This Tariff contains regulations, rates and charges applicable to the provision of Carrier Common Line, Switched Access Services, Special Access Services, and other miscellaneous services, hereinafter referred to as service(s), provided by the Telephone Companies listed below, hereinafter referred to as the Company, to Interexchange Carriers, Resellers, Telephone Companies and any other entity authorized to order service out of this Tariff, hereinafter referred to as IC(s). The services identified in this Tariff, with certain exceptions which are identified as follows, are also available to other customers, subject to the same constraints and limitations that are applicable to certified interexchange carriers Automatic Number Identification 2. Billing and Collection Services B. The provision of such services by the Company as set forth in this Tariff does not constitute a joint undertaking with the IC for the furnishing of any service. C. The regulations, rates, and charges contained herein are in addition to the applicable regulations, rates, and charges specified in other tariffs of the Company which are referenced herein. D. The regulations, rates and charges contained within this Tariff are not applicable to Radio Common Carriers, except for those Cellular Mobile Carriers who receive additional authority (certification) from the North Carolina Utilities Commission to provide Wide Area Call Reception Service for intercgsa traffic. The regulations, rates and charges for Wide Area Reception Service are contained in Section I17. of this Tariff. I1.2 Concurring Carriers 2 A. The following North Carolina Telephone Companies concur with the provisions of this Tariff for the provisioning of intrastate access services: 1. Windstream Concord 2. Ellerbe Telephone Company 3. MEBTEL Communications 4. North State Telephone Company d/b/a North State Communications 5. Randolph Telephone Company B. The following North Carolina Telephone Companies concur with the provisions of this Tariff for intrastate Access Services with the exception that different rates, applicable to Switched Access and Special Access Services, will be found in I6.9, I7.6, I7.7 and I7.8 of this Tariff. Rates and charges in I6.9, I7.6, I7.7 and I7.8 are based on the interstate rates in effect as of May 31, Concord Telephone Company concurs in I6.9 for Local Transport. 1. For usage sensitive (i.e., access minutes and calls) chargeable rate elements, multiply the percent intrastate use times the actual use (i.e., measured or Company assumed average use) times the stated tariff rate. The intrastate percentage will change as revised jurisdictional reports are submitted as set forth in I preceding. I Trouble Determination A. The IC shall be responsible for receiving trouble reports from its end users and determining, by testing or other means if the source of trouble is caused by the equipment or facilities provided by the IC, its end user, or by the Company. B. When more than one Company service is used to provide service to an IC, the IC is responsible for determining which service provided by the Company is causing the reported trouble. C. The IC shall be responsible for payment of Maintenance of Service Charges as set forth in I following when reporting trouble to the Company and the obligations in A. and B. preceding are not met. I Reserved for Future Use I Reserved for Future Use I Number Provisioning The provision of 800 Access Ten Digit Screening Service, provided from Section I6. following, requires the IC's subscription to basic 800 Number Service features found in Section I13. of this Tariff or as an alternative the provision of those features by other responsible organizations or through direct access by the IC to the Service Management System. I Switched Access Credit A. As ordered on May 17, 1994 in Docket No. P-100, Sub 126, Defined-Radius Discount Calling Plan, and on February 9, 1994 in Docket Nos. P-141, Sub 19 and P-100, Sub 65, and Sub 72, the following Companies will credit the IC the difference between the average revenue per minute for (1) MTS, Defined-Radius Plans, Defined-Area Plans, DDD, OCPs, Saver Service, WATSSaver and any future services of a similar nature in the aggregate; (2) WATS; or (3) 800 Service; and the average intrastate calculated composite switched access rate. The Company will recalculate and change, if necessary, the credit applicable to IC traffic when any switched access rate element changes, once a proposed change in the price of one of the existing toll services becomes effective, or on January 1, 1995 and July 1, 1995 and on July 1 of each year thereafter. The credit will be given as long as the intrastate switched access charges exceed the average intralata toll rate. 1. BILLING AND COLLECTION SERVICES I8.1 Recording Service (Cont'd) I8.1.6 Rate Regulations A. For each IC message recorded, the recording and the assembling and editing charges apply except when the IC orders Message Processing Service. When Message Processing Service as set forth in I8.2.1 following is ordered for North Carolina and for the same month that Recording Service is ordered, the assembling and editing charge does not apply per IC message. The charges for recording and for assembly and editing apply per message recorded and per message assembled and edited whether or not the IC's schedule of rates specifies billing on a per message basis or any other basis. B. The per Special Order charge