3 Former 55 th Street South Trust Bank - ACBM Survey 1.0 EXECUTIVE SUMMARY One Stop Environmental (OSE) has performed this Asbestos-Containing Building Material (ACBM) Survey Report for the site referenced above to aid Woodlawn Foundation in the proposed renovations of the structure as well as for future renovations at the building. The ACBM survey at the site was conducted on January 29 th of Mr. Gerrad Hicks, with OSE performed the Asbestos Containing Building Materials (ACBM) via sampling suspect asbestos-containing building materials from the interior and exterior of the building (excluding roof and other inaccessible areas). The asbestos bulk samples were sent to Cardno ATC Laboratory in Centennial, Colorado to be analyzed by polarized light microscopy (PLM). All sampling locations and frequencies were determined in the field under the direction of Mr. Gerrad Hicks, with OSE. Laboratory results from the ACBM sampling are found later in this report. Please see Appendix D for the asbestos inspector accreditation certifications. This report includes the areas that were deemed to be accessible and safely accessible. Several areas that are not accessible by reasonable means or requiring a lift were not sampled. No roof samples were collected due to warranty concerns. All sampling limitations were discussed with and agreed to by Mr. Gamble prior to conducting the survey. OSE was directed to conduct a semi-destructive NESHAP asbestos survey for this report. It should be noted that ACBM may exist at the site in areas that were inaccessible to OSE such as chases. OSE was not directed to perform and additional investigation into Lead, mold or any additional regulated materials. OSE was not provided with any copies of former environmental surveys for review. This report is designed to aid the client in regards to ACBM as they pertain to the proposed renovations at the building. This report should not be used as an Abatement Workplan, Specification, Bidding Document or as a Project Design. Please refer to Appendix D for the Site Plan depicting sampling locations. Please see Table 1 for the ACBM sample analysis and Appendix A for the ACBM sampling laboratory report. OSE s services and conclusions have been conducted in accordance with commonplace principles and practices within the Industrial Hygiene field. 5 Former 55 th Street South Trust Bank - ACBM Survey 2.0 PURPOSE The purpose of this task is to complete a pre-renovation environmental assessment for the Woodlawn Foundation located at st Avenue South in Woodlawn, Alabama. The task includes preparation and submittal of visual comprehensive inspection to determine likely presence of ACBM, a sampling effort to confirm existence or absence of suspect ACBM, and preparation of a pre-renovation asbestos survey report. 3.0 SITE ACTIVITIES OSE performed a visual inspection and collected ACBM samples in each building/floor (as accessible) in accordance with U.S. Environmental Protection Agency (EPA) protocols. Semi-destructive sampling was performed on all suspect ACBMs within the structures unless presumed to be ACBM. It should be noted that none of the structures were occupied at the time of the survey. The following sections present details of the building(s) surveyed, including construction, area, building observations, and ACBM sample inventory summary. Diagrams of sampling locations in each building/floor are exhibited in Figure 1 including a summary of where ACBM was sampled. A summary of ACBMs are presented in Table 1 The table presents the sample numbers, material, description, asbestos analysis (percentage and type of asbestos), estimated quantity of ACBM, and friability status. ACBMs containing less than one percent asbestos were not included in the tables, since these materials are typically not considered to be ACBM. Due to time constraints, OSE chose not to point count asbestos samples that initially ranged between 1-5% asbestos in an attempt to determine if the material could be found to be below 1%. Should the client request this, OSE can arrange for the aforementioned samples to be point counted if requested prior to the lab disposing of the samples. A photographic log is included in Appendix C. Samples were submitted to Cardno ATC, Inc. in Centennial, CO for analysis. There were no unique sampling requirements, conditions, or major obstacles identified during the survey. Tables and diagrams identify all ACBM that was identified. Any estimated quantities within this report are not to be used for any bidding purposes, nor as a specification.

6 Former 55 th Street South Trust Bank - ACBM Survey 4.0 REGULATORY INFORMATION The United States Environmental Protection Agency (USEPA) and the Occupational Safety and Health Administration (OSHA) are tasked with the regulation of all airborne asbestos fibers levels. The aforementioned agencies have established the standards for permissible airborne concentrations of asbestos fibers and specific requirements for abatement and renovation activities. OSHA is responsible for worker safety and the (USEPA) for the general environment. In addition, each individual state can adopt its own requirements, which are allowed to be more stringent than the standards set forth by OSHA or the USEPA. The asbestos general industry standard was established by OSHA in This standard was primarily directed toward industrial uses (29 CFR ). The construction industry standard (29 CFR ) became effective on July 21, 1986 in response to the growing asbestos abatement industry and the additional concern regarding asbestos exposure. The standards oversee asbestos abatement procedures, respirator selection and fit testing, air sampling, the air analysis, and employee protection from exposure to airborne asbestos fibers. A standard for exposure including monitoring of the time-weighted average (TWA) and permissible exposure limit (PEL) of 0.2 fibers per cubic centimeter of air (f/cc), and a short-term excursion limit of 1.0 f/cc were additionally generated. If concentrations exceed these levels, specific employer-initiated activities such as instituting a respiratory protection program and medical surveillance for exposed employees are required. In October of 1994, OSHA changed these standards to include the reduction of the PEL for an 8-hour TWA to 0.1 f/cc in the revised construction industry standard of 29 CFR and the revised general industry standard 29 CFR Currently, building owners are now required to communicate to all employees, subcontractors, and tenants of the quantity and location of asbestos-containing materials (ACMs) identified in this survey.

