3 EXECUTIVE SUMMARY Environmental Planning & Management as a sub-consultant of Genesys Engineering performed an asbestos material survey on June 30 th and July 3, Certified New York City Department of Environmental Protection (NYCDEP) Asbestos Investigator and New York State Department of Labor (NYSDOL) Asbestos Inspector Joseph Cascio and NYSDOL Asbestos Inspector Thomas Tompkins performed the limited asbestos survey at the Hunter College Campus School, located at 71 East 94 th Street, New York, NY The purpose of the survey was to identify the location and quantity of asbestos containing materials (ACM greater than 1% by PLM analysis) that might be impacted by the proposed HVAC Upgrade Project at the Hunter College Campus, as described in drawings prepared by WASA Architects, dated 10/24/2014. The scope of work includes the demolition of three Boilers and a Chiller Unit and associated equipment piping in the basement. An old cooling tower on the south side of the roof will be demolished down to the steel dunnage which shall be left in place. Fan coil units within the building on the first, second, third and fourth floor throughout the building will be renovated. Four new chases will run from the second floor to the roof and tie into renovated fan coil units and a new cooling tower on the roof. The Third Floor MER will be renovated. EPM also used sample results from a JLC Survey Report for WASA Studio A at the Hunter College Campus School dated September 18 th, 2014 for the HVAC and Boiler Upgrade Project. The JLC report can be found in Appendix G. EPM bulk samples were collected from suspect asbestos containing materials including pipe and elbow insulation, pipe cover, pipe flange gaskets, boiler insulation, boiler door insulation, boiler rope insulation, hot water generator tank insulation, feed tank insulation, 9 x 9 floor tile mastic, green multi-colored flooring, adhesive, 1 x 1 white fissured ceiling tile, ceiling tile glue, 1 x 1 white flat ceiling tiles, blue multi-colored flooring, adhesive, cooling tower black gasket, black flashing tar on cooling tower equipment and built-up roof samples. Laboratory analysis conducted by layers indicated that asbestos was detected in the following materials: Boiler rope door insulation; Pipe insulation on oil piping for Boiler # 1; Brown pipe gaskets above the catwalk; Hot water generator tank insulation; Mud packing insulation in the chiller room; 9 x 9 Floor tile mastic; Black gasket material on cooling tower; i EPM, INC.

4 Black flashing tar on cooling tower equipment. Additionally, the following materials are assumed to be asbestos containing due to limited access for sampling: Pipe flange gaskets in the Boiler Room, Chiller Room and 3 rd floor MER; Pipe insulation and Pipe Gaskets in Room B14A & B30; Pipe insulation Room 135; Pipe insulation Stair No. 5; Many of the classrooms identified within survey with asbestos containing 9 x 9 Floor tiles were not accessible during the survey; Pipe gaskets on the 4 th Floor Roof cooling tower; The Boiler Room Tank room was not accessible. Quantities of ACM pipe were based on the JLC Survey Report. It should be noted that there was no interior access to Boiler #1, #2 & #3. Asbestos containing materials may be present inside the boilers and inside the boiler base. Assumed asbestos containing materials have been included in asbestos summary Table I. A summary of all asbestos containing materials with the potential to be impacted by this project, along with the description, location, and quantity can be found in Table I. A list of the sampled suspect materials, along with the associated laboratory analysis can be found in Table III. Photographs of the asbestos containing materials are included in Appendix A. EPM inspected and sampled materials which were observable and accessible to the survey team. It is possible that additional suspect ACM may be present within concealed spaces. All suspect asbestos containing materials encountered during the course of the renovation, which have not been tested, must be assumed ACM, until tested otherwise. Polychlorinated Biphenyl (PCB) Survey EPM personnel, Joseph Cascio and Thomas Tompkins conducted an on-site assessment of suspect PCB containing materials with the potential to be impacted by the proposed scope of work. EPM identified a total of one (1) homogeneous, suspect, PCB-containing sealant, with the potential to be impacted by the proposed scope of work. Three samples from each homogenous material group were collected in the field and combined by the laboratory resulting in a total of one (1) caulk sample that was analyzed for PCBs. The sample was analyzed by EPA Method Based on the laboratory results, PCBs were non-detected and the sealant can be disposed of as regular construction waste. A list of the suspect materials that were sampled, along with a summary of the associated laboratory results can be found in Table III. ii EPM, INC.

5 Universal Waste and Miscellaneous Hazardous Materials Inventory EPM inspected all areas and materials that are anticipated to be impacted by the HVAC Upgrade for the presence of universal wastes and miscellaneous hazardous materials. Universal wastes and miscellaneous hazardous materials are located within the Mechanical rooms. The types of universal wastes and miscellaneous hazardous materials that were identified include: Thermometers (mercury); Batteries packs; and Electrical circuit boards. A summary of all identified universal wastes and miscellaneous hazardous materials affected by work is included in Table II. All identified universal wastes and miscellaneous hazardous materials impacted by the proposed work and any universal wastes and miscellaneous hazardous materials discovered during work that require removal for this project should be properly removed and recycled / disposed of in accordance with all applicable local, State and Federal laws, rules and regulations iii EPM, INC.

