2 CONTRACTOR SAFETY ENVIRONMENTAL, SAFETY, HEALTH AND SECURITY RULES AND REGULATIONS I. INTRODUCTION Welcome to PPG. The rules discussed in this manual are not all inclusive; other rules could be added as you perform your duties in the plant. Our greatest concern is the safety and health of our employees, contractor employees, and the preservation of the environment. We have a written Emergency Action Plan and Hazard Communication Program, Material Safety Data Sheets (MSDS's) for all raw materials as well as for all finished products, Safety and Health Index system Labels (SHIS Labels), and PPG's own product labels. Contractors are responsible for the training of their personnel. Process Safety The PPG East Point facility is covered under the US OSHA guideline for Process Safety Management (PSM). PSM is the application of management systems to the identification, understanding, and control of process hazards to prevent process related injuries and incidents. PSM applies to facilities that process or handle certain highly hazardous substances over the threshold quantities as well as facilities that process flammable liquids in excess of 10,000 pounds (4,356 kg). The PPG East Point facility does not store, use, transfer, handle, or process any of the Highly Hazardous Chemicals, Toxics or Reactives listed in the Appendix to PPG s Global Requirements for Process Safety: Applicability and Definition. The facility does, however process various flammable liquids in quantities of 10,000 pounds (4,356 kg) and greater; and is therefore covered under the standard. The East Point facility has three PSM covered processes: Paint Manufacturing, Filling and Port-A-Wash. Conducting contract work on any of these processes requires special instruction from your Project Coordinator. 1
3 1. All contractors performing work for PPG must adhere in detail to Safety, Health and Security rules and procedures. Failure to do so will be grounds for discharge of contract employees violating the rules and or termination of all existing contracts. The designated Project Coordinator will enforce all PPG rules and procedures. 2. All contractor employees must wear a badge or other identification supplied by PPG Security. 3. It is expected that the contractor shall be informed of and conform to all safety regulations of federal, state and local government or other regulatory agencies having jurisdiction. 4. Contractors must provide MSDS s to the PPG Project Coordinator for all chemicals brought onto the site. 5. The use of or possession of alcoholic beverages, drug paraphernalia or drugs on the controlled substances list are not permitted on PPG premises and are grounds for immediate discharge. 6. No individual may possess, use or distribute a weapon when on the facility grounds. The Administration at PPG East Point will act to enforce this policy and to discipline or take other appropriate action against any individual who violates this policy. Any reports of weapons shall be investigated. A "weapon" means any object, device or instrument designed as a weapon or, through its use, capable of threatening and/or producing bodily harm including, but not limited to, any firearm, whether loaded or unloaded, firearm ammunition, air guns, pellet guns, BB guns, all knives with a blade in excess of 3 inches, metal knuckles, explosives, fireworks, pepper spray and other propellants, stun guns, arrows, or any object that has been modified to serve as a weapon. 7. Any use of equipment, change of work methods, or movement of PPG material or equipment not described at the outset of work shall be cleared through the PPG Project Coordinator prior to performance of work. 2
4 8. All unusual incidents should be reported immediately to the PPG Project Coordinator. (Accidents, injuries, damage to property, fires, etc.) 9. While the contractor has workers or material on PPG premises, a 24-hour telephone number for the contractor shall be left with the PPG Project Coordinator. Likewise a 24-hour PPG number will be supplied to the contractor. 10. Special arrangements must be made with the PPG Project Coordinator for access to the plant beyond the normal workday, on weekends or holidays. 11. The contractor's employees will be permitted access only to those areas necessary for the performance of contractual work. Under no circumstances are the contractor's employees allowed to tour or roam the plant or to enter areas not necessary to the performance of their work. 12. The PPG Project Coordinator shall assign the following areas for use by contractor's employees: Entrance & Parking Areas Break & Lunch Areas Toilet Facilities Material Storage Area 13. No personal vehicles are allowed in the plant. Any vehicle, which must be driven into the plant, must be cleared through the PPG Project Coordinator. No more than two (2) company vehicles may be parked inside the perimeter fence. These vehicles must have the company name prominently displayed on the outside of the vehicle. All closed type vans, trucks, car trunks, toolboxes and other contractor vehicles and equipment are subject to inspection by a duly authorized PPG representative while on PPG premises. Any deviation of this procedure must be approved by the PPG Project Coordinator. 14. No PPG products or other PPG materials are for sale. 3
