2 City of Fontana C A L I F O R N I A DEPARTMENT OF ENGINEERING EXCAVATION AND TRAFFIC CONTROL PERMIT REQUIREMENTS Before a Traffic Control/Lane Closure Permit is issued, the contractor shall obtain a City of Fontana Business License, provide a copy of their Contractor s License (classifications A or C only no B license), and provide proof of General Liability insurance and Workers Compensation insurance. In addition, the City of Fontana shall be named as additional insured. Insurance requirements include: Minimum $1,000,000 General Liability Limit Minimum $1,000,000 Worker s Compensation Minor Projects which are determined to have minimal traffic impact will generally not require a Traffic Control Plan submittal. The permittee shall use, as guidance, the latest edition of the California Joint Utility Traffic Control Manual (CJUTCM) for the placement of work zone temporary traffic control devices. Simply list the appropriate page number on the application. If the CJUTCM does not apply then it is more than likely the project will require the submittal of a Traffic Control Plan. If a Traffic Control Plan is determined to be required, the application and plan must be submitted to the front counter of the Engineering Department at City Hall. Please submit 2 copies. Note: It is the responsibility of the permitee performing work on, or adjacent to, a roadway to install and maintain appropriate work zone temporary traffic control devices as necessary to provide safe passage for the traveling public (including pedestrians and bicyclists) through the work zone, as well as for the safeguard of workers. Applicant shall save, keep, and hold harmless to the City of Fontana, its officers or agents from all damages, costs or expenses in law or equity that may at any time arise or be set up because of or in the course of performing work authorized by this permit which may be occasioned by any act or omission of the permittee. Major Projects involving traffic control beyond the scope of the California Joint Utility Traffic Control Manual (CJUTCM) will require engineered Traffic Control Plans. Such projects can include: Work on major streets and/or intersections Road closures requiring detour routes Overnight traffic control/night work Work near schools, Kaiser Hospital and Railroads - 2 -
3 If an engineered Traffic Control Plan is required, the City will require at least two weeks to review and process. Plans must be submitted to the Front Counter of the Engineering Department at City Hall. Please submit 2 copies. Engineered Traffic Control Plans shall include: Existing lane geometry and roadway widths The proposed work zone All existing traffic control signing & striping Temporary covering or removal of conflicting signs, pavement legends and striping Proposed lane widths adjacent to the work zone (12 minimum). All proposed temporary traffic control signing & striping (including spacing dimensions) The restriction of parking as necessary Appropriate length transitions throughout the work zone area Provision for the safe transit of pedestrians adjacent to the work zone Jurisdictional boundaries (as necessary) In addition, Traffic Control Plans shall include: Legend for signs and other traffic control devices (barricades, arrow boards, etc.) Work hours (as approved by the City) Project duration General notes (as provided by the City) Contact information for contractor Contact information for plan engineer Provision for access to residents, businesses, etc. within the work zone Provision for bus stops, mail delivery, trash pick-up and other essential services affected by the construction Signature blocks for all affected jurisdictions The fee to review engineered Traffic Control Plans is $900 per sheet (not including the cover sheet). If it is determined that Traffic Control Plans are required then you will be issued a Case Number and your application and plans will be taken in as a plan check. Please submit 2 copies. Special Conditions Time Restrictions The City reserves the right to restrict work times as well as work days. No work shall encroach into the travel lane of a major roadway between the hours of 7:00 a.m. to 8:30 a.m. or from 3:30 p.m. to 6:00 p.m. Night work, when required, is limited to the hours of 9 p.m. to 5 a.m. Work within the vicinity of schools will be restricted to 1 hour after school begins to 1 hour before school ends. Time restrictions do not apply when school is not in session. Inspection Fees Fees for inspections scheduled for evenings and weekends will be at the overtime rate, as per the City s comprehensive fee schedule
4 Restoration of Traffic Control Devices The applicant is responsible for restoring to original condition all striping, legends, signs and any other existing traffic control devices at the end of the work shift and at the end of the project. This includes any traffic control devices damaged by any means or rendered ineffective. Replacement shall be per current City Standards. Road Closures If a project involves a road closure, traffic detour plans will need to be reviewed and approved prior to permit issuance. 14 day Advance notification of emergency services, affected residents, etc. will be required. Informational Signs or Message Boards If informational signs are required, they shall be posted a minimum of seven days prior to the start of construction. The legend and placement locations of the signs are subject to review and approval by the Traffic Engineering Section. Notification of Emergency Services, Schools, Transportation Services and Impacted Residents If required, the contractor shall be responsible for notifying police, fire, and other emergency services as well as any impacted residents of the construction/road closure and detour route (if applicable) at least seven days prior to the construction/road closure.
