1 Aug. 23, 2016 To: All Vendors of Record Re: Addendum #2 All vendors are to accept the information contained herein as the official response of IPTC. TO ALL BIDDERS OF RECORD AND TO WHOM IT MAY CONCERN: This Addendum is being issued prior to the due date for receiving proposals. This Addendum forms a part of the Contract Documents and modifies the original Request for Proposal (RFP) as noted below and shall be incorporated into the Contract Documents. All other provisions of the RFP released July 25, 2016 with the exception of changes below, shall remain unchanged. This Addendum is issued in accordance with the provisions of Section 3 of the Request for Proposal document. All Proposals shall be based upon work as modified by this Addendum. Acknowledged receipt of this Addendum on the Acknowledgement of Addendum Form is required. Failure to do so may result in disqualification of the Bidder. This addendum addresses questions presented by bidders before the deadline to the submission of proposals for RFP Coach Operator Uniforms. IPTC Request / Vendor will need to send seamstress/equivalent personnel to measure all new students of Coach Operators, prior to release of training. Bid Offer Cost Form. 2 1. Can we get a copy of the IndyGo logo used for the hats and any other shirt that needs embroidery? We need a vectorized EPS file preferably. IPTC Response: Please see Attachment File for IndyGo Logo in PNG format. 2. It is possible to get samples, pics or, manufacturer item numbers to make sure we are apples to apples with current items provided? Including all employee groups, drivers, supervisors, and center operators. IPTC Response: Please see Attachment for Pictures. 3. In the bid meeting, it was indicated that you purchase other colors of shirts that was not on the bid. Can you get info on those colors and items? It was also indicated that boots will be part of the program; can we get a spec on these?, all colors are still permitted and the same boots as operators will be fine (black and polishable) 4. In the bid meeting, it was indicated that there will be a 1 or 2 line of personalization embroidery on the shirts. What needs to be on the shirts and what employee group will this be for? Also the font and color of embroidery. IPTC Response: Operators: First name and last name initial, badge (underneath name). An example is listed below. Bold or Arial font will be fine. Ex. Jane D. 3 5. Is this tax exempt? 6. Is there a list of what employee group is able to buy what uniform item? IPTC Response: Operators: List provided, Shirts are Blue. Service Center: Colored shirts are acceptable and all other items are that same as operators Transportation Supervisors: Shirts can be polyester or cotton (with embroidery), Shirts are White. 7. As far as the process for the accounts of new hires and employees leaving. If an employee leaves, that account is frozen and refunded the following March 31st. If a new employee is hired, IndyGo will cut a check for that employee's allotment? Is this process correct? 8. Is it possible for a new vender awarded this program to purchase any leftover inventory from the current vender? And also, when this contract is up, can the vender sell inventory to the new vender? IPTC Response: IPTC does not own the inventory, therefore we cannot control or request current vendor to release. 3

4 9. Can we get current prices paid by IndyGo employees on each of the items listed? IPTC Response: Please see Attachment for current uniform prices. 10. In regards to quoting alternatives (see page 14 of your terms/conditions), do they still need to be approved 5 days prior to submission date if we are submitting samples with the bid? 11. Please advise Sole & Heal size criteria? IPTC Response: According to the uniform policy no shoe sole over 1 inch. 4

5 12. Please advise Lead times expectations, example ordering/shipping/delivery? IPTC Response: Training is an 8-week process; the order takes place after the 3 rd week. It normally takes 4 weeks before receiving the order. (Class start dates vary) Delivery to IPTC upon contact of current training staff Seamstress/equivalent will be scheduled to IPTC to measure all new employees 13. Please advise expectations of customer service, selecting/sizing/ordering/delivery? IPTC Response: Store front preferred, order the same day and a seamstress available to size. Delivery 1-2-week max 14. Is there Probationary/Temporary employee items Name Tags/uniforms?, the probationary period is 6 months. Probationary operators only receive name tags, no embroidery (First name, last initial and badge). 15. Do employees typical spend their entire allotment?, within 2-3 years 5

6 15. Are Items to be delivered to IPTC or to personal residence? IPTC Response: Delivery optional. Prefer to pick-up items. 16. Embroidery requirement? IPTC Response: First name and initial of last name. Underneath the name should be the badge Ex. Listed below Ex. Jane D. # Provide artwork (hat) to quote properly? 7 18. Provide service center operator shirts on list of approved items? IPTC Response: Please see Attachment for Pictures. 19. Provide pictures of all items? Pictures provided IPTC Response: We are not able to provide pictures of all items at this time. 20. Is an online store acceptable? IPTC Response: It will be considered. However, store front preferred 21. Catalog or Flyer for employees to look at? 7

8 22. Regarding accessories (hats, socks, belts) are those Pre Approved? 23. Please clarify Samples needed with Proposal Offer, Coach Operator/Service Center Employees/Transportation Supervisor each, also what about criteria, men/woman shirts/pants/shoes/accessories?
