(where applicable) HR-FRM-208 National Police Clearance Working With Children Check WA Identification as a person of Australian Aboriginal and/or Torres Strait Islander descent Key Responsibilities Additional Responsibilities (as appropriate) Establish and maintain links with local Aboriginal communities to encourage and support access to health services. Plan and implement culturally appropriate community health programs with particular focus on child and maternal health, increasing physical exercise, chronic disease management and support for Aboriginal people affected by mental health, alcohol and other drug issues. This will involve undertaking home visits, health assessments, and health promotion activities. Liaise with General Practitioners and primary health care teams to facilitate access to health services for Aboriginal clients including Aboriginal health checks, chronic disease care plans and follow up, care coordination, supplementary services and PBS co-payment measure. Liaise with WACHS SW Aboriginal Liaison Officers, Allied Health Professionals, Diagnostic Services, Specialist Services, community pharmacies and other services as required to facilitate access to health services for Aboriginal clients. Work constructively as part of the DSAH team. Develop and maintain strong working relationships with other primary health care organisations to implement collaborative health program activities for the benefit of local Aboriginal communities. For example, the South West Community Drug Service Team, WACHS SW and SWAMS. Respond to requests from members of the local Aboriginal communities for assistance with health related issues. Enter all client contacts into patient record keeping system. Participate in regular DSAH team meetings and Community Advisory Group meetings. Planning Participate proactively in GP down south s strategic and annual planning processes Participate in relevant data gathering processes as required for long term planning Service Delivery Provide all service delivery in accordance with relevant program guidelines, plans and contractual / statutory requirements Maintain service delivery records to allow for appropriate knowledge management and information sharing within the organisation Deliver services and contribute to GP down south programs and activities in a timely and efficient manner Reporting / Evaluation Comply with all relevant reporting requirements Maintain up to date records, in accordance with portfolios / key responsibilities. SELECTION CRITERIA Essential 1. Aboriginality is a genuine occupational requirement of this position under Section 50D of the WA Equal Opportunity Act. 2. Minimum of Certificate IV in Aboriginal Primary Health Care. 3. Demonstrated clinical skills and experience relevant to role. 4. Proven computer skills and experience in the use of Microsoft windows based products. 5. Demonstrated experience communicating and assisting ATSI peoples to access primary health care services. 6. Demonstrated experience communicating and working effectively with health service providers. 7. Conflict management and negotiation skills. Desirable Criteria 1. Demonstrated experience in delivering or being involved in the delivery and organisation of culturally appropriate community health promotion programs. 2. Ability to manage workload and work with a team to ensure quality support is provided to clients 3. Experience with Communicare clinical software Pre-requisites As part of the condition of employment you are required to advise if you hold any of the following: 1.
