In accord with the Community Right -to- Know {CRTK} Program, this publication is also available to anyone interested in the emergency planning and local risk assessment process for hazardous materials, threats to life, property and the environment. Detailed instructions on determining the need to reports and relevant regulations are included. Beginning January 1, 2017 the Anchorage Fire Department will require all Community Right To Know Tier II reports to be filed electronically using the State of Alaska Tier II Database at The Anchorage Fire Department will no longer accept paper forms, CDs or /DVDs submissions. The MOA reserves the right to request a complete inventory of hazardous materials at any time. Per Municipal Ordinance, a site map is required annually with each submittal. A photograph showing the location(s) of your NFPA 704 Placard is required the first time you file and every subsequent even-numbered report year. The annual filing deadline is March 1. Tier II forms received using the State of Alaska Tier II Database will automatically be submitted to the Anchorage Fire Department (which fulfills submission to the LEPC [Local Emergency Planning Committee]) and the State of Alaska Department of Environmental Conservation. 4 IMPORTANT DETAILS FOR CTRK SUBMITTER Who Must Submit The Form If the facility is required by Occupational Safety and Health Administration (OSHA) to keep any Material Safety Data Sheet (MSDS) or Safety Data Sheet (SDS) on file for hazardous chemicals present at the facility, and one or more of the chemicals is stored in quantities equal to or greater than the threshold level (page 7) for that chemical, then you must file a Tier II form. Section 312 of Title III requires that the owner or operator of a facility submit the Tier II form if so requested by a State Emergency Response Commission (SERC), a Local Emergency Planning Committee (LEPC), or a fire department with jurisdiction over the facility. In Anchorage, the Anchorage Fire Department has jurisdiction. The purpose of this Tier II form is to provide State and local officials with specific information on hazardous chemicals present at your facility during the past year. Submitting your Tier II with the Anchorage Fire Department fulfills your to Local Emergency Planning Committee (LEPC) filing What Chemicals are Excluded? AS (6) excludes the following substances: (A) A food, food additive, color additive, drug, or cosmetic regulated by the federal Food and Drug Administration; (B) A substance present as a solid in a manufactured item to the extent exposure to the substance does not occur under normal conditions of use; (C) *A substance to the extent it is used for personal, family, or household purposes, or is present in the same form and concentration as a product packaged for distribution and use by the general public; (D) A substance to the extent it is used in a research laboratory or a hospital or other medical facility under the direct supervision of a technically qualified individual; or (E) *A substance to the extent it is used in routine agricultural operations or is a fertilizer held for sale by a retailer to the ultimate customer. * All Consumer commodities in quantities of 1,000 pounds or more when present at the facility longer than fourteen (14) days are not excluded from reporting. OSHA regulations, Section (b), stipulate exemptions from the requirement to prepare or have available an MSDS or SDS. When To Submit This Form Owners or operators of facilities that have hazardous chemicals on hand in quantities equal to or greater than threshold levels, listed on page 7f, must submit a Tier II for by March 1st. The reporting period covers the activity of the previous calendar year (i.e., calendar year 2016 reporting is due March 1, 2017). How To Submit A Tier II Form Beginning January 1, 2017 the Municipality of Anchorage will require all Tier II reports to be filed electronically using the State of Alaska Tier II Database at The Municipality of Anchorage will no longer accept Tier II paper forms, CDs or DVDs. All Tier II paper forms, CDs or DVDs will be returned and may be subject to additional fees. 5 Instructions for Completing the Tier II Form Please read these instructions carefully Register You must register if this is your first time accessing the State of Alaska Tier II Database at You will receive an once your registration is approved. State of Alaska Tier II Database Modules 1. Login using your username and password 2. Click Facilities 3. Double click your facility located under Facility Name or select your facility located under Facility Name and click Edit 4. The database contains 10 modules, complete all modules unless indicated otherwise Address Module Provide the complete address of the facility where the hazardous chemicals are stored. Complete all fields marked with a red asterisk. Click Save, click OK and proceed to the next module. Parent Company (This module is optional) Enter the name, mailing address and address of the parent company. Click Save, click OK and proceed to the next module. Phones Module Provide a general phone number for the facility. Click Add, select Work from the drop down menu and enter the facility s 10 digit work phone number, click save. Click Save, click OK and proceed to the next module. Contacts Module A Tier II Information Contact and Owner/ Operator contact are required. If you are reporting an Extremely Hazardous Substances (EHS) an Emergency Contact, including a 24hour phone number, is also required. For a complete list of Extremely Hazardous Substances (EHS) refer to the EPA Consolidated List of Lists at Click Add and complete all fields marked with a red asterisk for the facility owner/operator, select owner/operator for the Contact Type. Select the Phones tab, click Add, select Phone Type and enter the owner/operators 10 digit phone number and click save. Click Save, click Return to Facility and return to the Contacts module. Click Add and complete all fields marked with a red asterisk for the Tier II Information Contact, including selecting Tier II Information Contact for the Contact Type. Select the Phones tab, click Add, select Phone Type and enter the Tier II Information Contact 10 digit phone number and click save. Click Save, click Return to Facility and proceed to the next module. 2
