2 Hazardous Waste Consignment Note Completion Guide This guide is to assist in how a Hazardous Waste Consignment Note should be completed. When is a Hazardous Waste Consignment Note needed? A Hazardous Waste Consignment Note is required each time hazardous waste is moved from Site to a Receiver. The Hazardous Waste Consignment Note is needed for the movement of the following wastes: Light Tubes Oil Coldroom Panels How do you complete the documentation? PART A Consignment details This section should be completed at the time of removal by the person removing the waste. 1. Waste Removed From This is the address from where the waste is coming from. 2. Waste Taken To This is the address from where the waste is going to. 3. The Consignment(s) will be One single should be ticked here. 4. Expected Removal Date of First Consignment & Last Consignment The first consignment date should be the date of the waste movement and the last consignment should be left blank as it is one single consignment. 5. Name, Signature & Date This is the name and signature of the person removing the waste and the date that this was signed. 6. Telephone This is the telephone number of the store. 7. The Waste Producer was Only needs to be completed if this is different from the address where the waste is being removed from (#1)

3 PART B - of the Waste This section should be completed at the time of removal by the person removing the waste. 1. The Waste is: The following choices should be used for this part; Oil Light Tubes Insulation Panels 2. Six-Digit EWC Code EWC Codes Oil Light Tubes Physical Form Oil Light Tubes Form Gas Liquid Mixed Solid Solid 4. Colour Colour Clear Oil Light Tubes

4 5. Total Quantity for Removal & Container Size, Type & Number For the quantity of removal you will need to specify the volume being removed and the measurement, i.e. 3kg. For the container size section please write down the cylinder bottle number. 6. The Chemical/Biological Components Concentration Name Up to 100% Oil Oil Up to 100% Light Tubes Mercury 0.1% Machine Machine 7. The Hazard Codes Oil Light Tubes EWC Codes H14 H14/H7 H5 H14 H14 8. The Process Giving Rise to the Waste Recovery Decommission Service Maintenance

5 PART C Carrier s Certificate This section is to be completed by those transporting the waste to its next destination. I.e. By the engineer that is moving the waste to Climate or by the Climate driver picking up the waste from site. Name & Signature This is to be completed by the person transporting the waste On Behalf of This is the company name of those transporting the waste, i.e. Climate or Integral. Date & Time This is the date & time of the collection 1. Carrier Registration This is the registration number of the company transporting the waste. Integral s registration number is: CB/PM3180LF. 2. Vehicle Registration This is the registration number of the vehicle transporting the waste PART D Consignor s Certificate This section is to be completed by those that own the waste (the Store). Name & Signature This is the name of the person from the store present at the removal of the waste. On Behalf of This is the company name of which the waste belongs to. Date & Time This is the date & time to confirm that the check of the form has been completed. Note. This should NOT be before the date and time provided in Part C PART E Consignee s Certificate This section is completed by those receiving the waste once the transportation has taken place and would not be completed by Integral.

6 What does a completed form look like?

7 Who receives each copy of the Hazardous Waste Consignment Note? PRENOTIFICATION COPY (White) Send to Leicester Office, once section B has been completed DEPOSIT COPY (Yellow) Send to Leicester Office, once section D has been completed. CONSIGNEE S COPY (Pink) Is given to the carrier transporting the waste once section D has been completed, which is then given to the receiver of the waste. CARRIER S COPY (Blue) - Is given to the carrier transporting the waste (or kept In the Integral vehicle transporting the waste) CONSIGNOR S COPY (Green) - Is given to the person at store once section D is completed Note. The Leicester Integral Office will chase the receiver of the waste for a fully copy of the completed Hazardous Waste Consignment Note completed (to include section E), once the red copy has been received from the engineer. Where can you get spare copies of the Hazardous Waste Consignment Note? Speak to your Line Manager who will get to spare documents out to you. Is anything else required by the Leicester Integral Office? Please also add the Integral job reference in the top right hand corner of the Hazardous Waste Consignment Note before posting to assist with waste traceability.
