2 Hazardous Waste Consignment Note Completion Guide This guide is to assist in how a Hazardous Waste Consignment Note should be completed. When is a Hazardous Waste Consignment Note needed? A Hazardous Waste Consignment Note is required each time hazardous waste is moved from Site to a Receiver. The Hazardous Waste Consignment Note is needed for the movement of the following wastes: Light Tubes Oil Refrigerant Coldroom Panels How do you complete the documentation? PART A Notification details This section should be completed at the time of removal by the person removing the waste i.e. the engineer. 1. Consignment Note Code This should start NYZ753 which is our premises code, followed by a unique reference number. Note: Hazardous Waste Consignment Note supplies for England & Wales can be obtained from customer.reporting@integral.co.uk (the waste consignment notes are uniquely numbered). 2. Waste Removed From This is the address from where the waste is coming from. 3. Premises Code This should be the premises code for the site you are collecting from. Note. If this is unknown, we can use our premises code (NYZ753), the environmental agency will allow us to do this for sites that do not have their own code. 4. Waste Taken To This is the address from where the waste is going to. 5. Waste Producer Only needs to be completed if this is different from the address where the waste is being removed from (#2)

3 PART B - Description of the Waste This section should be completed at the time of removal by the person removing the waste i.e. the engineer. 1. The Process Giving Rise to the Waste(s) was: The following choices should be used for this part; Recovery Decommission Service Maintenance 2. SIC for the Process Giving Rise to Waste: England and Scotland the SIC code is not compulsory and therefore you do not need to fill this section out, however if you do use the 2003 codes and NOT The 2003 codes for disposal of refrigeration equipment and collection of hazardous waste is Waste Details Please complete this section by referring to the below table for the waste you are removing Description EWC Codes Qty Component Concentration Form Hazard Code Refrigerant Mixed Up to 100% Gas H14 Container No & size Oil Liquid Up to 100% Oil H14/H7 Light Tubes Mixed 0.1% Tubes H5 Insulated Panels Solid H14 Display Cases Solid H14 For each EWC code identified in the first table please refer to the below detail in order to the second table in this section. 4 PART C Carrier s Certificate This section is to be completed by those transporting the waste to its next destination. I.e. By the engineer that is moving the waste to Climate or by the Climate driver picking up the waste from site. 1. Carrier Name This is the company name of those transporting the waste, i.e. Climate or Integral. 2. Carrier Registration This section has already been filled out on the forms provided by Customer Reporting for instances when Integral are the carrier for transporting the waste. Integral s registration number is: CB/PM3180LF. 3. Vehicle Registration This is the registration number of the vehicle transporting the waste Signature, Date & Time This is the driver s signature for the transportation of the waste and the date & time of the collection. PART D Consignor s Certificate This section is to be completed by those that own the waste (the Store). 1. Consignor Name This is the name of the person from the store present at the removal of the waste. On behalf of This is the name of the store Signature, Date & Time This is the signature of named Consignor (#1) and the date & time to confirm that the check of the form has been completed. Note. This should NOT be before the date and time provided in Part C PART E Consignee s Certificate This section is completed by those receiving the waste once the transportation has taken place and would not be completed by Integral.

5 What does a completed form look like?

6 Who receives each copy of the Hazardous Waste Consignment Note? CLIENT COPY (White) Is given to the person at store once section D is completed WASTE DESTINATION COPY (Yellow) Is given to the carrier transporting the waste once section D has been completed, which is then given to the receiver of the waste. INTEGRAL OFFICE COPY (Red) Is sent to the Integral Leicester office once section D has been completed for the attention of Customer Reporting. The Leicester office address is: Integral UK LTD, 1 & 2 Geoff Monk Way, Birstall, LE4 3BU. Note. The Leicester Integral Office will chase the receiver of the waste for a fully copy of the completed Hazardous Waste Consignment Note completed (to include section E), once the red copy has been received from the engineer. Is anything else required by the Leicester Integral Office? Please also add the Integral job reference in the top right hand corner of the Hazardous Waste Consignment Note before posting to assist with waste traceability.
