All information provided in this addendum is incorporated into the solicitation document as set forth therein. All other parts of the solicitation have been maintained as originally distributed. This addendum supersedes any verbal and/or other instructions given to any bidder/proposer qualified to respond pursuant to the requirements set forth in the solicitation document. I. DUE DATE This Addendum does not change the solicitation due date. As such, the solicitation response is due no later than Tuesday, June 7, 2016 at 2:00 P.M. Local Time. II. QUESTIONS AND ANSWERS (Q&A) The City has received the following question(s) concerning the solicitation: Q1. What is the engineer s estimate? A1. The engineer s estimate for this project is $1,937,000.00. Please note this estimate does not include alternate bid items add-ins and corresponding deductions as a result of the alternate bid add-ins. Q2. Will subcontractors FDOT qualification be accepted in some of these categories? Example: fencing, sodding, Portland cement, landscape, pavement markings (We the Prime have Drainage, Grading, Flexible Pavement, etc.) A2. Yes. Q3. In the advertisement it is stated that the following FDOT Pre-qualifications are required; #7 Drainage, #9 Fencing, 1 Grading, 2 Grassing/Sodding, 0 Flexible Pavement, #29 Portland Cement Concrete, 8 Landscape, #7 Drainage and #28 Pavement Markings. Will the use of subcontractors FDOT Prequalified in the areas that the contractor is not, be acceptable, provided that proof of current certifications is provided with the bid documents? A3. Please refer to response A2. Q4. Would full depth reclamation be considered from Station 114+71 to 125+90.50 (since access will be needed to be restored daily)? A4. Full depth reclamation can be considered, however, the contractor shall submit a pavement design signed and sealed by a professional engineer registered in the State of Florida. PUR-F-212 1
Q5. What line item is the proposed 8 reclaim paid under? A5. The reclaimed water line will be paid for under pay item 1050 11224 - Utility Pipe, F&I, PVC, Water/Sewer, 8-19.9. The cost shall include all fittings required to install the waterline and tie into the existing main. Q6. Station 118+60 to 121+77 (the sewer line that is outside of the ROW,), is the Contractor going to need to get a Right of Entry or will this section be abandoned in place and grouted? A6. The pipe outside the right-of-way shall be abandoned in place and grouted. Q7. On pages 7 13 of the plan view, is the indicated water line supposed to be the proposed reclaimed water line? A7. Yes. This is the reclaimed waterline shown on Sheets 27-29. Q8. Is item #56 Hardscape supposed to be bid as an alternate? Also could the City provide a breakdown of quantities for this item? A8. No, Item 56, 999-999-2 Hardscape is not an alternate bid item. It includes all items shown on Sheet L-8, Site Furnishings Details. Q9. Can the City provide a break-down of quantities for Item #57 Sanitary System (LS)? A9. Because this is a lump sum item, the following quantities are for informational purposes only. Please refer to Sheet 29 Utility Adjustment Sheet 3. Remove existing sanitary main and manholes as shown on Sheet 29. Furnish and Install 730 linear feet of 10 PVC Pipe and 3 new sanitary manholes SAN-1, SAN-2, & SAN-3. The cost shall include connecting the new main to the existing manholes EX-1 and EX-2. Q10. Does the City have more information for measurement and payments/specs or are the items per FDOT standards? A10. All items are per FDOT standard specs except for items 56 and 57. Item 55 580-1 Landscape and Irrigation shall include all work shown on the Landscape Plans except Sheet L-8 Site Furnishings Details. Item 56 999-999-2 Hardscape includes all items shown on Sheet L-8 Site Furnishings Details. Item 57 999-999-2 See the City of Casselberry Utilities Standards & Specification Manual. Q11. Does the prime contractor need to hold all the FDOT pre-qualifications, or can a subcontractor help meet the requirement? A11. Please refer to response A2. Q12. One of the FDOT pre-qualifications for this project is Portland Cement Concrete. The only bid item we could find in the plans that pertains to this prequalification is the 104SY of Ribbon Curb, which is an alternate. Is this prequalification necessary for such a small amount of this type of work? If so, would the City accept previous work Portland Cement Concrete experience performed by the prime contractor on a FDOT project? A12. Please refer to Change #4. PUR-F-212 2
III. City of Casselberry Finance Department Purchasing Division CHANGES, ADDITIONS AND/OR CLARIFICATIONS CLARIFICATION : MAINTENANCE OF TRAFFIC (MOT) PROCEDURES Please refer to REVISED Sheet No. 6 Traffic Control Note No. 5. [Attachment #3] CLARIFICATION #2: TEMPORARY AGGREGATE SURFACES The Contractor shall limit the existence of milled surfaces and temporary aggregate surfaces within vehicular travel lanes to forty-eight (48) hours, after which a permanent asphalt surface must be restored for vehicular traffic. CLARIFICATION #3: SOILS REPORT The geotechnical report for Oxford Road is hereby made available as Attachment. CHANGE : Replace Sheet No. 5 Project Layout with REVISED Sheet No. 5 (The line table has been updated) [Attachment #2] CHANGE #2: Replace Sheet No. 6 Oxford Road General Notes in its entirety with the REVISED Sheet No. 6 (Traffic Control Note 5 has been updated.) [Attachment #3] CHANGE #3: Replace Sheet No. 29 Oxford Road Utility Adjustment Sheet 3 [Attachment #4] CHANGE #4: The FDOT Portland Cement #29 Pre-Qualification is hereby removed from this project. However, bidders shall demonstrate in their bid response sufficient experience with concrete flat work; including but not limited to, sidewalks, entry aprons, curb and gutter. IV. SIGN-IN SHEET The sign-in sheet for the mandatory pre-bid conference held on Wednesday, May 11, 2016 is hereby attached. V. TRIPLET LAKE DRIVE IMPROVEMENTS PROJECT DETOURS Due to ongoing construction around the City Hall area, it is advised and strongly encouraged bidders review the information on detours associated with the Triplet Lake Drive Improvements Project. It shall be the sole responsibility of the bidder to determine most appropriate route to ensure on time delivery of your proposal response. Statement released by Public Works: Construction of Triplet Lake Drive Improvements Project has begun and will reconstruct Triplet Lake Drive from US 17-92 to east of the N/S Triplet Lake Drive intersection. Triplet Lake Drive from US 17-92 to Quail Pond Circle will be FULLY CLOSED for several months starting on or about March 21, 2016.
