August 1, 2017 Forsyth County Procurement Donna Kukarola, CPPO, CPPB, Procurement Director ADDENDUM #2 Bid 17-72-3340 For: Providing all materials, equipment and labor for the Jot em Down Road Water Tank Site Grading. This addendum supersedes and supplements all portions of the bidding documents and becomes part of the contract documents for the above-referenced project. Where any item called for in the specifications or indicated on the drawings is supplemented hereby, the original requirements shall remain in effect. Where any original item is amended, voided or superseded hereby, the provision of such item not so specifically amended, voided or superseded shall remain in effect. Clarifications from Pre-Bid 7/20/17: 1. Sign in sheet is posted on the county website, www.forsythco.com 2. Forsyth County is in the process of obtaining a LDP. The contractor is responsible for the NOI fee based on the acreage. 3. Time to complete the project is 210 calendar days. 4. REPLACE the current bid schedule (pages 17 and 18) with the attached revised bid schedule. 5. Brown and Caldwell did not determine any issues with the drawing scales. 6. Forsyth County will hire a geotechnical firm as a 3 rd party to provide necessary geotechnical services and materials testing throughout the project. 7. Hand auger records for the site are attached to this addendum as information for the bidders. 8. Contractor shall provide a 24-inch undercut of rock to an elevation of 1380.00 underneath the future tank and drive around the tank; and then backfill with soil and compact to the tank finished floor elevation of 1382.00 and to the drive elevations indicated on drawings. 9. Tack coat for the asphalt drive shall be completed by others. The attached updated bid schedule has been revised to indicate this clarification. 10. The typical asphalt driveway section detail on sheet C-001 indicates 6 SUBGRADE STABILIZED WITH 150 LB OF STONE PER SQ. YD. (UNLESS MATERIAL IN PLACE WEIGHT 95 LB / CF). 11. A new 24 RCP is shown at the entrance drive of the site but not provided on the bid schedule. The attached revised bid schedule has been updated to include 24 RCP. 12. A bid item has been added to the attached revised bid schedule for additional bedding if approved by owner and required beyond standard bedding requirements. See specification section 31 23 00, 3.08 for bedding requirements. 13. Clearing chips from the trees can be used onsite for erosion control. The remainder chips or chips used for temporary erosion control shall be hauled off the site. 14. Permanent stabilization shall be achieved at the tank site in accordance with the Manual for Erosion and Sediment Control in Georgia, latest edition. 15. Forsyth County will set a 1-inch water meter for the site at the property right-of-way line. The water meter will be set at the County s expense and shall be used by the contractor for water source during construction. The contractor must set up a billing account, and will pay for water used during construction (including water used for pressure testing of the waterline). At project final completion, the contractor will close the account and the meter will be put in the County s name. 16. Contractor shall attend monthly progress meetings with the owner and engineer. 17. Forsyth County Water & Sewer Inspectors will be onsite, as well as Engineering Inspectors for erosion control. Brown & Caldwell will also provide periodic inspections. 18. Record drawings are to be provided in electronic and paper format 19. Hydrant meters are currently not available for dust control. The contractor shall obtain water from the tap at the meter (see item 15. above). Response to questions received: (response in italics) 1. When is this project scheduled to begin? It is anticipated this project will be taken to the Board of Commissioners for award at their September 12, 2017 Work Session; so it is anticipated the project will begin by the first of October 2017. 2. I am confused with the two different alignments A & B shown on sheets C-105 and C-106. The earthwork quantities are different. Please explain. Sheets C-105 and C-106 represent plan and profile views for the two water lines along the drive. Sheet C-105 is for Alignment A, which is the waterline along the west side of the drive. Sheet C-106 is for Alignment B, which is the waterline along the east side of the drive. 3. On the bid schedule it states at line item #25, 20' WIDE ACCESS ROAD, INCLUDING GAB, TACK COAT, NOT INCLUDING ASPHALTIC CONCRETE AND SUPERPAVE. However, on plan sheet C-001 detail #4, it shows the Tack Coat by
others. To better understand what is to be included in this bids scope of work. Are we only to clear, and add in the 6 stone and 8 G.A.B. rock and leave the asphalt and tack coat to others? Or are we to include everything into our quote and scope of work? Please explain. Contractor shall clear, add 6 subgrade where necessary, add 8 G.A.B., and leave the tack coat and asphaltic concrete to others (under a separate contract). See also Clarification 9. above. 4. On the drive around the tower is it paved or GAB only See response to question 3. above. 5. The Gate to be installed. Is it Electronic as Specifications describe? The swing gate is not electronic.
