1 DEPARTMENT OF THE ARMY U.S. ARMY CORPS OF ENGINEERS AFGHANISTAN ENGINEER DISTRICT - SOUTH APO AE REPLY TO ATTENTION OF USACE-EC-TSB 1 June 2013 (Expiration: 2 June 2015) 2013 INSPECTION AND CERTIFICATION FOR THE SHAKIB DASTAGGIR CONSTRUCTION & DESIGN COMPANY (S-CAD) MATERIAL TESTING LABORATORY LOCATED IN HERAT CITY, AFGHANISTAN This letter confirms the completion of 2013 inspection and certification for the S-CAD Material Testing Laboratory, located in Herat City, Afghanistan. While this laboratory is located in Heart, this facility will be approved to conduct field tests anywhere in the Afghanistan Engineering District (AED) South. The laboratory will also be recommended for use anywhere in AED North. The inspection/evaluation is based on ASTM lab checklists and procedures including facility, equipment, calibration, knowledge, personnel, references, reporting and attitude. The inspection reflected the lab s positive and professional reputation. The following page provides recommendation and/or requirements. This laboratory is certified for use by Afghanistan Engineering District South for the construction materials tests listed in the lab inspection checklist (attached) conditional on compliance with ASTM and USACE standards. This certification will be included with records that are maintained at the AED Headquarters in Kandahar, Afghanistan. Retaining certification will require periodic inspections by AED-S, as indicated by the certificate expiration date. This certification is also contingent upon the continued employment of Ghulam Dastaggir Karimi (Lab Manager), Ibrahim Danish (Deputy Manager), and Shakib Ahmad Mahvey (Sr. Lab Technician). Without the oversight of these gentlemen, the laboratory will require recertification. Also, if the laboratory moves to a new location, it will require recertification. The inspection and certification process for the S-CAD Laboratory adhered to procedures outlined by the Materials Testing Center (MTC), located at the U.S. Army Engineer Research and Development Center (ERDC) Vicksburg, Mississippi, USA. The MTC is the USACE agency for certifying labs for quality control testing for USACE construction projects. To facilitate construction in Afghanistan, AED-S has authorized these authors to conduct laboratory inspections and certifications with adherence to MTC standards. The inspection is a two-man day comprehensive review that includes review of the quality system, verification of test equipment, and observation of test procedures performed. If the lab is owned and operated by the prime contractor performing the USACE contract, a resume or CV of the Senior Geotechnical Engineer that would work permanently in that location is also required.
