JOB DESCRIPTION TEAM LEADER DAY OPPORTUNITIES TEAM LEADER DAY OPPORTUNITIES Responsible to: The Home Manager Direct Line Manager: Home Manager Work Base: The service will operate from a base at Dorrington and/or Bardney, The Team Leader may also have responsibility for Day Opportunities delivered from other Company bases or within the community. The nature of the post requires the Team Leader to travel between bases and have access to a vehicle to transport people being supported, for which a mileage allowance will be paid. SUMMARY OF ROLE To provide a range of Day Opportunities to adults with a learning disability. The service will be outcome focused and based on a person centred approach that meets individual needs and aspirations; fosters self awareness, personal growth and independence and gives each person the strongest voice with regards to decision making and lifestyle choices. The Team Leader will be responsible for providing support specifically to individuals receiving Day Opportunities and staff supporting them. MAIN RESPONSIBILITES, TASKS & DUTIES Leadership Ensure staff members work effectively and support individuals, in the way they choose, following their support and daily activity plans. Provide leadership to the designated team acting as a positive role model at all times. Plan and manage staff rotas on a daily basis. Arrange and provide on call cover, working within on call guidance. Provide effective supervision to the members of the staff team. Ensure good team working and motivate, support, coach and mentor the staff when needed. Ensure staff members complete all necessary paperwork to an acceptable standard and sign off on a daily basis. Contribute to the development of the service. Contribute to monitoring the quality of the service. At all times to work within the Equal Opportunities policy and procedure. - 1 of 6 -
Support Encourage people to make choices and decisions enabling each person to take control of their own lives and increasing independence. Encourage the involvement of people being supported in the development of and day to day operation of the service. Actively promote effective communication, including accessible information to facilitate involvement and decision making. Support the development of person centred planning, ensuring each individual s plan has meaningful and achievable goals with positive outcomes that meet aspirations as well as needs. Ensure all personal care tasks are carried out with respect for privacy and dignity and in line with the needs, wishes and preferences of the individual as defined in the support plan. Support people to participate in the community by accessing local facilities and wider community resources and promote social inclusion. Support people to develop, expand and maintain social networks and friendships. Supervise and support key-workers/ co key-workers to coordinate and develop the support to the individual including Person Centred Plans, Daily Activity Plans and other agreed plans. Monitor progress of individuals and contribute to / attend reviews and multi disciplinary meetings. Develop positive working relationships with family and carers and develop their understanding of the principles of the service delivery. Work in partnership with care managers and other professionals to improve life chances for individuals. Promote healthy life styles for individuals, liaising with other professionals and agencies as appropriate. Administer medication in line with the policy and the individual s care plans. Work in line with the company s Adult Protection and Prevention of Abuse Policy and Whistle Blowing Policy to ensure that people are kept safe at all times. Maintain the security of the building and the safety of the people receiving support and members of the staff team. Ensure risk assessments are implemented as necessary, in line with policies and procedures. Behave in a manner that reflects positively on the company at all times Provide regular supervision to Personal Support Workers in the designated team and enter actively into all supervision and appraisal processes. Lead regular team meetings, set the agenda and ensure minutes are kept. Update knowledge and skills and attend training on a regular basis to ensure that all mandatory and personal training and development needs are met in line with company and statutory requirements. Work closely with other members of the team for the ultimate benefit of the people being supported. Finance Work within the allocated budgets. Support individuals to manage their own money on a daily basis. Manage the petty cash allocation, ensuring it is checked, balanced and recorded at the end of each day. Financial discrepancies must be reported to the manager immediately. Keep up to date with all financial policy and procedures. - 3 of 6 -
Administration Be available to work flexibly on a rota basis to meet the needs of the Day Opportunities service; this includes weekends, evenings and bank holidays, where necessary to meet individual needs. Ensure all records are maintained accurately, appropriately and timely in compliance with the company requirements and that records made and personal information used are in compliance with the Data Protection Act and the standards of information governance.