6 Chemical Inventory Module This is the main module of the Tier II database. This module requires specific information relevant to the chemical physical state, hazard, health effects, quantity and location. For each reportable chemical you will need the chemical or chemical mixture name and the Chemical Abstract Service (CAS) number located on the Material Safety Data Sheet (MSDS) or Safety Data Sheet (SDS). Click Add, enter the chemical name in the Chemical Name field and press enter (this will automatically populate the CAS number in the CAS # field). For chemical mixtures enter the CAS number of the mixture as a whole, if it has been assigned a number distinct from its constituents, and enter the name of the mixture in the Chemical Name field. For a mixture that does not have a CAS number, leave the CAS # field blank and enter the name of the mixture in the Chemical Name field. Check the In Inventory box if the facility is a non-transshipment facility or check the In Transit if the facility is a transshipment facility. Transshipment facility means any land, building, equipment, structure, stationary item or combination thereof that is located on a single site or on contiguous or adjacent sites owned, leased, operated or otherwise controlled by the same person whose principal business consists of the handling of materials including handling hazardous materials or hazardous wastes while such materials remain in the stream of interstate, intrastate or international commerce. Indicate if this chemical is an Extremely Hazardous Substances (EHS). For a complete list of Extremely Hazardous Substances (EHS) refer to the EPA Consolidated List of Lists at version. For any mixture containing an EHS that the facility is reporting as a mixture, the facility must check the box yes to indicate that the mixture contains an above-threshold EHS. You must also include the name of the EHS(s) contained in the mixture in the Chemical Name field. Check the boxes for the appropriate Physical State, Hazards and Health Effects, this information can be obtained from the chemical MSDS or SDS. Hazard Category Comparison for Reporting EPA s Hazard Categories OSHA Hazard Categories Fire Hazard Flammable, Combustible Liquid Pyrophoric, Oxidizer Sudden Release of Pressure Reactive Immediate (Acute) Health Hazards Delayed (Chronic) Health Hazards Explosives, Compressed Gases Unstable Reactive, Organic Peroxide Water Reactive Highly Toxic, Toxic, Irritant, Corrosive Other hazardous chemicals with an adverse effect with short term exposure. Carcinogens Other hazardous chemicals with an adverse effect with long term exposure 3
7 Check the Chemical information identical to previous year if this chemical information is identical to last year s submission. Do not check this box if this chemical was not reported last year or was reported in a different quantity or location. Enter the number of days that the hazardous chemical was present on-site. If you are reporting hazardous chemicals below the reporting threshold levels (see page 7 for threshold levels) check the Below Reporting Threshold box. Calculate all chemicals amounts in pounds, do not report weight in other units of measures. To convert gas or liquid volume to weight in pounds, multiply by the appropriate density factor or contact your vendor. For the Max Daily Amount (lbs.) estimate and enter the greatest amount present at the facility on any single day during the reporting period. Using the drop down menu select the applicable range value for the Max Code. For the Avg. Daily Amount (lbs) estimate and enter the average quantity that was present at the facility during the reporting period. Using the drop down menu select the applicable range value for the Max Code. For the Max Amt. in Largest Container (lbs.) estimate and enter the amount stored in the largest container at the facility. Using the drop down menu select the applicable range value for the Max Code. Click Save and select the Location tab. Under EPCRA Section 324, you may elect to withhold location information on a specific chemical from disclosure to the public. If you choose to do so, check the box marked yes and complete and attached the Confidential Location Information Sheet in the Attachment Module. The Confidential Location Information Sheet can be found at EPA s website at: Attached the form in the Attachment Module If the location(s) is non-confidential you will need to provide a brief description of the precise location of the chemical, so emergency responders can locate the area easily. Click Add and enter the location and the max amount of the chemical. Using the drop down menus select the storage container type and the applicable pressure and temperature. Click Save. Repeat if the chemical is present in more than one building, lot, or area location. List each location as appropriate. Click Save, click Return to Facility and return to the Chemical Inventory module, if you have additional reportable hazardous chemical or Click Save, click Return to Facility and proceed to the next module. ID & Regs. Module The facility Dun & Bradstreet and NAICS numbers are mandatory. The Dun & Bradstreet number is a Data Universal Numbering System (D-U-N-S) used as a standard business identifier number for each business physical location. If you do not know your number or if you need a number assigned, contact the regional office of Dun & Bradstreet at or at Click Add, select Dunn & Bradstreet from the ID Type dropdown menu, enter your Dunn & Bradstreet number in the ID Value and click Save. The primary North American Industry Classification Systems (NAICS) code is the standard used to classify business establishments. If you do not know your number contact the State of Alaska Business Licensing office at or go to Click Add, select NAICS from the ID Type dropdown menu, enter your primary NAICS number in the ID Value and click Save. 4
