INSTRUCTIONS TO BIDDERS MULTIPLE PARK DEMOLITION PROJECT BID#P2014-04 1. SUBMISSION OF BIDS Bids must be received no later than Friday, September 5, 2014 at 2:00 P.M. Formal sealed bids (must have signature) will be received by the Metropolitan Park District of Tacoma, and to be delivered to 4702 S 19 th St Tacoma, WA 98405. There will be a non-mandatory pre-bid meeting Monday, August 25, 2014, 8:00 A.M. at the Jane Clark Park (4825 N 39th St, Tacoma, WA 98407) and proceeding from there to the following locations: 1. Dickman Mill Park (Concrete Deck Replacement) - 2423 Ruston Way Tacoma, WA 98402 2. South Park (Garage) - 4851 S. Tacoma Way Tacoma, WA 98409 3. BIDDING PROCEDURE AND FORMS Bidders shall submit their bids on the bid proposal and signature sheet form included in this invitation to bid. A bid bond is not required with the bid proposal, however, a Faithful Performance and Payment Bond must be provided upon contract award. Forms shall be filled in completely in ink or typewritten. The signature sheet must be signed by a responsible company official with their designated title. Bidders may submit additional product information with their bid as desired. 3. BIDDER S REPRESENTATION Each bidder by making his bid represents that he has familiarized himself with the General Conditions for Metropolitan Park District of Tacoma Public Works Projects, Small Works Master Agreement, Metro Parks Tacoma Master Contract for Work Awarded From Small Works Roster and the attached Instructions to Bidders and Specifications. 4. REJECTION OF BIDS & ACCEPTANCE OF BID The MPD shall have the right to reject any or all bids or any items of the bid and to reject a bid, which is in any way incomplete or irregular. Any or all bids may be rejected for good cause. It is the intent of the MPD to award one contract to the lowest responsible bidder, provided the bid does not exceed the funds available. Contract award will be determined by the base bid amount. The MPD shall have the right to waive any informality, irregularity, or technical defect in any bids or bids received and to accept the bid or bids which, in its judgment, is in its best interests. 5. AGREEMENT BETWEEN OWNER AND CONTRACTOR A Bidder whose proposal is accepted shall sign the written agreement between MPD and Contractor within ten (10) days from the date of mailing of a Notice from MPD to Bidder of acceptance of his proposal. 6. WARRANTY AND GUARANTEE Bidder warrants and guarantees to MPD that all work will be in accordance with the plans and specifications and will not be defective. If within one year after the date of delivery, any material is found to be defective, the Contractor shall promptly and without cost to MPD, correct such defect. 7. PUBLIC WORK & PREVAILING WAGE RATES Please be aware when submitting a bid that this project is classified as "public work" and is subject to Washington State Prevailing Wage Law. The law requires in part that the following conditions be met: All workers or laborers must be paid the prevailing wage rate for their job classification as determined by the Department of Labor & Industries. A copy of current prevailing wages is attached to this notice. The prime contractor and each subcontractor must file a Statement of Intent to Pay Prevailing Wages with the Department of Labor & Industries, ESAC Division, in Olympia. Statements of intent are to be filed prior to beginning work. The prime contractor and each subcontractor must file a corresponding Affidavit of Prevailing Wages Paid with the Department of Labor & Industries at project completion. Weekly certified payroll forms must be completed by prime contractor and each subcontractor and submitted to the Metropolitan Park District. Industrial Insurance: All Industrial Insurance premium payments must be current for prime contractor and each subcontractor working on the project. The Metropolitan Park District will check with the Department of Labor & Industries to verify current premium status. No progress payments will be made to the prime contractor until approved Statements of Intent to Pay Prevailing Wages have been received by the Metropolitan Park District. Final payment will not be made until approved Affidavits of Prevailing Wages Paid have been received along with determination that the prime contractor and each subcontractor is in a current premium status. Please note that it is the responsibility of the contractor to forward the statements and affidavits. BID PROPOSAL FOR MULTIPLE PARK DEMOLITION PROJECT BID#P2014-04 Bid Due: Friday September 5, 2014, 2:00 P.M. Scope of Work: Multiple Park Demolition Project Bid#P2014-04 Metro Parks Tacoma is seeking bids for demolition at the locations listed below. Contractor shall furnish all labor, materials, facilities, equipment, services, insurance, and incidentals necessary to perform demolition within the following specified work areas: 1) Jane Clark Park (Wading