Broken bones 4. Intrusions 5. Tipping stacks 6. MHE damage YES Risk Assessment Matrix Certainty Probable Possible Remote Improbable Harm Potential Risk level Negligible Minor Fatality 1-4 Low Scratch or Laceration or bruise etc First Aid 5-12 Medium High Serious Medical treatment or over 3 day absence Major Broken bone or long term absence Assessment of Risk: What is the likelihood of the RISK OF INJURY (risk rating) regarding the hazards described above? Harm 4 X Likelihood 3 = 12 MEDIUM RISK Who is at risk? Operator, all staff working in the immediate area, visitors, contractors and MHE. Remedial Action: What controls are in place to reduce the risk? 1. Proper storage of empty pallets, Good Housekeeping procedures 2. Staff to wear gloves when handling pallets 3. Control of height of stacks and movement restrictions 4. Good Housekeeping procedures 1 Issue
2 What is the likelihood of the RISK OF INJURY (risk rating) following implementation of control measures? Harm 3 X Likelihood 2 = 6 MEDIUM Description of work: - Safe manual handling of pallets. Safe movement of euro pallets using fork lift truck Safe movement of general pallets using fork lift truck Note: Forklift truck driver must take extreme care when carrying pallets and especially near pedestrian areas. Picking items onto pallets (All operatives) Always check the integrity of the pallet before use Always use good manual handling technique when lifting or moving pallets. Take care when working on or off the pallet as there is a risk of cuts from splinters or nails. Check pallet for contamination such as glass, chemical spillages or protruding nails Remove old strapping, or shrink wrap from around the blocks Check the wood for cracks, splits, twisted boards or over hanging boards Do not over load the pallet as this will make it difficult to move. Always ensure the load is secured using shrink wrap Lifting pallets from ground level Always wear gloves when lifting pallets to protect your hands from wood splinters and nails. When lifting pallets always bend your knees to reach the pallet Grab edge of pallet. 2 Issue
3 Using your legs, lift pallet onto its edge Personnel preference Grab the pallet either inside or outside edge of the pallet. Make sure fingers do not get trapped between the wooden slats. Lift pallet up and place onto next pallet. If needed ask for assistance. Two man lift!!!! Slide pallet into position Note some pallets are heavy, if the pallet is too heavy to lift don t lift it, ask for help or use mechanical aid such as an FLT. Note: - wet pallets can become very heavy and slippery to handle, never lift wet pallets. Ask for help and conduct a two-man lift. Removing pallet and placing onto floor level Slide pallet towards you and stop at centre of pallet Place hands at side of pallet and bend knees as the pallet is slowly lowered onto the floor. Slide pallet into desired position. 3 Issue
4 Do Not allow pallets to be stored on their edges (sides). Never use pallets on their edge (side) to make any type of barrier. If a pallet falls over from this position it can cause serious leg injuries, abrasions, broken bones etc. They must always be stored flat. 4 Issue
5 Sign off sheet. The trainee should sign this SOP to demonstrate they fully understand the contents and training received. How long did the training take minutes. How does the trainer know the trainee fully understands the SOP.?