5 15. All equipment or appurtenances supplied by PPG or purchased for installation are the property of PPG. Excess materials shall not be removed from the plant without written permission of the PPG Project Coordinator. II. HOUSEKEEPING 1. Job site housekeeping is the contractor's responsibility, and the job site must be as clean and orderly as possible while work is being performed. 2. Flammable and combustible waste must be removed from the premises at the end of each working day if the PPG Project Coordinator so requires. The contractor is responsible for the proper disposal of all regulated waste unless prior arrangements have been made in writing with PPG. Sewers, ditches, trenches, and waterways are not to be used for disposal of waste. Dumpsters, and other normal plant rubbish receptacles, may be used only with the permission of the PPG Project Coordinator. 3. Should the contractor fail to remove rubbish, etc., such removal will be done by PPG at the contractor's expense. 4. At the completion of the work, the job site must be left in an acceptable condition. The PPG Project Coordinator will make a final inspection to determine the adequacy of the final cleanup. III. CONTRACTOR EMPLOYEE CONDUCT 1. The contractor is responsible for the conduct of his employees. 2. Horseplay is not permitted. 3. PPG East Point is a tobacco free site. Smoking and the use of tobacco products are prohibited throughout the plant including outside break areas, parking lot and inside vehicles on PPG property. 4
6 4. All forms of lighters and matches are not permitted in the plant 5. If a contractor is hired to perform work that is classified as Hot Work the contractor must follow a program that meets or exceeds that in place at the plant for Hot Work. A Hot Work Permit shall be required for each specific job requiring the use of open flames, welding, cutting, burning, grinding equipment, solder pots, roofer s kettles or explosive devices to shoot anchor bolts. This permit is one shift only and a new permit must be issued every shift. A Special Tool Permit shall be required for each specific job requiring the use of any spark producing tools or any corded or uncorded tools, etc. This permits is for ten (10) hours only and a new permit must be issued every ten (10) hours. The permit must be conspicuously displayed at the job location. These permits are not permission to smoke. Contact your PPG Project Coordinator for permit. It is the contractor s responsibility to provide all necessary material and equipment to safely conduct hot work. 6. If a contractor is hired to perform work in an area marked "CONFINED SPACE" the contractor must follow a program that meets or exceeds that in place at the plant for confined space entry. A Confined Space Permit will be required before beginning work. Contact your PPG Project Coordinator for permit. It is the contractor s responsibility to provide ALL equipment and training documentation necessary for the safe entrance and rescue of personnel including appropriate, properly calibrated air monitoring equipment. 7. If a contractor is hired to perform work on equipment the contractor must follow a program that meets or exceeds that in place at the plant for LOCKOUT / TAG-OUT of all energy sources. The contractor will provide all necessary locks and tags. 8. If a contractor is hired to perform work that will require opening a tank or line, the contractor must follow a program that meets or exceeds that in place at the plant for Line & Tank Opening. Contact your PPG Project Coordinator for permit. 5
7 9. Cell phones other personal communications devices, or entertainment devices are not allowed to be used in the facility. All electronic devices must be left in your vehicle or in the off position while inside the plant. 2-way radios may be permitted but must be rated for use in Class 1, Division 1 environments. Check with the PPG Project Coordinator. IV. PERSONAL PROTECTIVE EQUIPMENT 1. Safety glasses with side shields, substantial shoes (not tennis shoes/sneakers, sandals, etc,) are required in the plant. In addition, shirts with long sleeves and long trousers must be worn. No tinted safety glasses are permitted. No jewelry should be worn and long hair and/or beards must be secured. 