1 November SCHEDULE 94 Performance Measurement: Questions This Schedule contains questions about service delivery. This information, together with the schedules containing notes and information on cross boundary service delivery and service delivery arrangements in a two-tier system, will be useful in determining factors that influence operating costs and total costs for efficiency measures. Schedule 94 also asks municipalities to identify the method used to allocate general government - program support to other functions. This information is required for submission of the Financial Information Return. column 1 Response: Y, N, NA This column is used for questions with a yes or no answer. A pull-down menu appears when the cursor is placed in column 1. column 2 Lane Kilometres or Library Uses Column 2 is used to report numeric data. Lines 2202 through 2205 are used to report the number of lane kilometres. Lines 7403 and 7404 are used to report the number of library uses. column 3 Description (Drop-down Menu) This column features a pull-down menu for lines 0201, 1100, 3300 and The menu appears when the cursor is placed in column 3. The column also accommodates text for lines 0202, 1105, 2207, 2209, 3302 and 7401.

2 November General Government 0201 Method used to allocate Program Support to other functions in Schedule 40 When the cursor is placed in column 3, a pull-down menu will appear. Select one of the following entries: OMBI Method Modified OMBI Method Percentage of Total Expenditures Modified Percentage of Total Expenditures Other Method (Please describe below) Note that if this line is not completed, a critical error will be triggered (94C 001) If Other Method is selected in line 0201, please describe method of allocating Program Support If Other Method is selected in line 0201, the cell in line 0202 will turn white indicating text can be entered. 3 November Fire 1100 Type of Fire Fighting Force that exists in the Municipality? When the cursor is placed in column 3, a pull-down menu will appear. Select one of the following entries that best describes the type of fire fighting force in the municipality: Volunteer Fire Fighters only Salaried Fire Fighters only Combination of Volunteer and Salaried Fire Fighters Not Applicable 1104 Does municipality have property with significant assessed value that has its own Fire Fighting Force? Select N or Y. Select Y if the municipality has property with significant property assessment which has its own fire fighting force. Some properties, such as federal airports, have their own fire fighting capability and do not make use of the fire fighting resources of the municipality. If this is the case, the efficiency measures for fire services, Operating costs for fire fighting services per $1,000 of assessment and Total costs for fire fighting services per $1,000 of assessment, may be lower than they would otherwise be If Y is selected in line 1104, please briefly describe the property. If the answer to line 1104 is Y, SLC will turn white indicating text can be entered. Briefly describe the types of properties with significant property assessment that have their own fire fighting force.

4 November Police Note that it may be possible to have a Yes answer to more than one question below, particularly if the municipality was amalgamated. Schedule 93, Notes, may be used to provide additional information, if desired Are police services provided by your own police department? 1202 Are police services provided by another municipality? 1203 Are police services provided by the Ontario Provincial Police (OPP)?

5 November Building Services 1301 What method does your municipality use to determine total construction value? (See efficiency measure SLC ) When the cursor is placed in column 3, a pull-down menu will appear. 6 November Roadways 2201 Is the number of lane kilometres maintained in winter in own municipality the same as the number of lane kilometres in the municipal road system? Y or N is automatically entered in this cell when lines 2202 and 2203 are completed. If a municipality does not provide winter control services to all roads, N will automatically be entered. Lines 2202 to 2005 For lines 2202 to 2205, enter the number of lane kilometres in column 2. A lane kilometre is a continuous lane of road which conveys traffic in one direction. Total lane kilometres are determined by multiplying the number of centre line kilometres by the number of lanes, for each road. Continuous two-way median turn lanes and assumed access lanes may be counted as a single lane. A two-way median turn lane is a continuous centre left turn lane that accepts traffic travelling in both directions. Parking, storage, ramp and turning lanes should not be counted. Exclude private roads, such as those maintained by the municipality on a fee for service basis Number of lane kilometres in the municipal road system Number of lane kilometres maintained in winter in own municipality Number of lane kilometres maintained during the rest of the year in own municipality (exclude private roads) Number of lane kilometres maintained in winter in own municipality and any other municipalities served Does your municipality clear sidewalks and parking lots in winter?

Wastewater and Storm Sewer Systems For lines 3101 through 3106, a pull-down menu will appear when the cursor is placed in column Does your municipality provide wastewater collection? 3102 Does your municipality provide storm water collection? 3103 Does your municipality provide wastewater treatment and disposal? 3104 Does your municipality provide storm water treatment and disposal? 3105 Are wastewater and storm water systems integrated in all parts of the municipality? 3106 Are wastewater and storm water systems integrated in some parts of the municipality?

Select the type of water billing system from the following list: Flat Rate billing system only Metred billing system only Combination of Flat Rate and Metred billing system Not Applicable 3301 Is any water treated to standards which are lower than drinking water standards, such as the provision of raw water to industry? 3302 If Y is selected in line 3301, please describe briefly. If the answer to line 3301 is Y, SLC will turn white indicating text can be entered. Enter a brief description of water not treated to drinking water standards.

(A lower-tier served by the upper-tier library board does not report.) 7400 Type of library service arrangements When the cursor is placed in column 3, a pull-down menu will appear. Please select from the following list: Lower-tier or single-tier with a library board Lower-tier receives service from upper-tier library board Upper-tier with a library board Member of a union public library No library. Purchases service. Other Not Applicable 7401 If Other is selected in line 7400, please describe. Lines 7402 to 7404 If the answer to line 7400 was "No library board. Purchases service." do not complete lines 7402 to 7404 of this schedule Does your library board or union public library provide service on a contract basis to other municipalities without a board? 7403 Total library uses for the library board Enter total library uses for the library board in column 2. This information is provided by the library. To calculate this information, the library may use the workbook posted on the MAH web site entitled, Library Data for MPMP. 10 November Total library uses for your municipality only The amount entered on this line should equal the number entered in the denominator in SLC The instructions for apportioning library uses are contained in an Excel file called Apportioning Library Uses. This is posted on the FIR home page in the section MPMP Information. Also see the instructions to Schedule 91 for the measure: Operating costs for library services per use (SLC ) and Total costs for library services per use (SLC ). The number of Total library uses for your municipality only in SLC will equal the number of uses for the library board in SLC only if the municipality has a library board that does not provide contract service to a municipality without a library board. The number of Total library uses for your municipality only in SLC will be less than the number of uses for the library board in SLC under the following circumstances: Municipality has a library board which provides service on a contract basis to one or more municipalities without a library board. (In this case, library uses are apportioned between the municipality with the library board and the municipalities purchasing service based on the percentage of total operating expenses for all the municipalities involved in the contractual arrangements.) Municipality is a member of a union public library. (In this case, library uses are apportioned between members of the union public library based on the percentage of operating costs, as calculated for the efficiency measure.)
