9 Engine Company T-Card Instructions: When filling out a T-card for a single resource, the minimum information needed to start the T-card is the agency designator code, the kind and type of resource, and the resource identification number. Figure 4 shows the T-card format, which is used for the rose, yellow, and tan T-cards. The information that is recorded on this T-card includes: Agency: ST or TF: Kind/Type: The three-letter agency designator code This block is used only for Strike Team or Task Force For single resources, the resource kind code followed by the resource type code. The kind code used on rose-colored T-cards would be E, indicating engine resources. The kind code used on yellow T-cards would be D, indicating dozer resources. The kind codes that might be used on ten T-cards would include T for truck company, BP for brush patrol, WT for water tender, and DT for dozer tender. On the rose and yellow T-cards (for engine and dozers respectively) the kind code would be followed by a type code of 1, 2 or 3. For the tan T-card, the kind code of BP or DT would not be followed by a type code designator. If the resource is a type code designator. If the resource is a Strike Team or Task Forces, leave these spaces blank. Page 9 of 16
10 ID.NO: Order/Request No.: The resources identification number. If the resource is a Strike Team, the ID number is followed by the kind/type codes for the resource. Order or request number assigned to the resource by the agency dispatch center. Date/Time Check-in: The date and time the resource checked-in to the incident. Home Base: Departure Point: Leader/Name: Resource ID (No./Names): Destination Point ETA: Remarks: Location: Time: Status: The location at which the resource is based. The location, from which the resource was dispatched, followed by the approximate time of departure. The name of the person in charge and number of personnel with that particular resource. If the resource is a Strike Team or Task Force, list the Resource Designators for all of the component resources. This is for Demobilization Unit. For use in documenting additional information pertinent to that resource or it s status. The current organizational or geographic location of the resource. The time and date, which the location was reported. Check appropriate status: Assigned -- Check if the resource is currently assigned and working the incident. Available -- Check is the resource is not currently working the incident but is available for assignment and can respond within three minutes. O/S Rest -- Check if the resource is out of service for rest purposes. O/S PERS -- Check if the resource is out of service for personnel Page 10 of 16
11 reasons (e.g., insufficient manning, illness, injury). ETR -- Enter the estimated time of return to service. Note: Record other information pertinent to the status of the resource such as the nature of mechanical failures, etc. The rest of the T-card is used to record changes in the status of the resource for which the card has been completed. Status Changes The primary function of the Resources Unit in the Incident Command System is to supply the Incident General Staff information on the resources assigned to the incident. To provide this service it will be necessary to maintain current status on all personnel and resources assigned. The T-card (ICS 219) provides spaces for recording current status, as well as status changes, for that resource. Figure 12 is an example of that portion of a T-card used for status keeping. Status Information Incident Location: Where on the incident is this resource presently located, (e.g., Division B, Incident Base, Staging Area, Helibase). Time: At what time and date did this resource report to that location/assignment? Status: You will notice that although there are several items listed, there are only three general categories. A resource is either considered assigned, available, or out-of-service. The additional items identify more specifically why a resource is out-of-service. For example: O/S Rest out-of-service for rest O/S Mech out-of-service for mechanical reasons O/S Pers out-of-service for personnel reasons The ETR space is used for recording estimated time of return for out-of-service resources if this information is available. 12 Note: Use this space to document additional information pertinent to that resource or its status. Example: Can be reached at Diddy Wells Forest Fire Station Obtaining Status Information: There are several sources of information regarding the status of incident resources. Status change cards, and check-in lists are the most common sources. However, it may be necessary to obtain this information through direct communications with other incident personnel. Page 12 of 16
