2 Summary: Leading Indicators. 2 First Aid cases were reported in the region with one each in Abu Dhabi and Iraq Nearmisses were reported in the region, with Saudi taking the lead with 3 nearmisses, Dubai and in Baku Zero Harm/ Safestart cards were submitted in Jan by employees in the region safety inspections were conducted in the region, with Abu Dhabi and Saudi taking the lead with 2 inspections each. 5. The EHS audit action s closure improved from 45% last month to 47.5% in Jan in the region, however it remains significantly less than the target of % closure. Saudi and Iraq facility has completed and implemented more than 6% and 72% audit findings respectively, all other facilities are below 4%. Baku s closure rate is the lowest at 6% without any significant change. Regional EHS Manager s visit to all facilities has been scheduled in Q 25, and part of the agenda will be to review the progress against audit action plans and provide support in effective closure of these action plans. Lagging Indicators. Jan 25 remained LTI free in the region. 2. medical treatment injury happened in Saudi in the month of Jan 25. Injury Details: Recordable Injuries Incident # 5-KSA- MTI- Injury Type Medical Treatment Injury Injury Body Part Middle finger of the left hand Date Status Incident Details Corrective & Preventive Actions 4-JAN- 25 Ongoing The helper removed a wheel from a wheel barrow and took it to the Maintenance Dept for it to be inflated. He over inflated the tyre and the inner tube burst causing an aggressive displacement of the rim and his hand being in the line of fire resulting in the incident. A regulated tyre inflation system ordered. Only trained and authorized employees allowed to do the job. All areas of restricted access communicated to all personnel. A tyre inflation guide implemented and enforced. Inspection schedule of tyres implemented. All wheel barrow tyres changed to solid tyres. A tyre and equipment review conducted in the region in Oil & Gas services. An EHS Warning prepared and circulated in the region for information of all concerned employees.

First Aid Injuries: Incident # 5-AUH- FAI- 5-IRQ- FAI- Injury Type First Aid First Aid Site Date Status Incident Details Corrective & Preventive Actions AUH Iraq -JAN JAN- 25 Completed On-going An employee suffered injury to his thumb when it came between the forklift forks and slings. The employee and the forklift operator were transporting an adaptor flange. IP was releasing the jack after positioning the CNC machine for foundation preparation. During the last rotation he released the jack faster than required causing the jack to lower quickly and jerk. IP afraid of the quick movement of machine, quickly moved left side that caused the left side of his face to struck against machine guarding. Use of forklift to transport items from one place to another by hanging them with slings has been prohibited. The use of level 5 cut resistant gloves have been emphasized in a toolbox talk. All items to be transported from one place to another using forklift will be palletized. Forklift use SOP has been revised to include the restriction of transportation using slings. A toolbox talk conducted on safe use and operation of forklift and for employees to always maintain distance to a mobile equipment. Make the handle longer which enable employee to do the task from outside rather than going underneath. SOP/Risk assessment to be revised to include correct positioning of jacks and use of jack with longer lever and communicated to all concerned employees. Communicate the incident to all employees through a toolbox talk. Use of STAR POWRA to be emphasized through an awareness session to all supervisors & foremen. 2
