Dan Law, (231) Walters and Hemming Scope of Services: Phone: Fax: We have been asked by our client, Charter Township of Garfield to provide this request to each of the local mechanical contractors and request for proposal to complete the following work: GFA Job No C Page 1 of 4

2 The project would consist of providing evaluation and implementing corrective repairs to the Township s existing boiler system that is used to generate the Township Hall s sidewalk snow melt system located at 3848 Veterans Drive. The existing boiler system has been maintained over the years by Temperature Control, Inc. and has experienced operational issues over the years. The intent is to replace the existing boiler with a high efficiency unit and respective system pump. The existing boiler is a Water Tube type, Raytherm TM manufactured by Raypak and built in 2004 and installed in The scope of work for this project is to remove the existing boiler (standard efficiency), and associated pump, piping and connections to facilitate installation of a new high efficiency condensing boiler and pump. The information contained below are the specific qualifications each contractor must meet in order to provide an accurate proposal. Attachments including system photos, Certificate of Boiler Inspections, Inspection Reports are included for reference. Requirements - General: Work must comply with all applicable laws, regulations All equipment shall be installed compliant with manufacturers recommendations Contractor shall be responsible for obtaining all local regulatory permits (including fees) which may include plumbing, electrical, mechanical and building. Date of completion to be within 15 days of equipment delivery as coordinated with the Township Engineer and Township Staff. Provide in addition to all other equipment factory warranties, a (1) year full labor and material warranty on all workmanship, material and equipment furnished for this project. Contractor shall be responsible for researching, obtaining and processing of all applicable rebates (utility and/or regulatory) on behalf of the Township. This amount shall be reflected in the price of the RFP. Prospective bidders may conduct a site visit prior to bidding although not required. Coordinate with the Township for site access: o Bill Mouser, Garfield Township - (231) Terms of Agreement: General: To hold bid open for 60 consecutive calendar days from the bid due date To enter into and execute a contract with Charter Township of Garfield Insurance: Contractor will have Worker's Compensation Insurance in limits required by state law and Comprehensive General Liability Insurance coverage in force for all of its operations under this contract Bonds: The Contractor shall include in the proposal price the cost to provide the following: Maintenance and Guarantee Bond in the amount of 50% of the proposal amount, guarantying for a period of one (1) year from final acceptance of the project work GFA Job No C Page 2 of 4

3 Letter of Surety, licensed to do business in the State of Michigan, stating ability to obtain a Performance Bond, and Labor and Material Bond for 100% of the project amount. Shop Drawing / Operation and Maintenance Submittals: Provide one hard and one pdf copy of material specification sheets, O&M manuals and warranty information to Township Engineer. Do not proceed until written approval is received Services / materials to be provided: Contractor shall provide all equipment and materials necessary to complete the work described herein. The scope of work shall include but shall not be limited to the following. Refer to the attached Specification Section for additional information. Include and subcontract all necessary electrical work and temperature control work. Demolition and disposal of existing boiler and associated venting, controls, etc. Procurement and installation of new boiler and associated components. New boiler venting shall be coordinated with Township. Any roof work, wall work, patching, etc., shall be included. Boiler controls, and interconnect with existing radiant and snowmelt controls. Provide factory authorized start-up of new boiler. Provide test and balance of new boiler system and existing radiant and snowmelt system. Include minimum two (2) hours of Owner s training on new boiler system and controls. Mobilization, site restoration and cleanup Coordination of delivery and unloading of new equipment Services / Materials Not To Be Included: Site accessibility (provided by owner). Contractor shall be responsible to coordinate and provide construction schedule and minimum 24 hour notice before completing work. GFA Job No C Page 3 of 4

4 Contractors Proposal Form Bidders are instructed to submit bids for this project on a lump sum basis as stated in the Proposal. All bid items are tax inclusive. 5 SnowMelt Boiler Replacement Garfield Twp Hall SECTION SUMMARY OF WORK SUMMARY: Work includes the following: The project includes the removal and replacement of a gas fired boiler used for snowmelt. The installation shall be allinclusive, including all electrical wiring, conduit, piping, temperature controls, water balancing, and removal and discard of any indicated equipment, etc. for a complete and functional system. The work shall meet all current codes and specifically includes the following: 1. Removal of existing Raypak HI-0330 gas-fired boiler and associated venting and accessories. 2. Installation of new Lochinvar FTXL 399 boiler or approved equal with unit supplied 30 GPM boiler primary pump. 3. Venting of new boiler with new PVC venting. 4. Changing the piping to primary secondary and reusing the existing piping and pump for the secondary loop. The new primary loop will be Filling the system with 40% glycol to match existing glycol in system. 6. Insulating all new hot water lines with minimum 1-1/2 fiberglass insulation and all service jacket per Michigan Energy Code 7. Wiring up the new boiler and pump. Provide an electrical Emergency Stop switch for the new boiler mounted adjacent to the boiler room door. 8. Provide an Electrical Emergency Stop also for the exiting space heating boiler that is to remain. 9. Reconnect and verify operation of snowmelt control system. 10. Provide test and balance report on new boiler system. CONTRACTORS: The Contract for Work encompasses all work indicated including the work of the various Construction Trades. The Contractor must work with the Owner to assure a minimum of inconvenience to the Owner's day-to-day operation. Any scheduling problems anticipated by the Contractor shall be brought to the Owner's Representative's attention before the occurrence. The Owner will occupy the facility throughout construction period. GENERAL REQUIREMENTS: All debris resulting from demolition operations shall be promptly disposed of off the site. Materials deemed unreusable, such as concrete, etc., shall become the property of the Contractor or Subcontractor responsible for its removal. Debris shall not be allowed to accumulate in the building or on the site. Clean up on a daily basis. LAYING OUT WORK: The Contractor shall immediately locate all general reference points and lines and preserve them, lay out his work and be responsible for all lines, elevations and measurements of buildings, grading, paving and other work executed by him under the Contract. The Contractor shall verify all plan dimensions and elevations before laying out work, and he shall be held responsible for the cost of correcting any errors resulting from his failure to exercise such precautions. DELIVERY OF MATERIALS The Contractor and each Subcontractor shall inspect the site and determine the availability of storage space and trucking facilities to bring material or equipment into the building, or any other factors affecting the work under this Contract.
