5 Construction Quality Assurance is required per K.A.R (a)(9) for Construction and Demolition Landfills is required per KDHE-BWM Policy for ALL Kansas Solid Waste Landfills. 5
6 Definitions A. Construction Quality Assurance (CQA) is a system of activities designed to assure that landfills are constructed as specified in the approved design drawings and the approved CQA Plan (CQAP). B. The CQA Plan (CQAP) is a document prepared and sealed by a Kansas-licensed Professional Engineer (P.E.) that outlines the responsibilities of the various parties and activities that will be performed at the facility to verify that the construction of landfill disposal units and/or final cover is performed in accordance with the Design Drawings. 6
7 Purpose of CQA A. To ensure that solid waste is disposed of in a manner that is protective of the water, soil, and air of the state. 7
8 Construction/CQA Project Planning A.Pre-Construction Checklist for the Permittee/Consultant: 1. Notify the KDHE-BWM Permit Manager at least 6 months prior to construction of the need for capacity and your plan to construct. 2. Make sure an approved CQAP is on file and up to date. 3. Advise KDHE of your proposed Scope of Work - There are 3 types of Construction for Unlined Landfills: a. Construction of a NEW waste disposal area(s) for disposal b. Construction of Additional Waste on Top of a previously-filled area(s) c. Construction of Final Cover over a specified area(s) Note: All 3 types of CQA Projects require submission of a CQA Report for review and approval by KDHE-BWM. 1. Prepare Construction Drawings that show survey control and the limits of construction, and submit them to KDHE for review and approval. 2. Prepare a Cost Estimate that fits within your Budget. 3. Prepare and submit a Construction Schedule to KDHE-BWM. 4. Determine who will provide construction staking for the project(s). 5. Decide if a Pre-Construction Meeting is necessary. 8

Implement CQA activities: Observe and verify construction in accordance with the Construction Drawings and CQAP. 2. Prepare a project log Take good notes and measurements! 3. Take and log photographs. 1. Note the date, location, feature, and direction the photographer is facing. Note: Liners and Final Covers require a lot of materials testing that is not covered here. 9
10 CQA Project Planning (continued) C. Post-Construction Checklist for the CQA Engineer 1. Prepare and seal the CQA Report. 2. Obtain approval of the CQA Report and authorization from KDHE to dispose of solid waste into the newly constructed area. 3. Make sure that storm water run-on and run-off controls are in place prior to disposal of solid waste. 10

12 Permittee s Responsibilities a. Archiving and safe-keeping CQA Reports b. Advising KDHE of planned construction at least 6 months prior to any construction. c. Utilizing the most recently approved CQAP d. for ensuring that the construction drawings have been reviewed and approved by the KDHE prior to any construction. 12
13 Permittee Responsibilities (continued) a. For notifying KDHE: i. The name of the CQA Engineer, ii. The Contractor, iii. The proposed Scope of Work 1. illustrated in construction drawings. 13
14 Proposed plan to phase the cell excavation into three parts. First section at 48.00' wide has already been Excavated to grade. 14
15 Permittee Responsibilities (continued) 1. Address the type of construction that is proposed at the facility (see Slide 8): 2. Illustrate/define the construction limits of the disposal area for the proposed project construction, using coordinates. 3. The construction drawings typically provide more detail than the design drawings 4. The construction drawings must be in agreement with the approved Design Drawings. 5. The approved Design Drawings for a project may be used for construction drawings if approved by the KDHE-BWM Permit Manager. 15
16 Permittee Responsibilities (continued) i. The Permittee must to notify KDHE-BWM of the Scope of Work, construction drawings, and the construction schedule at least 2 months prior to any construction at the facility. ii. To make provisions for construction staking. a. Will submit a detailed project schedule beginning with mobilization and construction staking activities for the specific project. b. Shall review and approve the Scope of Work, Construction Cost Estimate, CQA Cost Estimate, and project schedule prior to any construction of a project or projects. c. Will preserve and archive all Permit Documents. 16
17 Responsibility of the Contractor a. Provides construction quality control (CQC) 17
18 Responsibilities of the CQA Engineer: a. To research previous facility CQA Reports and As-Built drawings relevant to proposed construction. b.knowing the project specifications. c.establishing tolerance criteria for horizontal and vertical control. 18
19 Horizontal control tolerance for the edge of a disposal unit is +/- feet. 19
20 Responsibilities of the CQA Engineer (continued) a. To implement the approved CQAP b. Direct supervision of all CQA activities, CQA personnel, and documents c. To verify that the project was constructed in the proper location and elevations shown in the Design and construction drawings. d. To address problems that arise during construction and will document (in the CQA Report) how the problems were solved. e. To prepare As-Built (or Record) Drawings. f. Comply with other agencies. g. To certify (using his/her professional engineer s seal) that construction was performed in accordance with the approved construction drawings and CQAP. 20

22 KDHE s Responsibility To review and approve CQA Plans and Reports for compliance with Kansas Solid Waste Regulations and Policies. The CQAP must be reviewed and approved by KDHE-BWM prior to any construction staking/construction. 22
23 CQA Report - is a stand-alone document (pdf) (no binder) A. Insert a copy of the approved CQAP in the CQA Report after the Table of Contents. B. Executive Summary 1. Detailed discussion of CQA activities from the construction staking to authorization for disposal by KDHE-BWM. C. Include your Project Log. D. Comment on horizontal and vertical control tolerances. 1. Horizontal and vertical control (location and elevation). Use project coordinates throughout to define As-Built locations. 2. Liner and final cover thicknesses (as applicable) E. Photographs F. As-Built (or Record Drawings) 1. Use project coordinates throughout to define As-Built locations. 23

C.Certifies (P.E. seal) that the project(s) was constructed and completed in accordance with the Design Drawings, construction drawings, and approved CQAP for the facility. 25