Pool) - 4825 N 39th St, Tacoma, WA 98407 2) South Park (Garage) - 4851 S. Tacoma Way Tacoma, WA 98409 3) Dickman Mill Park (Concrete Deck Replacement) - 2423 Ruston Way Tacoma, WA 98402 4) SERA Work Center (Float Removal and Disposal) - 3873 S. 66th St., Tacoma, WA 98409 Metro Parks Tacoma has buildings/structures located throughout the District which are considered old assets and need to be removed from the park system. Metro Parks Tacoma is requesting bids for each building/structure demolition and complete restoration. Metro Parks Tacoma will assist the contractor in identifying utilities and locates, but utilities and locates in the demolition area are the contractors full responsibility. The contractor is responsible for keeping the area safe and secure for the public at all times, from demolition through completed restoration. 1) Jane Clark Park (Wading Pool) Complete demolition of the wading pool and restoration of the site. The wading pool is approx. 52 x 92 and constructed primarily of reinforced concrete, concrete decking, steel skimmer piping, supply lines, drain lines, entry gates, fence posts, and fence fabric. The construction zone will primarily be the area just outside of the fenced wading pool area. All removal work performed outside of the construction zone, if required, will be identified and labeled by Metro Parks Staff with spray paint. Fill material is estimated to be 275 to 325 CY. Metro Parks Tacoma reserves the right to salvage equipment. 2) South Park (Garage) Complete demolition of the garage, concrete retaining wall, rock retaining wall, concrete slab located on the north side of the building, and restoration to the site. The garage is a single story structure approx. 21 x 34 that consists of wood framing, composite roofing and a concrete foundation, and slab. Existing power pole must be left in place. This location may contain asbestos and lead paint and will need to be properly tested and legally abated prior to demolition.

3) Dickman Mill Park (Concrete Deck Replacement) Demolition and removal of MPT identified concrete deck section panels and replacement with new concrete deck panels. Area for replacement is approximately 481 sqft. Concrete work will match existing depth and width. It will be finished to match existing surfaces, concrete panels, and construction details. This includes reinforcing rebar bonded and pinned to existing panels, expansion and contractual joints. Sub-surface will be back-filled with crushed rock and compacted to meet panel specifications. MPT Staff will remove existing bollards prior to contractor demolition. 4) SERA Work Center (Marine Floats) Remove marine floats from sight and dispose of properly. Every effort will be made to keep the construction zone to a minimum. MPT will work with the contractor to identify an entry area and construction pad for each location. The contractor is responsible to keep the area safe from the public. This may require signage and temporary construction fencing during this work. All removal work performed outside of the construction zone, if required, will be identified and labeled by Metro Parks Staff with spray paint. Metro Parks Tacoma will provide bidders with photos of the buildings/structures, documents and if available historic drawings. Metro Parks Tacoma reserves the right to salvage equipment prior to the project starting. The contractor will be responsible for inspection and testing of hazardous waste and, if required, disposal per city, state, and federal regulations. The contractor is responsible for attaining demolition, filling, grading, and other required permits. It is the contractor s responsibility to insure that all utilities are disconnected and terminated per city code. Metro Parks Tacoma will approve all terminations prior to any demolition. The City of Tacoma may require the contractor to install silt fencing and storm water plan during demolition and restoration of areas. Every effort will be made by the contractor to limit the area that is impacted by this work at all locations especially Jane Clark Park, South Park, and Dickman Mill Park. It is the contractor s responsibility to restore the area to existing or better pre-construction conditions once construction has been completed. This includes the impact to egress and ingress for each location. This also includes but is not limited to turf, trees, roads, sidewalks, paths, landscapes, and other park-assets. These assets near the demolition areas will be protected from any impact resulting from this project. Metro Parks Tacoma will mark the protection zone for the contractor. Metro Parks Tacoma advocates sustainable/green practices and requests that the contractor recycle the concrete debris, metal, and any other reusable materials. Upon complete removal of each building/structure and associated features, The Metro Parks Tacoma Project Manager will inspect and approve the demolition work performed before the contractor begins restoration of the area. For Jane Clark Park and South Park all restoration work will blend and be finished to the same grade as the surrounding area. Sloped areas will blend and be finished to the adjacent area. Metro Parks Tacoma Project Manager will inspect and approve restoration work. Site specific restoration details are as follows: 1) Jane Clark Park (Wading Pool) 4825 N 39th St, Tacoma, WA 98407 - restored to finish grade and grass-turf mix

