ST MARTIN OF TOURS HOUSING ASSOCIATION PROJCT WORKR JOB DSCRIPTION Role: Responsible to: Location: Project Worker Islington Registered Care Home Manager and Deputy Project Manager Islington, N1 St Martin s have three registered care homes where 24 hour support is provided to between 18 and 23 men of working age. Residents have often spent considerable time in institutional settings, i.e. hospitals or prisons. Residents are furthermore unable to live more independently at the present time in their lives due to the challenges of having to overcome the impact of having chronic long term mental health needs, and in some cases also having become trapped in debilitating alcohol and drug taking lifestyles. Project workers provide a central role, particularly in providing key working to a small caseload of service users, creating support and risk management plans with service users and overseeing aspects of service delivery with the central aim of helping service users to move onto more independent accommodation settings. Key Responsibilities and Tasks Include: Service users Participate in the assessment process by working alongside St Martins dedicated assessment workers to interview and discuss prospective support and risk management plans in relation to people referred to St Martins. Take the lead role in understanding service user needs, providing keywork services to an allocated number of service users and work with them on a variety of issues as set out in the key work guidelines and their individual care plan. Keep comprehensive assessment documents updated and maintain the support and risk management plans in respect of allocated service users as these are reviewed, evolve and change. Work closely with and provide leadership to, Support Workers. This includes
delegating relevant key work tasks to them as agreed with the Project Manager. Provide full information to Support Workers when planned leave is taken in order that they may take on key work duties entailing more responsibility with the allocated service users. Keep appraised of assessments, support plans, risk assessments that St Martins staff produce in relation to individual service users and take reasonable care so as to maintain personal safety. To accurately report untoward incidents and participate in any review of these within the team meeting or other forums. Communicate effectively with allocated statutory professions, e.g. Social Workers, Community Nurses, concerning the delivery of clinical aspects of service users support and risk management plans; give allocated statutory professionals copies of support and risk management plans after these have been updated. nsure that service users have copies of their support and risk management plans and that these are given again after they have been updated. nsure that all service users have an opportunity to set-out their own plan for managing their mental ill health, as per the Recovery Approach Model, and that this plan is given due consideration and supported where practicable by St Martins. nsure that at least one care plan goal is determined by service users themselves. Liaise with other agencies, participate in meetings to present reports as required in order to help service users fulfil their needs as set out in the support plan. nsure allocated service users are in receipt of all benefits to which they are entitled. Keep other relevant members of staff informed on issues concerning allocated service users. Administer and record service users oral medication and monitor service users compliance with depot medication as required. nsure breaches of service users license agreements and other conditiions are reported to project managers so that any actions necessary can be taken. Implement any resulting sanctions, evictions and appeals procedures concerning such service users. Keep abreast of issues concerning all service users in the Project. Follow a team approach to ensure the ongoing welfare of all service users and report any concerns or complaints which might suggest a vulnerable adult is at risk of harassment or abuse, using the St Martins Safeguarding Procedures.
nsure all paperwork concerning key worked service users is up to date Co-ordinate regular room checks, Health & Safety Inspections and fire drills, reporting any maintenance issues as and when appropriate, using the relevant systems. nsure that all service users statutory rights are upheld, in particular those contained within the Mental Health Act, Mental Capacity Act, qualities Act, Human Rights Act, Data Protection Act, and relevant housing legislation. Administration/Finance Undertake other administrative tasks as allocated by the project management. Keep account of the individual petty cash float and to submit claims when appropriate. Team and organisational involvement Attend staff meetings on a regular basis. nsure that staff cover is adequately provided by participating in the rota system and provide emergency cover at the Project as necessary. Attend and participate in other meetings within the organisation as appropriate and as agreed by the line manager. Represent the St Martins and attend meetings with outside agencies as agreed by line managers. Work with all service users, staff and outside agencies in accordance with the organisation's qual Opportunities policy. Attend supervision and participate in the annual appraisal meetings with the line manager Attend training as agreed by the line manager, ensuring you are up to date with the required core training Participate in the recruitment, induction and training of new staff members as appropriate. SALARY & BNFITS Salary Scale Band Role Grade and Salary Grade 1 21,116.07 H11 Project Worker Grade 2 22,309.59 Grade 3 23,594.91 Pay You will be paid at Grade 1 of your Band upon joining St Martins. In addition, you will be paid 50 per month forensic supplement. Holidays You will receive 25 days leave each year, in addition to statutory public holidays, for the first two years service; you will then be entitled to 28 days holiday. In your fourth year you will be entitled to 30 days. Probationary Period Hours Pension 6 Months 37.5 per week on average, including unpaid 30 minute break per shift. Start and finish times will be in accordance with the weekly rota, which will include weekend shifts and bank holiday shifts.