9 1.0 INTRODUCTION The field inspections was conducted on June 30 th and July 3 rd, 2017, by Environmental Planning & Management certified New York City Department of Environmental Protection (NYCDEP) Asbestos Investigator and New York State Department of Labor (NYSDOL) Asbestos Inspector Joseph Cascio (certificate number and ) and NYSDOL Asbestos Inspector NYSDOL Asbestos Inspector Thomas Tompkins (certificate number ) at Hunter College located at 71 East 94 th Street, New York, NY, EPM s scope of work consisted of the following tasks: 1. Perform a visual inspection of areas to be impacted by the proposed HVAC Renovation at Hunter College as described in drawings EPM had at the time of the survey which were dated 10/24/14 and prepared by WASA Studio A in order to identify the potential location of asbestos containing materials (ACMs) that might be affected. EPM was also provided with a copy of the JLC Survey Report of the Hunter College HVAC and Boiler Upgrade Project to use. 2. Conduct the delineation of homogeneous areas to be impacted and collect bulk samples of suspect ACM. 3. Submit the suspect ACM bulk samples to a certified laboratory for Polarized Light Microscopy (PLM) and/or Transmission Electron Microscopy (TEM), where applicable, analysis. 4. Prepare a report summarizing data collected techniques, analysis procedures, location and quantity of asbestos containing materials (greater than 1% as determined by PLM in accordance with New York City Title 15 and New York State Industrial Code Rule 56). 5. Prepare Universal Waste and Miscellaneous Hazardous Materials design drawings and specifications (if required) for removal and recycle of Universal wastes and miscellaneous hazardous materials that are anticipated to be impacted by the proposed renovation work. 1 EPM, INC.

10 2.0 ASBESTOS SURVEY This section describes the data collection activities including the site inspection and sample collection. EPM personnel conducted an on-site inspection of the Hunter College 71 East 94 th Street, New York, High School and Elementary School which will be impacted by the proposed HVAC Upgrade Project renovation. 2.1 Field Inspection The field inspection was conducted on June 30 th and July 3 rd, 2017, by EPM certified NYCDEP Asbestos Investigator and NYSDOL Asbestos Inspector Joseph Cascio and NYSDOL Asbestos Inspector Thomas Tompkins. Copies of EPM s company license and personnel certificates are presented in Appendix B Sample Collection Guidelines used for the inspection were established by the Environmental Protection Agency (EPA) in the Guidance for Controlling Asbestos Containing Materials in Buildings, Office of Pesticides and Toxic Substances, DOC #560/ , and 40 CFR Part 763, Asbestos Hazard Emergency Response Act (AHERA). Field information was organized as per the AHERA concept of homogenous area (HA). A strategy for sampling suspect asbestos containing materials was developed employing the delineation and grouping of homogeneous suspect materials. The delineation of homogeneous areas at the site was based on criteria including material type and location. Materials suspected of containing asbestos were identified for the area inspected. When suspect ACM s were found, representative bulk samples from the homogeneous material group (material which is uniform by color, texture, construction application date, and general appearance) were collected. The asbestos bulk sample location drawing is presented in Appendix C Field Survey / Observations The scope of work for this project includes Boiler Replacement and the upgrading of the HVAC System for the Hunter College Campus School. This includes: Demolition and replacement of three new boilers at Sub-basement; Remove all piping associated and pumping equipment s associated with boilers; Remove two hot water generator tanks and removal of all associated piping and equipment; Demolition of existing chiller and HVAC equipment to be relocated to the basement level and roof; Electric Room to be expanded at basement level and renovation of locker room; Renovation of the heater fan coil units on the First, Second Third and Fourth floors in the classrooms. 2 EPM, INC.

