How do you plan to continue regular physical activity after graduation? Explain how physical activity and a healthy nutrition plan are important to a healthy lifestyle? How do you manage stress? SECTION B: EVIDENCE/DOCUMENTATION Choose either Option 1 or Option 2 for your Evidence/Documentation Requirement OPTION 1: If you are currently taking PE or Dance attach a photocopy of your Term 2 Report Card OPTION 2: For other physical activities such as extracurricular activities outside of class time (i.e.: sport teams, walking, cycling, fitness programs, gym, etc.) you need to: a) Complete the Physical Activity Documentation Log documenting 150 minutes a week of physical activity for your grade 12 year (see example on next page) b) Get verifying signatures and contact information (name, phone number, ) If using any activity outside of school such as walking to/from school, you need to attach a typed letter from your parent/guardian verifying how often, and how many minutes per section Don t forget that all your assignments must be computer generated!
