We are excited to have you join our team as we work together to fulfill our mission of providing A Community of Healing, Commitment to Excellence and Collaboration with Caregivers. If you are interested in learning more about Providence Health Care prior to your start date, please visit our website at: Attached is a Campus Map and Directions to Sacred Heart Medical Center where your Day 1, New Employee Orientation will be conducted. On your second day, you will be required to obtain a badge and attend an online Lync session to Providence Journey 1. We have enclosed additional information that we ask you to complete prior to orientation. Please write down any questions you have for orientation. Please complete the forms listed below and bring them on your first day: Employment Eligibility Verification (I-9) Please make sure you bring the necessary piece(s) of ID listed on this form to Providence Journey 1 New Employee On-boarding session. Please complete Section 1 of the form and bring it with you. This is a Federal form and all employees are required to complete this within 3 days of hire date. New Hire Self-Identification Form Complete all fields. Submission of this form is voluntary, should you choose not to complete this form please print your name and simply write decline on the front and bring it with you to Providence Journey 1 onboarding session. 2 Orientation Directions Refer to your offer letter to determine which Providence campus, Sacred Heart Medical Center or Holy Family Hospital, that your New Employee Orientation will be held at. Directions to both locations are included below. Getting to Holy Family Hospital: New Employee Orientation held in the Health Education Center From Westbound I-90: 1. Take exit number Continue on Division street going North 3. Turn RIGHT on Rowan 4. Turn LEFT onto Lidgerwood. 5. Park and walk into the main building of Holy Family Hospital. 6. Take the main elevators down to the cafeteria and go to the health education center. From Eastbound I-90: 1. Take exit number Stay in left hand lane and follow Division street North. 3. Turn RIGHT on Rowan. 4. Turn LEFT on Lidgerwood. 5. Park in main parking lot and walk into main building of Holy Family Hospital. 6. Take the main elevators down to the cafeteria and go to the health education center. From the North (Highway 2 or 395): 1. US-395 and Hwy 2 merges into Division St 2. Follow Division street South. 3. Turn LEFT onto Rowan. 4. Turn LEFT onto Lidgerwood. 5. Park in main parking lot and walk into Holy Family Hospital. 6. Take the main elevators down to the cafeteria and go to the health education center. Turn LEFT onto Rockwood Blvd and park in TOP level of parking garage and walk across the street to Mother Gamelin Center. From Eastbound I-90: 1. Take exit number Turn RIGHT at the light on Division St th 3. Turn LEFT on 8 4. Turn RIGHT on Cowley 5. Turn RIGHT onto Rockwood Blvd until you reach the top level of the employee parking garage. Park and walk across the street to the Mother Gamelin Center. From the North (Highway 2 or 395): 1. US-395 and Hwy 2 merges into Division St 2. After crossing the Spokane River, turn LEFT onto Browne St and merge to right lanes. 3. Continue up Browne St, moving to the left lane (Browne will become McClellan) 4. Turn LEFT onto Rockwood Blvd and follow up to top level of employee parking garage. Park and walk across the street to the Mother Gamelin Center..

Providence Code of Conduct Booklet received. (Electronic Version) 8. Acceptable Use Agreement and Commitment to Code of Conduct signed/submitted. 9. Automatic Payroll Deposit Form Direct deposit is a mandatory requirement of employment with PSMS. 10. W-4 Form Completed Online - shown how to access online - If W4 information is not completed, a default status of Single with 0 Allowances will automatically be used to calculate your Federal Taxes. 11. Payroll and Holiday Calendar received. 12. Shown how to access ProvConnect, Employee Self-Service (ESS), Kronos, HealthStream and employee policies. 13.
