3 Team Leader Integrated Primary Mental Health Care Duty Statement POSITION TITLE: STATUS: REPORTS TO: Team Leader Integrated Primary Mental Health Care Full Time Program Manager 1. OVERALL PURPOSE Covering the Goldfields region this position will deliver place-based low intensity support and psychological interventions to clients. Working within the WA Primary Health Alliance Stepped Care approach, The Clinical Lead will match client needs to the level of intervention required and will endeavor to complete regular outreach services as depicted by the needs of the service, communities and individuals incorporating community workshops, group activities and educational seminars. Low intensity psychological interventions will be performed by Case Coordinators, Case workers and Counsellors, dependent on assessed need. In addition, this position will provide accessible, responsive, high-quality and integrated community mental health services ranging from whole of population needs for mental health promotion and prevention, through to severe, persistent and complex conditions. 2. SPECIFIC RESPONSIBILITIES 2.1. Coordinates Intake, screen and assessments of programs prospective clients in consultation with the Executive Manager. This includes the following: 2.2. Clinical Lead in consultation with the and Executive Manager, ensures the agency provides good quality intake, screening, assessment, intensive, long-term, early intervention support for children, young people and adults referred by their GPs for Mental Health Concerns Works effectively within WAPHA stepped care model that treat the whole person through the provision of integrated care management, low intensity digital phone based services, place-based community support services, and contribute to a reduction in potentially preventable hospitalisations Ensuring details of referrals are entered and maintained as per requirements of the program; 2.5. Allocation of clients to the most appropriate level of intervention according to assessed need 2.6. Conduct assessments and complete safety plans with clients and allocated workers as required; 2.7. Follow specific guidelines, policies and procedures as stipulated in the WA Primary Health Alliance contractual obligations 2.8. Ensure that all required administration in relation to the program is completed as per Goldfields Mental Health Portal processes 2.9. Facilitate regular team meetings and attend internal and external meetings on behalf of the service where relevant Undertake management supervision and monthly clinical group supervision with staff working within these services Assist the Executive Manager and Program Manager in the maintaining of data/statistics. And any other information as required Consult with the Program Manager and Executive Manager in order to develop an outreach support group focusing on low intensity mental health specific activities and workshops across the region Available to work flexible work hours Monday to Friday and Saturdays as required Actively participate and assist with any promotional activities and events in relation to the service. 4 2.15. Ability to go on regular outreach trips across the region Represent the agency internally and externally if required by the Executive Manager, Program Manager or the Director Prepare reports to the respective Funding Bodies in consultation with the Executive Manager, as required by the funding agreements, for WA Primary Health Alliance. 3. Staff Management 3.1. Facilitate monthly team meetings within allocated program areas ensuring a standard agenda is followed and minutes of meetings are documented Facilitate monthly group supervision with each program team or cluster of programs ensuring the supervision provided aligns with the Whole of Agency Group Supervision Model developed by Professional Excellence (PE) Undertake three monthly management supervision with team members within designated programs In partnership with the PM actively participate in team members Annual Performance Review which includes preparation of a performance summary outlining findings from the documentation audit, client observations, group supervision, management supervision, team meetings and general day to day performance Be available to discuss staff concerns or queries in real time. This may include face to face, , telephone and after working hours communication (exclusive of program specific on-call requirements) Be available for on call duties according to specific program requirements and at the direction of the EM Ensure staff concerns (in particular related to client work) is addressed in a timely manner (preferably before close of business), ensuring the information/advice provided is accurate and in accordance with both practice and agency guidelines When required be open and willing to seek advice and support from the EM supervisors and Across Agency Team when making decisions or providing information that may have a significant impact on clients, staff, branch and agency Actively work towards building and maintaining a positive and proactive team culture that works effectively and efficiently to achieve the desired outcomes of the program and clients. 4. SERVICE DELIVERY 4.1. The main focus of the direct service delivery role is to triage, assist in assessment and case management of clients that meet Goldfields Mental Health Portal and the WA Primary Health Alliance outcomes. This is within the Team Leader s core programs Assist the Executive Manager and Program Manager to prepare structures and procedures to ensure the high quality service delivery of Goldfields Mental Health Portal 4.3. In consultation with the Executive Manager maintain and monitor records consistent with Centrecare and Funding Body reporting requirements Liaise with funding bodies if required by the Executive Manager Prepare reports to the funding body in consultation with the Executive Manager or Program Manager, as required by the funding agreements, for Goldfields Mental Health Portal Assist the Executive Manager in ensuring QA processes are fully operational within Goldfields Mental Health Portal 4.7. Assist the Executive Manager to facilitate a supportive and effective team Assist the Executive Manager in the maintaining of the data collection systems for services within this team Provide reports as required. 5. PROFESSIONAL DEVELOPMENT 5.1. Receive regular supervision and consultation as per agency policy Maintain professional reading and knowledge 5.3. Develop networks with referrers and professionals in consultation with the Executive Manager and Program Manager. Page 4 of 8

5 6. GENERIC RESPONSIBILITIES 6.1. Abide by relevant professional and agency ethics and agency policies and procedures Attend agency staff meetings, team meetings and other meetings as required Comply with Agency Health, Safety and Environmental procedures, such as ensuring the establishment and maintenance of a healthy and safe work environment that protects personnel, clients, facilities, equipment, visitors and the environment Contribute to and be part of the organisational culture, where team work, cooperation, client service, quality, safety, confidentiality and a positive work environment are the focus Ensure appropriate dress at all times in accordance with the requirements of the Centrecare Dress Code Policy Ensure conduct is at all times professional in manner and in accordance with agency expectations Ensure the efficient use and maintenance of materials and equipment Maintain a high level of skill in Microsoft Office and/or other similar computer packages relevant to the position Maintain a sensitivity toward and awareness of cultural diversity and act accordingly Maintain all professional accreditations, police clearances, licenses and refresher courses as per program/agency requirements Participate in professional development both internally and externally Report any problems, suggestions Respond to, action and record any complaints Any other duties as directed by your Manager. 7. 6 8. Essential Criteria 8.1. A tertiary or post-graduate degree (of minimum three years duration) in a relevant discipline in nursing, Social Work or equivalent, Mental Health Nursing or Aboriginal Health Work A minimum of 3-5 years experience in a mental health related service Demonstrated knowledge and understanding around working with vulnerable, disadvantaged and complex needs clients In depth experience and/or understanding of working with remote Aboriginal communities and their unique needs Experience of leading geographically dispersed teams Worked in a supervisory / leadership position, preferably in a Mental Health /Clinical setting; 8.7. High level of communication skills and ability to network with a range of key stakeholders, particularly GPs and Primary Health Care Clinicians. Page 6 of 8

National Police Certificate/Working with Children Check Cards It is a condition of employment for all positions at Centrecare that a valid National Police Certificate (issued within the past 12 months) is sighted prior to any job offer being made.