2. Some areas of the plant require hearing protection. Signs will indicate where these areas are located. These areas are also identified by the orange entrance doors to the area and the orange stripe along the walls. Contractors working or walking through these areas are required to wear hearing protection. 3. The contractor is required to provide all personal protective equipment. Other PPE that is not listed may be required as determined by the scope of work. This should be discussed with the PPG Project Coordinator. 4. It is the responsibility of the contractor to enforce the use of required personal protective equipment by its employees. V. ACCIDENTS AND INJURIES 1. The treatment of injuries sustained by the contractor's employees shall be the responsibility of the contractor. 2. All injuries or illnesses sustained on PPG premises must be reported to the PPG Project Coordinator immediately. 3. Any property damage must be reported to the PPG Project Coordinator immediately. 6
8 VI. IN CASE OF EMERGENCIES 1. If the fire alarm is sounded, even if only briefly, all contractors must evacuate the work area and report to the main gate at the front of the plant. Workers are encouraged to shut down the equipment that they are using if this task does not delay their evacuation. 2. If a fire is detected, pull an alarm to report it, and then evacuate. 3. Fire extinguishers, hoses, hydrants, etc, must not be used for anything except fighting a fire. 4. If it becomes necessary to turn off a sprinkler system or use a hydrant or other firefighting equipment for anything except fighting a fire, the PPG Project Coordinator must be informed before this occurs. The FM Red Tag Process will be used and initiated only by the PPG Project Coordinator. 5. In case of a medical emergency, contact the nearest PPG employee and ask them to call a Code Blue emergency. 6. In case of a tornado warning, an announcement will be made over the PA to instruct you to get to an emergency shelter. Follow the nearest PPG employee to the emergency shelter. 7. In case of a chemical spill in the work area, evacuate the area and contact your PPG Coordinator. VII. CONTRACTOR'S TOOLS AND EQUIPMENT 1. All equipment/tools must be in first class condition. 2. All electrical or spark producing equipment or internal combustion engines must be approved for use in any area. Use of tools not properly rated is strictly prohibited. (Special Tool and/or Hot Work permit required) 7
9 3. Scaffold planks, ladders, ropes, ground wires, etc., shall be inspected by the contractor and replaced if they prove to be defective. 4. Portable metal ladders are not allowed in the plant. (Only fiberglass ladders are permitted) 5. All portable electrically powered tools and equipment shall be properly grounded. (Double insulated tools or the use of a grounding prong is acceptable.) Grounds for welding should be through the work piece rather than using the plant structure for ground. (Hot Work permit required) 6. Regardless of the type of fuel used, fuel-driven machines must be shut down for refueling. 7. Safety guards or other devices shall not be removed from tools or equipment except for repairs and must be replaced upon completion of repair. 8. The use of any photographic equipment within PPG facilities must be approved by the PPG Project Coordinator in writing with a special tool permit. 9. Thin mil plastics and flexible plastics can generate static electricity. As a result, these are restricted or prohibited in many areas of the plant. The use of these plastics needs to be cleared by the PPG Coordinator for use on PPG property. VIII. SAFETY NOTES 1. All hazardous openings or excavations must be barricaded at all times unless the work requires their temporary removal. 2. While overhead work is being preformed the area must be barricaded, or signs posted in a conspicuous manner "Danger- Work Overhead". If sparks from cutting, etc, or other materials are falling, the area below must be barricaded. 8
10 3. Work shall not be performed under or immediately adjacent to loads being hoisted and all loose items of equipment or material shall be secured from falling. 4. No riders are permitted on moving equipment, rigging, or loads. 5. Guy lines must be clearly marked where they cross the travel space of paths, walkways, or roadways. 6. Fire doors must not be blocked, fastened open, or wires, cables, hoses, etc, passed through a doorway that would prevent a fire door from completely closing. 7. Compressed gas cylinders must be securely fastened, and acetylene cylinders must be in the upright position. Oxygen storage shall be separated from fuel storage (20 feet or 5 feet high, ½ hour rated partition). 8. Jumping from docks, trucks, or other platforms is unsafe and prohibited. 9. Do not use packages, drums, bags, or other items in place of ladders. 10. Do not run inside plant. 11. Contractors shall report any malfunction or potential safety hazard to the PPG Project Coordinator. 12. Contractors are responsible for noting the clearance under pipe racks, conduit racks, doors, ramps and other such overhead obstructions and reconciling any possible interference with their equipment. 13. The contractor shall observe the plant speed limits when moving any type of vehicle. 14. Contractor personnel shall not connect to, adjust or otherwise tamper with PPG equipment. 15. Solicitation of any type is prohibited. 9