applies for each Special Order accepted by the Company for Recording Service. C. When message detail is entered on a data file or magnetic tape for provision of message detail to an IC, the per tape charge applies for each data file or magnetic tape prepared, and the per record charge applies for each record processed. A record is a logical grouping of information as described in the programs that process the information and load the magnetic tapes or data file. The Company will determine the charges based on the number of data files or magnetic tapes prepared and on its count of the records processed. The number of records processed will be determined using the number of records input to or the number of records output from the programs that process the information and load the magnetic tapes or data file, whichever number of records is higher. The Company will not render bills for IC private line services for which it does not provide Special Access Service that are connected at end user premises to IC private line services for which it does render bills. The Company will not render bills for usage-based rate elements under this section of the Tariff. Usage-based rate elements will be billed as set forth in I8.2.1 preceding. A. General Description Private Line Billing Service includes editing and rating, account establishment, rendering of bills, receiving payments, maintenance of accounts, treatment of accounts and inquiry (when ordered by the IC). Editing and rating is the examination and identification of all the rateable elements of an IC private line service and the application of the appropriate IC rates and charges to the service. These functions are performed when private line services for an IC's end user are established or changed. Rating is always performed, and editing may be performed, coincident with the implementation of a change in the IC's schedule of rates. Note 1: These rates are applicable to Windstream Concord, North State and MebTel Note 2: Rates and charges based on individual cases (ICB) will apply.

BILLING AND COLLECTION SERVICES I8.6 Clearinghouse Agent Provisions These provisions are available to the following Telephone Companies who are capable of complying with the requirements specified herein: Barnardsville Telephone Company Citizens Telephone Company Windstream Concord Ellerbe Telephone Company Windstream LEXCOM MEBTEL Communications North State Telephone Company d/b/a North State Communications Pineville Telephone Company Randolph Telephone Company Saluda Telephone Company Service Telephone Company I8.6.1 General Description A. For the purposes herein, the Company will provide billing and collection services to clearinghouse agents who are authorized to act as agents for certified ICs. All other access services will be limited to those entities which have been certificated and authorized by the Commission to provide intrastate long distance telecommunication service. When a clearinghouse agent acts as an agent for ICs, the clearinghouse agent will comply with the same requirements and obligations that apply to ICs under this Tariff. The Company will also provide billing and collection services to clearinghouse agents who are authorized to act as agents for certified Payphone Service Providers provided that the Payphone Service Providers comply with the following requirements: - Only automated collect calls may be billed. Automated collect calls are calls placed and billed to the called telephone number without the assistance or intervention of a human operator. The Payphone Service Providers must apply for and receive specific authority to employ automated collect telephones; - Collect calls will not be billed which have not been actively accepted by the called party (e.g., calls made from automated sets that default to billing to called party if called party does not disconnect within a given time period); - All requirements and obligations that apply to ICs under this Tariff will apply; - Payphone Service Providers can only purchase billing and collection services through clearinghouse agents; and - The name and telephone number of the Payphone Service Provider must appear on the end user's bill. The Payphone Service Provider must perform its own inquiry and may not purchase it from the Company or the clearinghouse. I8.6.2 Undertaking of the Company A. Prior to the provision of intrastate billing and collection service by the Company to a clearinghouse agent, the clearinghouse agent must provide to the Company a copy of the intrastate certification for each IC or Payphone Service Provider on whose behalf the clearinghouse agent is receiving billing and collection services. 3. Prior to the provision of intrastate billing and collection services by the Company to a clearinghouse agent, the clearinghouse agent must provide to the Company a copy of its agency agreement with each IC or Payphone Service Provider on whose behalf the clearinghouse agent is receiving billing and collection services. 4. Each message sent to the Company by the clearinghouse agent must be identified by the appropriate IC's or Payphone Service Provider's Carrier Identification Code (CIC); additionally, the clearinghouse agent must supply its own identifying CIC. 5. In the event that a clearinghouse agent ceases to provide intrastate billing and collection services to an IC or Payphone Service Provider, the clearinghouse agent must inform the Company immediately.

