1 The Change Leaders Programme Are you a leader and catalyst for change, focused on delivering improved patient experience and quality of care across the health and care system? Application Form Change Leaders

2 The Change Leaders Programme Great health care needs great leaders and leadership. Change Leaders is a high impact programme aimed at developing the leadership capability of clinical and nonclinical leaders from primary and secondary care to enable quality of care, service transformation and innovation across the East of England. The programme is specifically aimed at those who are leading business-critical quality and service improvement projects and programmes. It builds on current best practice to enable participants to develop the leadership styles, tools and techniques needed to enable change across systems. We welcome applications from individuals and from teams of participants who are collaborating on a specific project or programme. Participants in the programme will already have some leadership experience, and are likely to be operating at Band 7 and above (or similar level). By leadership we don t necessarily mean team leadership - this programme is about leading in the context of projects and programmes, and is open to matrix leaders who do not have any direct reports. Application form This is an opportunity for both clinicians and managers who are leading quality and service improvement projects and programmes. We welcome applications from individuals and multi-professional teams across the East of England. Applicants have responsibility to ensure that Section 4 of this application form is completed by their sponsor. I have seen my attendee change behaviours as a consequence of the diagnostic feedback and subsequent learning. 4 About your project Health Education East of England aims to ensure that the NHS workforce has the capabilities it needs to deliver the NHS Mandate, which focuses on the areas that matter most to people - our Healthcare priorities. Each participant on the Change Leaders programme will be leading a quality or service improvement project which supports delivery of these priorities. 5 Section 2: Supporting evidence In this section we ask you to provide further information about your quality or service improvement project, and why you are applying to take part in the Change Leaders programme. Please note that these responses to these questions are scored as part of the selection process so please ensure that you give full and specific answers which demonstrate how participation in the programme will benefit you and your organisation. 7 Section 3: Programme dates 1. If your application is successful, you will need to be available to take part in a one-day development centre. Please identify all dates that you are available by putting 1 for your first choice, 2 for your second choice, etc. and hold these dates until you are notified of the outcome of your application. Ideally please aim to attend the date in the area that you work in, and to attend with team members where applicable. The exact locations for the centres may be subject to change, but they will be in the area of the locations indicated. Below are dates for programme delivery, along with the area in which the modules will be delivered. Please note that these are subject to change but at this stage our intention is to use these as the central base for each cohort. You will need to ensure that you are free for all of the dates in the cohorts that you choose; ideally these should be the geographical area or close to the geographical area that you work in. This may involve accessing coaching and/or mentorship, in addition to involvement with local networks of support, including your sponsor, line manager, and/or past Change Leaders participants. It may also include self-directed learning, for example accessing online materials or attending other local master classes and events. You will also be applying the learning from the programme by testing tools and techniques out in the workplace and bringing your experiences back to share with fellow participants. In order to get the most from the programme, we would expect participants to spend around 8 hours a month on additional development activities of this sort. 7 NHS Health Education East of England Change Leaders Programme Application Form

8 Section 4: Sponsor support We ask all participants to provide details of a senior leader in their organisation who will act as a sponsor for them throughout the programme. This must be someone who has the authority to release you from the organisation for all of the scheduled activity. They will also ideally be someone who has a direct interest in the quality or service improvement project you are leading. The sponsor s role is to support you throughout the programme, to help you meet both your personal development goals and to support your project. They will also help you to ensure that your project remains organisationally relevant and integrated into the wider work of the organisation. In addition to providing this ongoing support throughout the programme, sponsors should also attend the opening and next steps events alongside the participant. The dates for these events are below.