2) South Park (Garage) - 4851 S. Tacoma Way Tacoma, WA 98409 - restored to finish grade and grass-turf mix 3) Dickman Mill Park (Concrete Panels) - 2423 Ruston Way Tacoma, WA 98402 concrete deck sections will be restored to match existing deck sections on all sides. Concrete details will match original elevations and specifications as listed. Metro Parks Tacoma is requiring that the contractor submit soil samples for approval: 1) The sub soil for backfilling the site to rough grade. The sub soil will match the native surrounding soils with less than 5% organic matter. It needs to be weed free and no evidence of vegetation and debris. It can resemble the clay, sandy, gravelly or loam soil that is in the area and needs to be compactable. 2) The top 6 of topsoil for finish leveling. The topsoil will be a blended 5/8 to minus size material, a three way mix of 40% organic material (compost & peat moss) and 60% sandy loam. Local soil is preferred. Top soil needs to be weed free. The sub soil will be compacted so there is no further compaction of soil. The topsoil will be incorporated into the top few inches of the subsoil, graded, racked, rolled, and readied for hydro-seeding. Metro Parks Tacoma will approve the finished grade prior to any seeding. The finish top seeding will be hydro-seeded with one of the following mix (or approved blend) Seed for full sun areas shall consist of three perennial ryegrass cultivars: o Charismatic Perennial Ryegrass 33.16% o All*Star 2 Perennial Ryegrass 33.05% o Pavilion Perennial Ryegrass 32.74% Seed for sun / shade areas shall consist of a ryegrass / fescue blend as follows: o o o o o 29.6% IQ Perennial Ryegrass 19.86% All*Star 2 Perennial Ryegrass 19.83% Top Hat 2 Perennial Ryegrass 14.70% Jamestown IV Chewings Fescue (85% germination) 14.51% Trapeze Creeping Red Fescue (85% germination) Seed for use in over-seeding or hydro-seeding sports turf in full sun areas shall consist of the following three perennial ryegrasses: o Ecological Perennial Ryegrass 33.03% o Top Hat 2 Perennial Ryegrass 33.05% o Pavillion Perennial Ryegrass 32.98% The contractor will guarantee the growth of new turf for that area and be responsible for 90% coverage before substantial completion is approved. Submittals for products used are required including MSDS and Product Data Sheets. The contractor shall, at its own expense, obtain and keep in force any necessary licenses, permits, and tax certificates.

All projects will be completed within forty-five days of notice to proceed from Project Manager. If not completed by such date, the contractor shall be liable to the District for damages. The Owner and Contractor agree that in the event this project is not completed on time, the Owner's damages are difficult to calculate. As a result, the Owner and Contractor agree that if the project is not completed by the completion date, Liquidated Damages will be assessed in the amount of $250.00 per day. Deviation from the scheduled completion dates due to adverse weather conditions will be considered if coordinated with the Metro Parks Tacoma Project Manager. The undersigned Bidder declares that he has read and fully understands the Notice Inviting Bids, General Conditions for Metropolitan Park District of Tacoma Public Works Projects, Small Works Master Agreement, Metro Parks Tacoma Master Contract for Work Awarded From Small Works Roster, and each and every other Contract Document referred to therein and agrees to all of the terms, conditions and provisions contained therein; that he has examined the site of the work and has made the investigations and formed the estimates as to all conditions and contingencies referred to in and required by the Contract Documents, and he proposes and agrees that if his bid as submitted in the Proposal be accepted, he will contract in the form provided to perform all of the work and in the manner required by the Contract Documents and to complete the same within the time stipulated; that he will accept in full payment therefore the prices named herein. Said prices are to include and cover the furnishing of all materials, the performing of all labor requisite or proper, supervision, overhead, profit, taxes (excluding State sales tax), and the providing of all necessary machinery, tools, appurtenances, equipment and other means required to fully complete this contract, except as otherwise specifically provided in the Specifications.
