3 The Multiple Patient Management Plan
4 MPMP Components Level 1 may be referred to as a Multiple Victim Incident (MVI) or a Multiple Casualty Incident (MCI) Level 2-3 may be referred to as a Multiple Casualty Incident (MCI) Level 4-5 may be referred to as a Mass Care Incident.
5 Local Countywide Alerts
6 Local Alerts Local Alerts are called by a public safety jurisdiction. Launches departments actions rather than County actions. Each Department must develop an internal procedure for a Local Alert.
7 Local Alerts Early notification of events occurring within the County. Hospital notification via EMResource. Notification made via CAHAN to partner organizations and PSAPs. Addition of a CAD-based messaging system will be the priority alerting system for provider organizations.
8 Countywide Alerts May be requested by anyone executed by the EMS Agency. Goal is to ensure 911 System stability and support a single or multiple incidents.
9 Countywide Alert Examples Earthquake, flood, fire, etc. Credible threat that could result in illness or injury. Overwhelming EMS call volume Public Health events Critical infrastructure failure
10 Activation Levels Activation 1-2 Tactical Focus at the local level with support from the County. Activation 3-4 Operational Focus at the County Level with tactical local level activity. Activation 4-5 Strategic & Policy Level Focus at the County level with the same at the local level in addition to operational and tactical actions.
11 Level 1 Activation Local Jurisdiction Multi- Victim Incident Up to (4) 911 System Ambulances Less than 10 Anticipated Patients that require ambulance transport. Diversion applies Automatic dispatch of 3- ALS Ambulances Code 3. Automatic dispatch of 1- EMS Supervisor Code 3. EMS Duty Chief monitors and manages the 911- System.
12 Level 1 Example Typical EMS event has 1 ambulance assigned. Local Jurisdiction initiates Level 1 Activation upon arriving on scene. 3 additional ambulances and 1 supervisor are dispatched to the incident. For a total of 4 ambulances = 4
13 Level 1 Example Local Jurisdiction initiates Level 1 Activation upon evaluating call details and prior to dispatch of initial ambulance. 3 ambulances and 1 supervisor are dispatched to the incident. More ambulances can be requested if needed = 3
14 Level 2 Activation Local Jurisdiction Multi- Casualty Incident Up to (10) 911 System Ambulances Less than 20 Anticipated Patients that require ambulance transport. Diversion may apply Automatic dispatch of 6- ALS Ambulances Code 3. Automatic dispatch of 1- EMS Supervisor Code 3. EMS Duty Chief monitors, ensures management the 911-System, and/or may respond.

18 Standard Dispatch Orders
19 Standard Dispatch Orders The following actions may be implemented in order to ensure the stability of the Santa Clara County Emergency Medical-Health Services System. These orders may be executed by the EMS Agency, County Communications Watch Commander, and authorized agents of the Public Health Department.
20 Standard Dispatch Orders SDO 1: Confirm EOA contractor s low ambulance levels & high system volume management actions are in place. SDO 2: Alert EMS Command Staff to monitor command 92 for instructions or information. SDO 3: Suspend all hospital ring-downs on command 92 - Allocate for countywide EMS command and control communications.
21 Standard Dispatch Orders SDO 4: Alert all EMS system units. SDO 5: Alert all private ambulance dispatch centers. SDO 6: Alert all hospitals. SDO 7: All hospitals ordered open.
22 Standard Dispatch Orders SDO 8: Discontinue all PCRs and replace with triage tags. SDO 9: Conduct and EMS system resource roll call - determine status and welfare. SDO 10: Place all available EOA and Non-EOA ambulances in service.
23 Standard Dispatch Orders SDO 11: Notify San Jose, Santa Clara City, and Gilroy Fire Departments that unrestricted use STAR Units have been authorized with their jurisdictions (Non system-wide use). SDO 12: Notify San Jose, Santa Clara City, and Gilroy Fire Departments that unrestricted use STAR Units have been authorized and units may be used system-wide.
24 Standard Dispatch Orders SDO 13: Dispatch BLS ambulances to Alpha, Bravo, and/or Code 2 EMS events. SDO 14: All ambulances responses are handled Code 3. SDO 15: All ambulance transports to the hospitals to be handled Code 3.
25 Standard Dispatch Orders SDO 16: Ambulances shall transport to the closest open emergency department. SDO 17: Automatic ambulances dispatches are suspended until a first response unit arrives on-the-scene and verifies that a patient needing emergency transport exists.
26 Standard Dispatch Orders SDO 18: Ambulance dispatches to Alpha, Bravo, and/or Code 2 EMS calls are suspended. SDO 19: PSAPs may discontinue use of pre-arrival instructions (PAI). SDO 20: PSAPs may discontinue use of emergency medical dispatching (EMD) procedures.
27 Standard Dispatch Orders SDO 21: Patient routing directed by the County. SDO 22: Move all ambulances to muster stations. SDO 23: Level 3 activation of the multiple patient management plan.
28 Standard Dispatch Orders SDO 24: Level 4 activation of the multiple patient management plan. SDO 25: Level 5 activation of the multiple patient management plan. SDO 26: CHEMPACK request notification