11 Demolition and installation of a new cooling tower on the fourth floor roof. The Boiler Room has three boilers. Boiler #1 is not in use and the doors are sealed shut. Boilers 2 & 3 are in use. Each Boiler has two sets of doors. A smaller set of doors on the lower half of the Boiler and two larger doors on the upper half of the boiler. A boiler rope insulation on the larger doors tested positive for asbestos. The lower door and exterior boiler insulation were investigated, and found to be non-asbestos containing. Behind the boiler are two large hot water generator tanks. The insulation on these tanks was surveyed and tested positive for asbestos. The pipe elbows surrounding these tanks were found to asbestos containing in the JLC survey report. EPM surveyed pipe gaskets that were accessible. The pipe gaskets sampled were found to be non-asbestos and the gaskets which were not accessible were presumed to be asbestos containing. An insulated oil pipe running behind the boilers and pipe gaskets on the catwalk above the boilers tested positive for asbestos. The Chiller has a non-suspect rubber and foam insulation. The Chiller Room boiler feed tank insulation was surveyed and found to be no asbestos containing. Pipe gaskets for condensate pumps #3 and #4 and red pipe gaskets for the Chiller were investigated and found to be nonasbestos materials. Mud packing insulation around fittings of a large pipe feeding the chiller tested positive for asbestos. Pipe gaskets that were not accessible were presumed asbestos containing. On the First through Fourth Floors the majority of the work is with the ventilator units which are located along the exterior wall in rooms along the perimeter of the building and interior rooms. The ventilators themselves have no suspect material but all the ventilator units are attached to the floor alongside 9 x 9 asbestos containing floor tile as per the JLC Survey Report. EPM collected black floor tile mastic underneath the 9 x 9 floor tile, which had not been surveyed in the JLC Report on the first floor, and is positive for asbestos. The work also entails making penetrations in the concrete floor on the third, fourth floor and roof to connect with the new cooling tower. EPM surveyed the third and fourth floor and collected green multi-colored flooring, flooring adhesive, 1 x 1 white fissured ceiling tile, ceiling tile glue, 1 x 1 white flat ceiling tile, blue multi-colored flooring and the associated flooring adhesive. The materials sampled were found to be non-asbestos. The fourth floor built-up roofing, top membrane, mineral board, black membrane between the foam board and mineral board and the black vapor barrier over the concrete roof deck, were found to be non-asbestos. The abandoned cooling tower on the fourth floor roof is to be demolished down to the metal supports and used for the new cooling tower. The cooling tower surveyed had a black gasket along the seams in the cooling tower walls and black flashing tar observed on the cooling tower equipment both tested positive for asbestos. The cooling tower has a PVC fill. The water supply pipes have gaskets which were not accessible and will be presumed to be asbestos containing. A silicone caulking on the cooling tower which is not suspect for asbestos was sampled for PCBs 3 EPM, INC.

12 and found to be non-detected for PCBs. Table III contains a detailed summary of the laboratory analysis results. 2.2 Analytical Procedures The bulk samples were analyzed by Alpha Labs LLC, th Avenue, LIC, NY Alpha Labs is accredited by the New York State Environmental Laboratory Accreditation Program (ELAP # 11833) and the National Voluntary Laboratory Accreditation Program (NVLAP # ) of the National Institute of Standards and Technology (NIST). Laboratory accreditation documentation is included as Appendix D. Bulk samples of suspect asbestos containing materials (ACM) were analyzed using Polarized Light Microscopy (PLM) with dispersion staining, as described in 40 CFR Part 763 and National Emissions Standards for Hazardous Air Pollutants (NESHAPS) regulations. ELAP PLM analysis protocol 198.1, which includes the identification and quantification of vermiculite, is used for friable samples, while protocol is used for non-friable organically bound (NOB) materials. NOB materials which tested negative by PLM were analyzed using Transmission Electron Microscopy (TEM) method by Analysis Protocol TEM analysis is the only method that can be used to determine if non-friable organically bound materials (NOBs) can be considered non-asbestos containing. Samples in a homogeneous group are analyzed until either the entire group is analyzed (all the results are negative) or a positive result is obtained. When a positive result occurs, the remaining samples in the group don t have to be analyzed. Laboratory analytical data is included as Appendix E. 2.3 Survey Conclusions and Recommendations Laboratory analysis conducted by layers indicated that asbestos was detected in the following materials: Boiler door gasket/rope on boiler #3 basement boiler room; Pipe elbow insulation on oil piping for boilers; Pipe gaskets above catwalk in boiler room; Hot water generator tank insulation; Mud packing insulation on chiller pipes in basement chiller room; Floor tile mastic for 9 x 9 floor tiles on first Floor; Black flashing tar - 4 th Floor roof cooling tower; and 4 EPM, INC.

Additionally, the following materials are assumed to be asbestos containing due to limited access for sampling: Pipe flange gaskets in the Boiler Room and 3 rd floor MER; Pipe flange gaskets in the basement Chiller Room; Pipe insulation and Pipe Gaskets in Room B14A & B30; Pipe insulation Room 135; Pipe insulation Stair No. 5; Many of the classrooms identified within survey with asbestos containing 9 x 9 Floor tiles were not accessible during the survey; Pipe gaskets on the 4 th Floor Roof cooling tower; Boilers #1 - #3 internal insulation, section gaskets and fire bricks/board; The Boiler Room Tank room was not accessible. Quantities of ACM pipe were based on the JLC Survey Report. Please the asbestos locations schematics included as Appendix F. EPM inspected and sampled materials which were observable and accessible to the survey team. It is possible that additional suspect ACM may be present within concealed spaces which were not accessible. Any suspect asbestos containing materials encountered during the course of the work, which have not been tested, must be assumed ACM until tested. All asbestos containing materials impacted by proposed renovations should be abated in accordance with all applicable federal, state and local regulations. 5 EPM, INC.