7 Former 55 th Street South Trust Bank - ACBM Survey 5.0 HAZARD ASSESSMENT FACTORS A physical assessment was performed for each homogenous material identified at the site. The physical assessment includes evaluating the condition, assessing the potential for disturbance, and determining the friability of each material. Friability is a term used to describe the ease in which a building material inherently lends itself to disturbance. By definition, "friable" materials are those that can be crumbled or reduced to powder by hand pressure when dry. Each material on the list was further classified into one of three categories, which have specific sampling requirements for each category. Surfacing Materials are materials such as spray-applied or troweled surfaces such as plaster ceilings and walls, fireproofing, textured paints, textured plasters, and spray-applied acoustical surfaces. Thermal System Insulation includes materials that are used to insulate to inhibit heat gain or loss on pipes, boilers, tanks, ducts, and various other building components. Miscellaneous materials are materials that do not fit in any of the above two categories such as resilient floor covering, baseboards, mastics, adhesives, roofing material, caulking, glazing, and siding. This category also contains wallboard and ceiling tile. All suspect ACBMs were then assessed according to their condition as good (intact), fair (damaged), or poor (significantly damaged). Material with localized significant damage was also assessed as poor when observed. 6.0 SAMPLING STRATEGY The asbestos inspection was conducted in general accordance with the NESHAP requirements utilizing a minimum number of samples collected from each HA, which also meets the sampling requirement found in OSHA regulations. If the analytical results indicate that all the samples collected per HA do not contain asbestos, then the HA (material) will be considered non-acbm. However, if the analytical results of one or more of the samples collected per HA indicate that asbestos is present in quantities of greater than 1 percent asbestos by weight (as defined by EPA), OSE considers all of the HA (material) ACBM regardless of any other analytical results. OSE did not sample building materials which can visually be determined to be non-asbestos (i.e., fibrous glass, foam rubber, etc.). Inspector judgment is cited in these cases. Bulk asbestos sample collection involves physically removing a small piece of material and placing it in a marked, airtight container. Sample containers are marked with a unique identification number, which is also noted in the field notes. 7.0 LIMITED ASBESTOS-CONTAINING BUILDING MATERIAL (ACBM) SAMPLING SUMMARY Mr. Gerrad Hicks with OSE collected forty two (42) bulk asbestos samples from the building on January 29 th, The bulk samples were sent to Cardno ATC Laboratories in Centennial, Colorado for analysis by polarized light microscopy (PLM) based on the number of layers (materials) associated with each bulk sample (ie: sheet flooring and mastic are two layers). Any material determined to be over 1 percent (1%) asbestos is considered an ACBM as defined by the National Emissions Standard Hazardous Air Pollutants (NESHAP) emissions standard. The scope of this report included sampling of suspect building materials from the aforementioned specified reasonably accessible areas that is believed to be affected by the planned renovations and potential future renovations. Please see Table 1 below for the ACBM sample collection laboratory results and Table II for a list of PACM identified at the site:

This survey was semi-destructive in nature. More heavily destructive means to access any additional concealed materials or the moving of any heavy items was not part of the scope of work for this project. OSE has made the best attempt to list any presumed asbestos-containing materials (PACM) that were unable to be sampled due to access limitations (if applicable). Four PACMs were identified during the site visit as are listed in Table II. The PACMs should be treated as ACBM until proven otherwise through laboratory analysis. Additionally, no

14 Former 55 th Street South Trust Bank - ACBM Survey samples were collected within any fire doors (if applicable). It is possible that there may be concealed ACBMs within inaccessible areas such as equipment interiors, wall chases, floor and ceiling spaces, interior ductwork, foundation spaces etcetera. Any exposed suspect ACBMs during demolition activities should be considered positive for asbestos content until confirmatory sampling is completed. Please see Appendix A for the full ACBM laboratory report. 9.0 LIMITATIONS Any personnel conducting renovation, demolition or maintenance at the site should be made aware of the potential for additional hidden ACBMs that may be encountered during their work. These areas will need to be tested prior to renovation or demolition activities that could disturb the materials. OSE s findings are based on the scope of work as well as the site conditions and access capability during the dates of the fieldwork. This report is not designed to be used as a bidding document, an abatement workplan, project design or for an abatement specification.

The EPA considers a material to be asbestos containing only if it contains more than one percent asbestos by Calibrated Visual Area Estimation (CVAE). EPA regulations also indicate that Regulated Asbestos Containing Materials (RACM) -- materials which are friable or may become friable -- be further analyzed by point counting when the results indicate less than ten percent asbestos by CVAE. Our laboratory utilizes CVAE on a routine basis and does not include point counting unless specifically requested. The results may not be reproduced except in full, and should not be used as a scope of work for abatement without consulting with Cardno ATC.
