1 SECTION CONTRACT C1 & C2, ADDENDUM #3 Addendum No. 3 Issue Date: December 5, 2016 Project Name: Noble County Courthouse HVAC Improvements Project Owner: Noble County Commissioners Contract: Contracts C1 and C2 Reminders: 1. The bids are due at 11:00 a.m. Thursday, December 8, General Items: 2. If needed to store and stage equipment, the Owner will rent storage space in/near Caldwell. 3. The Owner will contract for asbestos testing and abatement under a separate contract. The abatement will be completed in the applicable areas before the contractors begin work. Contractors shall identify items in the area of work and mark locations where holes will be drilled. 4. The cost for the job site trailer shall be listed as an alternate on the attached bid form. 5. No fire caulking is specified around the pipe and conduit penetrations through the walls and ceilings. 6. Contractors shall provide all materials and work necessary to provide a completed project and functioning HVAC system. Contract C1 Hot Water Boiler System: 7. Contractor shall clean and flush hydronic system piping with Sentinel X300 or equal. 8. A 5 gallon, 125 psi chemical shot feeder and associated valves shall be installed on the boiler system (A&F Machine Products Co. or equal). Water shall be treated with Sentinel X100 or equal. 9. Each NTH 285 boiler will require an Axiom Industries Ltd. # A neutralization kit installed on the condensate drain from the boiler. Install an overflow wye fitting to the drain. 2 10. A ½ ASSE 1013 back flow preventer shall be installed on the make-up fill water line to the boilers in place of the check valve shown on drawing M16. Contract C2 VRFZ Heat Pump System: 11. For the cabinet enclosures around the piping, where the piping does not extend across the entire length of a wall, the cabinet enclosure shall still be extended to the end of the wall to provide a continuous bulkhead across the ceiling. 12. As an alternate on the attached bid form, price the cost to supply and install new three-phase wiring from a new panel on the 1 st floor to the alternate dedicated outside air unit. The existing electrical conduit may be reused. Circuit breaker shall be minimum 40A, maximum 45A. Wire shall be 4C. 8AWG copper THWN with ground. 13. No additional insulating work shall be required in the skylight (Delete item in Scope of bids 2.02 H.) The existing insulation is pealed back in a few places between the ceiling joists. Contractor shall position this insulation to provide a continuous insulation layer. Addendum #3 includes the following additional information: 14. 3 NC-Series CONDENSATE NEUTRALIZATION KIT INSTALLATION, OPERATION AND MAINTENANCE INSTRUCTIONS Installation Instructions NOTE - Check with your local water authority for regulations regarding discharge of treated condensate to the drain or sewer system. WARNING - Risk of damage to appliance. The neutralization kit inlet and discharge must be at a lower elevation than the condensate drain from appliance. - Do not allow exhaust flue gases to vent through the neutralization kit. All condensate drains must have a trap to prevent flue gas leakage. Flue gas leakage can cause injury or death from carbon monoxide. - Connection to the appliance and neutralization kit must be installed to ensure that no condensate backflow into the appliance can occur. The inlet has a centre connection port and the outlet connection is off centre. Mount the neutralization capsule on the wall or floor securing it with the provided brackets. When mounting capsule in the horizontal position rotate the tube so the outlet is at its highest point (Figure 1 and 2). When mounting in the vertical position ensure the outlet is at a higher elevation than the inlet (Figure 3). The preferred mounting method is in the horizontal position. 4 FEEDER INSTALLATION INSTALLATION The feeder should be installed as close to the boiler or equipment being treated as possible. Figure 1 and Figure 2 are typical installations. Install the feeder in accordance with local plumbing codes or ordinances. FLUSHING THE FEEDER Backwash and flush out the feeder prior to every refilling. Close all valves around the feeder, including the flow regulator valve if using a by-pass feeder. Open the drain valve and inlet valve to flush the feeder. Close the drain valve and the inlet valve. If using a by-pass feeder, open the outlet valve and the flow regulator valve completely. Note the setting of the flow regulator valve so it can be returned to this setting after flushing. Open the inlet valve to flush the regulator valve. Close the inlet and outlet valves and return the flow regulator valve to the previous setting. FILLING AND OPERATION Close the inlet and outlet valves. Open the drain valve and the fill valve to completely drain the feeder. Close the drain valve. Open the air bleed valve. Fill the feeder through the funnel with the chemical treatment solution. When the feeder is full close the air bleed valve and the fill valve. Open the inlet valve and the outlet valve. The chemical treatment solution will be fed to the boiler or feed system in one slug if using a shot feeder or regulated if using a by-pass feeder. If the feeder has been installed as in Figure 2, the throttling valve will need to be adjusted to force liquid flow through the feeder.

