Likelihood Safe use and operation of Dock Levellers BBW & BRW Risk Assessment Identification of Hazard: Are significant HAZARDS involved in the operation? YES Hazards concerned with: 1. Slip trip fall 2. Entrapment 3. Vehicle movement 4. Communication Anything that creates a seal around the back of a goods vehicle can also reduce the communication that takes place between the delivery driver and site workers, which can introduce additional risk. Risk Assessment Matrix Certainty Probable Possible Remote Improbable Harm Potential Risk level Negligible Minor Serious Major Fatality 1-4 Low 5-12 Medium High Assessment of Risk: What is the likelihood of the RISK OF INJURY (risk rating) regarding the hazards described above? Harm 4 X Likelihood 4 = 16 Who is at risk? Operator and Pedestrians in immediate area Continued on next page 1 Issue
Remedial Action: What controls are in place to reduce the risk? 1. Good Housekeeping around Bay doors 2. Operator awareness through full training 3. Removal of vehicle keys whilst unloading / loading 4. Any communication with driver to be made face to face 5. On- going observations to make sure procedure are being followed What is the likelihood of the RISK OF INJURY (risk rating) following implementation of control measures? Harm 4 Likelihood 3 Risk rating = 12 MED Training To be trained in all aspects including dangers in this SOP. On-going observations to ensure procedures are being followed 2 Issue
The vehicle driver must reverse the vehicle up to the loading dock (the external bay traffic light should show green. The driver must ensure that the vehicle/trailer is parked fully up to the bay buffers and that it is positioned squarely on to the dock. NOTE: - if the trailer is equipped with Barn door style rear doors they must be fully opened prior to reversing to the dock. The driver must then secure the vehicle and remove the keys (locking the cab). On entering the loading bay the driver will place the keys in the tray or on the hook provided on the bay door. Ensure that the bay area is clear of any debris, equipment and personnel. Using the panel to the right of the door open the door Top = door up Middle = emergency STOP Bottom = door down The internal control panel lights will change from red to green and the external bay light will change to red (indicating that the vehicle must not be moved). 3 Issue
BBW To the left of the set of three buttons is a single button, which operates the dock leveller. To operate: - Press the button and keep it depressed until the dock leveller plate is fully extended, release the button and the dock leveller plate will lower onto the rear of the vehicle. Ensure that the dock leveller plate has a good overlap onto the vehicle. This should be a minimum of 4''. Never walk or drive onto a dock leveller unless the plate is correctly positioned as above. Be aware of the gap/drop to each side of the dock leveller when it is placed onto a trailer. Once the trailer has been unloaded/loaded the dock leveller must be relocated: - Press the dock leveller button (single one to the left of three others) and keep it depressed until the dock leveller extension plate drops to the vertical position, release the button and the dock leveller will relocate to its original position flush with the loading dock. If the vehicle has a roller shutter door, it should then be closed and any seal recorded if required. Close the bay door (bottom button, internal lights change from green to red, external bay lights change from red to green) Note when closing the bay door the button needs to remain pressed until fully closed (release the button in an emergency and door will cease closing instantly).. The driver may then take the keys from the box/hook. 4 Issue
When you have finished with the bay ensure that all personnel, stock, equipment and debris are removed. NOTE: - There is an isolation switch above and to the right of the control panel (used to isolate the system for maintenance or lockdown purposes). RIVERSWAY DOCK LEVELLER OPERATION Press and release top right green button (A) - and the white light above will flash (This Inflates air curtain around vehicle) When light stops flashing (Solid white light)- Press and release Door open button (1) and wait until door is fully open Press Leveller raise button (5) - Press until leveller height is above vehicle floor Press Lip Forward button (6) - Press until plate has extended into vehicle body a minimum 4".
