Instructions on proper usage to ensure safe blood transfusion By Julie G. Jackson MT(ASCP)SBB Revised 1/28/14 JJ Reviewed: MR Jones 1/28/14
Blood product storage is regulated by These regulations require a tight temperature range: 1-6 C Units of blood and plasma must be kept between 1-6 C Any unit returned to the blood bank over 6 C will be discarded. One patient in a cooler. Do not use any refrigerator or device other than the regulated blood bank cooler for storing blood products.
Red Blood cells and proteins in the plasma are very fragile They require strict handling When units get too warm the components start to break down and it also allows any bacteria that may be inside to proliferate and potentially cause sepsis
Packed red cells and Plasma (FFP) go in the RED cooler (1-6 C) Platelets and cryo go in the BLUE cooler(room temp products)
Red cooler- Blood & Plasma Blue cooler- Platelets & cryo Blood Bank coolers are validated to ensure that the blood products are kept at the regulated temperatures mandated by the FDA
The RED coolers consist of 2 parts The outer cooler shell (insulates the insert) The insert (which is kept at 1-6 C)
The insert The insert is conditioned in the Blood Bank in a regulated freezer and then stored in a regulated refrigerator until ready to use. These CAN NOT be conditioned or stored in a freezer or refrigerator OUTSIDE of the Blood Bank.
The insert lid must be tightly closed to maintain temp Properly closed insert lid No gap, it is sealed tightly This lid is ajar allowing the units to warm up above 1-6 C Even a small gap will let in warm air
The BLUE coolers consist of only 1 part The outer cooler shell (keep the temperature consistent at room temperature)
NEVER take the blood out and lay on the counter or bed NEVER take off the insert lid and leave the unit sitting Only remove unit when all vitals and checks are done and you are ready to give the unit Room temperatures can be very warm and the blood will warm up very quickly
ONE! Only 1 patient at a time can be in a single cooler. Each patient must have their own cooler. Patient 1 Patient 2
These coolers are validated for 10 hours. Blood Bank will write the time the cooler needs to be returned on a yellow piece of tape. If blood products are still stored in cooler, return to Blood Bank. If blood products are not stored, cooler can be returned to Nursing station.

This is an example of what information you will see on the screen.
Last year WFBMC lost $ 93,000 due to blood products being handled incorrectly Financial vitality is important for us to succeed Please be responsible and treat your blood products right! This will not only impact patient care but also our financial situation
Any units NOT transfused must be returned in your regulated cooler to the Blood Bank ASAP You can not transfer blood from one cooler to another. You must return the cooler to Blood Bank and get a new cooler or insert. Cooler 1 Cooler 2
LETS USE THE BLOOD BANK ISSUE FORM AS THE FLAG TO TELL US IF COOLER IS EMPTY. Remove BB Issue form from lid and discard = no product in cooler Leave BB Issue form on lid = product in cooler, returned to Blood Bank
LETS USE THE BLOOD BANK ISSUE FORM AS THE FLAG TO TELL US IF COOLER IS EMPTY. WHY? This is recognized as a risk on a busy Nursing unit. We recently had a platelet unit left in a floor cooler unknowingly and discovered when the cooler was used again to pick up another unit in Blood Bank. You also can always look inside
The Blood Bank coolers have an RFID tag attached. The RFID is programmed to recognize when it is at Home Base. Store cooler at Home Base with RFID facing outwards Home Base needs to be determined and communicated to all staff. Coolers when not in use must be returned to Home Base or will alarm in Blood Bank after a period of time. The coolers for blood and plasma are red as pictured. Coolers for platelets or cryo will be smaller blue coolers. The correct cooler must be brought to Blood Bank for the product being requested.