11 16. All excavation or similar personnel hazards must be visibly marked and barricaded. 17. Internal combustion engines may be left running at night or other unattended periods only by special permission from the PPG Project Coordinator. 18. Safety showers, fire extinguishers, and similar emergency equipment must be available at all times and kept free of material, equipment or other blockage. 19. Contract workers working in overhead platforms or riggers or on any surface (horizontal and vertical) with an unprotected edge which is 4 ft. or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems (safety harness) per OSHA (b)(1). 20. PPG will not allow contractors to use any of the company owned mobile equipment or tools. 21. Contractors must be aware of fork truck operations in the work area. CONTRACTOR NOTIFICATION ASBESTOS Asbestos is a naturally occurring fibrous mineral, found in two-thirds of the rocks in the Earth s crust. Asbestos has been added to materials such as roof coatings, insulation, tile and tile mastic, some ceiling materials, theater curtains, vehicle brakes, water and sewer pipes, elevator brakes, protective aprons and gloves along with other material. Asbestos is a fire retardant, which is one of it s qualities and one reason it had been added to construction materials. Because of health effects associated with asbestos the material is no longer used in construction and hasn t been since January Because our building was built prior to this date there were materials used in the construction which included Asbestos Containing Materials (ACM). Some of the health effects include Asbestosis, Lung Cancer*, Mesothelioma. The National Academy of Sciences and the International 10
12 Agency for Research on Cancer have concluded that a low level exposure does not pose a significant public health risk. *Smoking significantly increases the risk of lung cancer when working with asbestos containing materials. Very few lung cancer cases are found in nonsmokers. These conditions can occur after the fibers have been inhaled when it is in a friable state (easily crumbled). The fibers may lodge in the lung or body tissue and because the body cannot break down or remove the fiber once it is lodged over time one of the above mentioned conditions can occur. Asbestos is only a threat when it is in a friable state and the fibers can be released into the air. When it is encapsulated or wet there is no threat. Many asbestos containing materials have been removed from the East Point facility. However there are areas where it still exists. Information regarding the exact location of asbestos containing materials at the East Point facility has been reviewed with me by my manager or the facility project manager. The site management at the East Point location have taken measures to ensure employee safety but should you notice loose or broken materials known to contain ACM please leave it alone and notify your Project Manager or the plant EHS Manager. PPG East Point requires the following procedures to be followed with regards to care of asbestos containing material located in this facility. Sanding of asbestos containing floor material is prohibited. Stripping of finishes shall be conducted using low abrasion pads at a high speed lower than 300 RPM and wet methods. Burnishing or dry buffing may be performed only on asbestos containing flooring which has sufficient finish so that pad cannot contact the asbestos containing material. Dust or debris generated from visibly deteriorated asbestos containing flooring material shall not be dusted or swept dry, or vacuumed without using HEPA filter. 11

18 PPG EAST POINT CRITICAL SAFETY ISSUES 1. NO SMOKING is permitted anywhere inside the perimeter fencing and structures on plant property. This includes buildings, yards, or inside of vehicles. 2. The use of cell phones, cameras, pagers 2-way radios or other electrically operated equipment is prohibited anywhere inside the perimeter fencing and structures on plant property without prior permission from your PPG contact. 3. In the event of an emergency, exit the building by the nearest exit and circle around the outside of the building and report to the guard for headcount. For weather emergencies, wait for direction from your PPG contact.
