1 ADDENDUM NO. 2 CITY OF FORT MORGAN BUILDING MAINTENANCE DEPARTMENT CITY JANITORIAL SERVICES November 21, 2017 To all prospective bidders: This Addendum is part of the Bid Documents and modifies the original Bid Documents. City Clarification: Exhibits A, B and G have been revised as attached. Bidders should field verify square footages of buildings. The successful bidder is responsible to move/clear any and all areas of tables, chairs, etc., in order to perform required services. Disregard additional carpet shampooing on the bid form for the Senior Center as follows: SENIOR CENTER MONTHLY COST G. DESCRIPTION The approximate square footage of the building is 9456 SF consisting of 5 offices, 4 restrooms, 1 lobby area, 1 aerobics, 1 teen room, break room, and 2 small entry ways. Of the 9456 SF, the Recreation Center has approximately 828 SF of hard/resilient surfaces. B. BASIC SERVICES Holidays are as described in the Instructions to Bidders, Section III.B. Services to be performed during the business hours of 1:00 p.m. to 5:00 p.m. BASIC SERVICES Task Description Armory Daily Weekly Monthly Quarterly Yearly General, Private Offices, Lobby, Lounge Empty trash receptacles Replace trash receptacle liners Dust all furniture including desks, chairs, and tables Low dust all horizontal surfaces to hand height (70 ) including sills,, moldings, shelves, picture frames, ducts radiators, etc. Spot clean reception lobby including from door and any other partition or door Clean entire interior in partitions and Dust venetian blinds Remove dust and cobwebs from ceiling areas Dust mop resilient and hard floors or vacuum carpeted floors in traffic lanes only Damp mop resilient and hard floors Wash interior and exterior windows 2 Year Spot clean spills and stains on carpeted and resilient floors Vacuum furniture Spot clean furniture Shampoo carpet in entryway and offices Shampoo carpet in aerobics room 1 Year Machine scrub and apply 2 coats finish 1 Year to hard/ resilient flooring 1

Refill sanitizing spray bottles in weight room. Low dust all horizontal surfaces below 36 including sills, moldings,, shelves, frames, ducts, and heating outlets Sweep, damp mop and sanitize hard floors including stairs Pressurized deep clean and sanitize II.