INDIVIDUAL LOCAL EXCHANGE COMPANY (LEC) ACCESS SERVICE TARIFF OFFERINGS I15.1 General Description Services provided in this section of the Access Service Tariff are LEC specific tariff offerings. The LEC providing the service offering identified in this section will be solely responsible for the provision of such service, including billing, customer notification and customer contact. The LECs that concur in other sections of this Tariff, unless otherwise noted in this section, do not offer or concur in any LEC specific tariff offerings in Section I15. I15.2 Central Telephone Company - Public Station - Two Digit Access Code I Service Description A. Public Station - Two Digit Access Code is a service whereby an interexchange carrier (IC) can subscribe to abbreviated access dialing on public coin pay stations. The abbreviated dialing enables the end user to access an IC's service without dialing the IC's prescribed access code. B. Each IC subscribing to this service will be assigned a two digit access code. Dialing Instructions will be posted at each public coin pay station location having Two Digit Access Code Service. C. The sequence for listing participating IC names on the instruction cards located at each set will be determined by the Company. D. The minimum service period for Two Digit Access Code Service is one year. E. Nonrecurring charges will be applicable to each IC's initial establishment of Two Digit Access Code arrangement. Changes and additions to Two Digit Access Code arrangements after the initial establishment shall also be subject to the applicable business nonrecurring charges for service order, line connection and premises visit per public station as described in the Company's General Subscriber Service Tariff. Windstream Concord listed in I1.2 preceding will make FGB with an Abbreviated Dialing Arrangement (ADA) available in certain Company-designated end offices. Such FGB with an ADA will be provisioned in the same manner in which FGB is provisioned with the exceptions described following. When FGB with an ADA is made available in an end office, the Company will continue to make FGB with an associated 950-1XXX or 950-0XXX access code available to customers at non-premium rates. When FGD becomes available in an end office, FGB with an ADA will no longer be provided at that end office. B. FGB Switched Access Service with an ADA (FGB ADA) is available to all customers, other than providers of MTS/WATS, from Company-designated non-equal access end offices. Revenue Stability Fund (RSF) (Cont'd) If a RSF recipient files for an intrastate rate proceeding, the RSF recipient will include the RSF subsidy as a toll revenue loss in the requirement underlying the rate request. The new rates allowed by the Commission will be used to reduce the RSF subsidy. The LECs will obtain funding for the RSF by implementation of a surcharge on all intralata toll MOU. 1 All Companies that originate and/or terminate toll traffic in the LATA will participate in the RSF, with exception of an LEC that declines a subsidy from the HCF where the amount of subsidy waived is greater than its estimated contribution to the RSF. J. Maintenance of Records Each Company will keep records of its transactions relating to the payment of access-based compensation amounts in reasonably sufficient detail to permit the other LECs, by review or audit, to verify the accuracy and reasonableness of payments. Each Company agrees to cooperate in reviews or audits performed by or on behalf of the other LECs and further agrees to jointly review the findings of such reviews or audits in order to resolve any differences of opinion concerning the findings thereof. Each Company, whether or not in connection with a formal review or an audit, shall provide the LECs with reasonable access to the records it has maintained and to other relevant data within its possession relating to the ITORP compensation amounts paid and received.