8 Indicate if the facility is manned or unmanned. If the facility was manned at any time during the report year, including if the facility was manned at least part of the day, check the Manned box. The Unmanned box should only be checked if the facility was never manned during the report year. Enter the maximum number of occupants on site at any one time in the Maximum No. of Occupants field. Include contractors, vendors and people that may be present for any training or other events as well as employees. If the facility is unmanned enter the number zero. Indicate if the facility is subject to the emergency planning notification requirement under EPCRA section 302, codified in 40 CFR part 355. Checking the Yes box indicates the facility maintains at least one Extremely Hazardous Substances (EHS) at or above the threshold level of 500 lbs. For a complete list of EHS refer to the EPA Consolidated List of Lists at version Indicate if your facility is subject to chemical accident prevention provisions under section 112(r) of the Clean Air Act, also known as the Risk Management Program (RMP), codified in 40 CFR part 68. Facilities regulated by the CAA are required to coordinate emergency response actions with their Local Emergency Planning Committees (LEPC). For a complete list of chemicals and hazards subject to CAA, refer the EPA Consolidated List of Lists at ( Click Save and proceed to the next module. Map Data Module This module will automatically populate the latitude and longitude based on the faculty s physical address. Select Degrees and click Geocode Address. Click Save and proceed to the next module. Attachments Module Per the Municipal Ordinance a site plan/map, showing the location of all reportable hazardous chemicals, is required annually and a photograph of the posted NFPA 704 Placard is required the first time you file and every subsequent even-numbered report year. Click Add, select Other from the Document Type dropdown menu, upload your photograph of the NFPA 704 placard and click Save. Click Add, select Site Plan from the Document Type dropdown menu, upload your site map and click Save. If you choose to report one of the following check the appropriate box and attach the document(s). A) A site plan with site coordinates indicated for buildings, lots, areas, etc. throughout your facility. B) A description of dikes and other safeguard measures for storage locations throughout your facility. C) A list of site coordinate abbreviations that correspond to buildings, lots, areas, etc. throughout your facility. Click Save and click Return to Facility Note Module This module is for official use only, please do not write in this area 5
9 Checklist Module Check the MSDS received with Tier II forms box if your MSDS or SDS are attached in the Attachments module. MSDS or SDS are required the first time you file or if requested by the CRTK Administrator. Check the Check if all facility information (not including chemical information is identical) to last year s submission box if the facility information, not including chemical information, is identical to last year s submission. Do not check this box if this is the first time filing. The owner, operator or the officially designated representative of the owner or operator must certify that all information included in the Tier II submission is true, accurate, and complete. Enter the current date in the Date Tier II Signed field and your name in the Signature field. Do not enter the date or time in the Date Tier III Received field, the database will automatically update this field after you submit your Tier II form. Click Save and proceed to the next module. Submit Review the Tier II form prior to submittal. Click Save and click the orange Submit icon. If your Tier II form is accepted you will receive a dialog box indicating your form was submitted. Click the Home icon and click Log Off. If your Tier II form was not accepted you will receive a dialog box indicating errors with the modules. Correct the errors, click Save and click the orange Submit icon. Click the Home icon and click Log Off. Please be advised an acceptance of your Tier II form does not imply approval. You will be notified if your form was approved or if it contains any additional errors. Do Not send payment to the Anchorage Fire Department, you will be billed by our billing company. 6
10 ANCHORAGE HAZARDOUS MATERIALS REPORTING THRESHOLD LEVELS All facilities within the Municipality of Anchorage must report the following materials in the quantities specified below. Please note that some terms have specific definitions in EPA or DOT regulations. 1. ANY quantity of the following materials: A. Explosives Class I, Division 1.1 B. Explosives Class I, Division 1.2 and 1.3 {excludes black powder, smokeless gunpowder and ammunition} C. Poison Gas Class 2, Division 2.3 D. Poison Class 6, Division 6.1 E. Flammable Solid Class 4, Division 4.1, 4.2 and 4.3 F. Radioactive Class 7 2. A Hazardous Chemical or material requiring an MSDS or SDS, if handled in a single day, in quantities of 4,500 pounds or more. 3. An Extremely Hazardous Substance in quantities of 500 pounds or more, or the Threshold Planning Quantity, whichever is smaller. 4. A Hazardous Waste in quantities of 220 pounds or more. 5. An Acute Hazardous Waste in quantities of 2.2 pounds or more. 6. Compressed Gas with a hazard class rating of 1 or 2 in any category under NFPA 704 Standard Placard system for the identification of the fire hazards materials in quantities of 1,000 cubic feet or more. 7. Compressed Gas with a hazard class rating of 3 or 4 in any category under the NFPA 704 Standard Placard system for the identification of the fire hazards of materials in quantities of 200 cubic feet or more. 8. A Consumer Commodity of a hazardous chemical or material in quantities of 1,000 pounds or more when present at a facility longer than 14 days prior to placement for display or sale. 9. Reporting quantities for retail gasoline stations only: Gasoline stored in compliant UST s 75,000 gallons; Diesel stored in compliant UST s 100,000 gallons.
