2 We successfully employ the following purchasing and installation models: NOTE: In major metropolitan urban areas such as New York City and its immediate environs, it is the Interface policy that an order ship to the dealer contractor of the end use customer s choice. The purpose of this policy is to avoid situations in which deliveries cannot be easily handled by the end use customer. Customers can place their orders by fax, , or by original P.O. The contract naming NJPA must be noted on all purchase orders. F.O.B. Point: F.O.B. Destination from LaGrange, Georgia to Loading Dock in 48 contiguous states and Washington, D.C. * Additional charges for unloading and inside delivery will apply. Carpet Only Orders Material only purchases with the manufacturer: Interface Americas, Inc. dba Interface 1503 Orchard Hill Road, LaGrange, GA Federal ID# T: Orders can be ed to: Orders@Interface.com or by fax to (844) Material and Installation Orders Material and Installation purchases with the manufacturer: InterfaceSERVICES, a division of Interface Americas, is our turnkey solution provider assuming full responsibility for manufacturing, delivering and installing your carpeting needs. The contract installation and sundry item are only available through InterfaceSERVICES, and must accompany an Interface carpet tile installation project. Purchase orders must be issued in the name and address noted below. Interface would make contract pricing available to authorized dealers, who would be responsible for take offs and logistics. The contract users must instruct their dealers/agents purchasing on their behalf that the contract name must be noted on their purchase orders. Interface can provide a list of our dealers; however the end user has the responsibility for choosing the specific dealer. Payment Terms for carpet only orders: Net 30 Payment terms for carpet and installation orders: When installation is required in conjunction with carpet purchased under contract, InterfaceSERVICES may invoice separately for the carpet. The customer prior to installation shall make payment for carpet purchased. When installation is included, payment for material is expected according to the terms of sale (Net 30 days), and payment for labor is due upon installation completion. All customers desiring open credit will be required to supply a signed credit application prior to order submittal. A credit line will be established at the sole discretion of Interface's Credit Department. If the customer does not qualify for open credit, 100% payment is due before material is shipped. Methods of payment accepted: Credit cards, cashier check, certified check, wire transfer, ACH (company checks are held 5 to 7 business days)

They are described as light, medium, and heavy. Light Moving Removal and replacement of furniture for project with minimum amount of furniture per square yard of carpet for example executive offices, conference rooms Medium Moving Removal and replacement of furniture for project with normal multi-person open office space with desks, chairs, office partitions, and file cabinets, but does not include modular furniture disassembly and removal Heavy Moving Removal and replace of furniture for a project with high concentration of files and desks or involves the moving of sensitive equipment such as personal computers or computer terminals and printers. It does not include mainframes or modular furniture disassembly and removal, but does include replacing of broadloom or carpet tile under modular furniture with carpet tile utilizing a system for lifting the modular furniture in place without damage. $4.25 $7.35 $15.50 per SQY per SQY per SQY The contract installation rates and sundry item prices contained in this agreement are only guaranteed through InterfaceSERVICES, our in-house turn key project management group. 13 ReEntry Specifications and Cost 1.1 Scope of Work Owner specifies that existing carpet and existing carpet waste is recycled in the best possible manner. A reclamation plan will be submitted that will provide directions for the reclamation of recyclable carpet at the job site. Environmentally friendly carpet recycling methods should be performed subsequent to job completion. In this case, environmentally friendly methods consist of either: Page 1 of 2 1. Recycling is turning waste materials into new materials of the same value, such as vinyl backing into vinyl backing. 2. Upcycling involves turning waste materials into more valuable products. For example, using waste PET (plastic from soda bottles) to create Terratex panel fabric. 3. Downcycling creates less valuable products from waste materials. Turning nylon face fiber into car parts or carpet padding, including nylon face fiber in recycled backings, or using carpet for waste-to-energy are good examples of downcycling. 4. Repurposing allows a product to be reused by an organization or individual. Interface assist in facilitating the donation of used carpeting to charities and other non-profit organizations. 1.2 Description of Services Collection (Labor): Carpet must be removed from the existing installation and prepared for pickup based on the type of material and reclamation option selected. Specifications for removal from the jobsite are as stated: A. Removal of Carpet tile for recycling 1. Approved carpet tile must be palletized and secured for shipping, i.e. Shrink wrap, banding, strapping. Pallets must be a minimum size of 40 x Tile must be kept dry and free of any moisture damage. 3. Trailers must be clean of any non-carpet debris, i.e. construction waste, cardboard boxes, trash. 4. Material does not contain mold or mildew. 5. Material does not contain vinyl asbestos or adhesives containing asbestos. 6. Baled material will not be accepted. B. Removal of Performance Broadloom for recycling 1. Approved Performance Broadloom should be rolled or palletized and secured for shipping. Pallets must be a minimum size of 40 x40. If shipment is less than 4000 yards, must be palletized. 2. Performance Broadloom must be kept dry and free from moisture damage. 3. Trailers must be clean of any non-carpet debris, i.e. construction waste, cardboard boxes, trash. 4. Material does not contain mold or mildew. 5. Material does not contain vinyl asbestos or adhesives containing asbestos 6. Baled material will not be accepted. C. Removal of Broadloom carpet for recycling 1. Fiber or Urethane carpet padding will be accepted. Attached cushion is not acceptable. 2. Broadloom should be rolled or palletized (must be palletized if less than truckload quantity) and secured for shipping. Pallets must be a minimum size of 40 x Carpet must not exceed 9 ft width, manageable length. 4. Carpet must be a minimum width of 3. Charges will apply for any material received less than the minimum width. 5. The carpet must be accumulated and kept dry for shipping and processing.

14 6. Containers must be clean of any non-carpet debris, i.e. construction waste, cardboard boxes, trash. 7. Material must not contain tac strips or metal staples. 8. Material does not contain mold or mildew. 9. Material does not contain vinyl asbestos or adhesives containing asbestos. 10. Baled material will not be accepted. D. Repurposing of Carpet material 1. Approval of the material for possible donation will be determined at the jobsite or upon tile submittal. Material Loading: Processing: Certification: Shipper must load material on the carrier, drivers will not assist. Notification must be given prior to quote submittal of any special requirements such as lift gate, small trucks, etc. Additional fees will apply. All possible recycling options must be clearly presented and/or submitted on paper subsequent to job start. The accepted reclamation option must be approved in writing by the party requesting the services. A certificate will be furnished upon request verifying the reclamation of the carpet and the pounds of material diverted from the landfill. 1.3 Recycling Cost Matrix Interface offers reclamation under the terms and rates noted below.