6 SECTION REVISED BID FORM ARTICLE 1 BID RECIPIENT 1.01 This Bid is submitted to: Noble County Commissioners Office, Courthouse Room 210, Caldwell, Ohio The undersigned Bidder proposes and agrees, if this Bid is accepted, to enter into an Agreement with Owner in the form included in the Bidding Documents to perform all Work as specified or indicated in the Bidding Documents for the prices and within the times indicated in this Bid and in accordance with the other terms and conditions of the Bidding Documents. ARTICLE 2 BIDDER S ACKNOWLEDGEMENTS 2.01 Bidder accepts all of the terms and conditions of the Instructions to Bidders, including without limitation those dealing with the disposition of Bid security. This Bid will remain subject to acceptance for 90 days after the Bid opening, or for such longer period of time that Bidder may agree to in writing upon request of Owner. ARTICLE 3 BIDDER S REPRESENTATIONS 3.01 In submitting this Bid, Bidder represents that: A. 7 of construction expressly required by the Bidding Documents; and (3) Bidder s safety precautions and programs. E. Based on the information and observations referred to in Paragraph 3.01.E above, Bidder does not consider that further examinations, investigations, explorations, tests, studies, or data are necessary for the determination of this Bid for performance of the Work at the price(s) bid and within the times required, and in accordance with the other terms and conditions of the Bidding Documents. F. Bidder is aware of the general nature of work to be performed by Owner and others at the Site that relates to the Work as indicated in the Bidding Documents. G. Bidder has given Engineer written notice of all conflicts, errors, ambiguities, or discrepancies that Bidder has discovered in the Bidding Documents, and the written resolution thereof by Engineer is acceptable to Bidder. 1. The Bidding Documents are generally sufficient to indicate and convey understanding of all terms and conditions for the performance of the Work for which this Bid is submitted. ARTICLE 4 BIDDER S CERTIFICATION 4.01 Bidder certifies that: A. This Bid is genuine and not made in the interest of or on behalf of any undisclosed individual or entity and is not submitted in conformity with any collusive agreement or rules of any group, association, organization, or corporation; B. Bidder has not directly or indirectly induced or solicited any other Bidder to submit a false or sham Bid; C. Bidder has not solicited or induced any individual or entity to refrain from bidding; and D. Bidder has not engaged in corrupt, fraudulent, collusive, or coercive practices in competing for the Contract. For the purposes of this Paragraph 4.01.D: 1. corrupt practice means the offering, giving, receiving, or soliciting of any thing of value likely to influence the action of a public official in the bidding process; 2. fraudulent practice means an intentional misrepresentation of facts made (a) to influence the bidding process to the detriment of Owner, (b) to establish bid prices at artificial noncompetitive levels, or (c) to deprive Owner of the benefits of free and open competition; 3. collusive practice means a scheme or arrangement between two or more Bidders, with or without the knowledge of Owner, a purpose of which is to establish bid prices at artificial, non-competitive levels; and C-410 BID FORM

8 4. coercive practice means harming or threatening to harm, directly or indirectly, persons or their property to influence their participation in the bidding process or affect the execution of the Contract.
